Injury and recovery patterns of the blood-testis barrier and spermatogenic function in a surgical unilateral cryptorchidism model in rats.

BACKGROUND: Cryptorchidism is one of the most common congenital disorders of the pediatric urogenital system. In addition to regulating the passage of substances into and out of the seminiferous epithelium, the blood-testis barrier (BTB) plays a critical role in maintaining a stable microenvironment for spermatogenesis. BTB alterations in cryptorchidism remain poorly characterized. This study aimed to establish a surgical unilateral cryptorchidism model in rats (SUCMR) and to investigate the injury and recovery patterns of both the BTB and spermatogenic function within this model. METHODS: Th

Abnormal sexual differentiation and neoplasia.

The prevalence of neoplasia is increased in individuals with certain disorders of sexual differentiation. Etiology and frequency of neoplasia vary with the particular disorder. In uncomplicated cryptorchidism, the testis is at least 10 times more likely to undergo neoplastic transformation than a normal scrotal testis. Neoplasia probably is a function of both testicular location (intraabdominal) and underlying dysgenetic structure. If cryptorchidism is unilateral, and if orchiopexy has not been performed prior to age 6-10 years, orchiectomy should be encouraged. In those forms of gonadal dysge

Shortcomings in the management of undescended testis: guideline intention vs reality and the underlying causes - insights from the biggest German cohort.

OBJECTIVES: To assess the implementation of the current guideline and identify potential underlying causes for late surgery in children with undescended testis (UDT) in Germany. UDT is the most common surgical issue in paediatric urology and to avoid malignant degeneration and subfertility current guidelines recommend orchidopexy during the first year of life; however, this seems not to be implemented in practice. PATIENTS AND METHODS: In all, 5 547 patients with cryptorchidism at 16 hospitals nationwide were studied regarding age at orchidopexy between 2003 and 2016. Multivariate analysis wa

Decision making among different treatment options for neurologically impaired boys with undescended testis: a multinational pediatric survey.

OBJECTIVE: To determine the attitude of referring pediatricians towards the decision of treatment modalities for undescended testis (UDT) in neurologic impaired boys (NIB). METHODS AND MATERIALS: An online questionnaire was offered to registered pediatricians in Austria and Germany for online completion. RESULTS: 221 male (61.6%) and 138 female (38.4%) pediatricians completed the survey; