BACKGROUND: Older adults are particularly vulnerable to complications from proton pump inhibitor (PPI) drugs. We sought to characterize the prevalence of potentially low-value PPI prescriptions among older adults to inform a quality improvement (QI) intervention.
METHODS: We created a cohort of patients, aged 65 years or older, receiving primary care at a large academic health system in 2018. We identified patients currently prescribed any PPI using the electronic health record (EHR) medication list (current defined as September 1, 2018). A geriatrician, a gastroenterologist, a QI expert, and

Capsule endoscopy (CE) was launched at the beginning of this millennium and has since become a well-established tool for evaluating the enti … phy, magnetic resonance enteroclysis, and push enteroscopy, for diagnosing small bowel pathologies. Gastroenterologists would prefer the convenience of a single capsule that can create images of the area from the oral cavity to the anal canal in one 'shot'. Because of anatomic and physiologic differences in the GI tract, however, it may not be possible to use the sam …

Magnetic resonance cholangiopancreatography (MRCP) represents a new development in MR technology that provides a noninvasive accurate means of evaluating the biliary tree and pancreatic duct. Recent technical refinements that allow for imaging of the entire biliary tree and pancreatic duct in 18 seconds make this examination easily performed even in critically ill patients. The clinical applications of MRCP are illustrated in a variety of scenarios that include choledocholithiasis, malignant obstruction, incomplete/failed endoscopic retrograde cholangiopancreatographies (ERCPs), postsurgical a

Some epidemiological data point to an association between infection from Helicobacter pylori (Hp) and gastric cancer, although several unresolved issues still cast doubts on the real weight of this association. These issues are as follows: the male-to-female ratio of gastric cancer ranges from 4:1 to 1.5:1 in all studies, whereas the prevalence of Hp infection is the same in both sexes; the prevalence of Hp infection is as high as 90% in several developing countries where the frequency of gastric cancer is very low; the acquisition of the infection at a young age, considered very important wit

Esophageal dilation, a technique developed four centuries ago, continues to be an important method of treating the symptom of dysphagia in patients with luminal narrowing of the esophagus. Dilation is safe, with < 0.5% chance of perforation and bleeding and a 0.01% risk of death. Mercury bougienage (Maloney dilators), hollow polyvinyl dilators (Savary-Guillard), and balloon dilators (Through the Scope) are the principal types of dilators in use today. Few trials have compared the differing dilating methods. The results of these trials are mixed, and further randomized trials are necessary to d

Barrett's esophagus is the premalignant lesion of adenocarcinoma of the esophagus and gastric cardia, two rapidly increasing incidence cancers. Barrett's esophagus has been documented to be reversed with a combination of pharmacologic acid control or antireflux surgery and endoscopic ablative therapy. A variety of endoscopic techniques can be applied to ablate the Barrett's epithelium. With sufficient acid reduction, squamous repopulation of the injured area occurs. This injury can be accomplished using laser, photodynamic therapy, electrocautery, and mechanical techniques. The relative succes

The indications for surgical intervention in chronic pancreatitis are suspicion of malignancy, local complications, and intractable pain. Chronic pancreatitis is a risk factor for development of pancreatic carcinoma, and carcinomas may present, initially with a clinical picture of chronic pancreatitis. Local complications of chronic pancreatitis such as common bile duct or duodenal obstruction and enlarging or symptomatic pseudocyst also mandate surgical intervention. Thrombosis of the splenic vein with left-sided portal hypertension is common and associated with a 10% incidence of gastric var

Cholangiocarcinoma is associated with several etiological factors including cystic dilation of the bile duct, clonorchiasis, hepatolithiasis, and sclerosing cholangitis. Jaundice is the presenting symptom in over 90% of patients who present with cholangiocarcinoma. The disease is suspected on the basis of an abnormal ultrasound or computed tomography (CT). Tumor markers are often normal, but a significant elevation of carcinoembryonic antigen or CA 19-9 should alert the clinician of a possible diagnosis of cholangiocarcinoma. Cholangiogram is essential to dileneate the bile duct anatomy in hil

Because of their high prevalence in clinical practice, the field of gastrointestinal motility has tended to focus its clinical and research efforts on such functional disorders as nonulcer dyspepsia, the irritable bowel syndrome, and functional constipation. Because these disorders are difficult to define and their diagnosis remains exclusively symptomatic, progress has been difficult in these areas, and advances in clinical gastrointestinal motility generally have been hampered. This review attempts to emphasize the prevalence and importance of "organic" motility disorders, ie, those disorder

Total parenteral nutrition (TPN) has become an important tool in the management of acutely ill patients and in patients unable to tolerate oral nutrition. Although the benefits of this treatment are obvious, there are various hepatobiliary complications associated with use of TPN. We review the spectrum of liver and biliary abnormalities associated with use of TPN, as well as discuss the management of these complications in both adults and infants.

Short-bowel syndrome refers to the clinical sequelae of resection of a substantial portion of the small intestine. Features of diarrhea, dehydration, malabsorption, malnutrition, or weight loss generally occur, at least transiently, when more than 75% of the small intestine is resected, although other factors in addition to residual length affect clinical outcome. Adaptation of the remnant bowel has been well documented to occur over months to years following resection, although the agents responsible for this process are not well defined. Ambulatory total parenteral nutrition has greatly impr

Recent evidence suggests that a significant proportion of primary gastrointestinal lymphomas are driven by exogenous agents/antigens. In the stomach, Helicobacter pylori appears to be responsible for most cases of low-grade lymphomas (MALToma), whereas an infectious etiology is suspected in immunoproliferative small intestine disease (IPSID). Similarly, enteropathy-associated T-cell lymphomas appear to result from a disordered response to gluten, although this profile remains controversial. Accordingly, although traditional antineoplastic treatments, such as surgery and radiation, are still im

Routine vitamin supplementation for the elderly has been advocated by many. Specific vitamin deficiencies are rare in free-living elderly, but are not uncommonly encountered in hospitalized and institutionalized patients. Deficiency may result from interactions with medications or overall poor dietary intake. Low blood or plasma vitamin concentration is not necessarily indicative of a deficient state. Specific vitamin supplements are useful in the treatment and prevention of a deficient state. However, there is little, if any benefit from supplementation for reasons other than replacement ther

The etiology and the pathogenesis of different forms of hepatotoxicity are discussed; case reports are included to illustrate the importance of history-taking and examination of liver tissue in establishing a specific diagnosis. The role of alcohol as a hepatotoxin as well as an enzyme-inducing agent is stressed. Genetic factors have been identified that may determine susceptibility to alcoholism and the hepatotoxic effects of alcohol and other compounds. Some cases of drug-induced cholestasis may be explained by disturbances in the known pathways of bile acid uptake, transport, and excretion.

All of vitamin B12 in nature is of microbial origin. Cobalamin, as vitamin B12 should correctly be termed, is a large polar molecule that must be bound to specialized transport proteins to gain entry into cells. Entry from the lumen of the intestine under physiological conditions occurs only in the ileum and only when bound to intrinsic factor. It is transported into all other cells only when bound to another transport protein, transcobalamin II. Congenital absence or defective synthesis of intrinsic factor or transcobalamin II result in megaloblastic anemia. The Immerslund-Graesbeck syndrome,

Laparoscopic technique has become the norm for cholecystectomy, one of the most commonly performed operations. Unfortunately, the advent of laparoscopic cholecystectomy has been accompanied by a significant increase in bile duct injuries and a new set of postsurgical problems when compared with traditional, open cholecystectomy. Endoscopic retrograde cholangiopancreatography (ERCP) has emerged as a central method for diagnosis and treatment of bile duct injuries and other problems arising after laparoscopic cholecystectomy. The most common biliary problems include bile leaks from the cystic du

Mesenteric venous thrombosis (MVT) and its clinical expressions have become better defined as a result of improvements in both diagnostic imaging and our understanding of the various intestinal ischemic syndromes. Acute, subacute, and chronic forms of MVT are currently recognized, and these forms may differ in the symptoms they produce, the methods by which they are diagnosed, and the treatments they may require. Moreover, an underlying disease that predisposes a patient to MVT can be found in most instances.

Constipation is a symptom but can generally be defined as less than three bowel movements per week. The history and physical examination should be evaluated for stool size, frequency, and straining and discomfort on defecation. The influence of age, gender, and society should also be considered. The etiologies of constipation can be classified as 1. dietary; 2. drug induced; 3. metabolic; 4. neurologic; or 5. anatomic. If hard or small stools are part of the initial evaluation, then a dietary approach of increased dietary fiber intake can be used as a therapeutic trial. If it does not succeed

In the second article of this series, we discuss the diagnosis and treatment of some important intestinal nematodes. Common parasites discussed in detail include Ascaris lumbricoides, Trichuris trichiura, hookworm, and Enterobius vermicularis (pinworm). In addition, Strongyloides stercoralis has emerged as an important pathogen in immunocompromised patients, and Capillaria philippinensis is now recognized as an important cause of diarrhea in Asia. Finally, anisakiasis and other illnesses due to parasitic nematodes are discussed.
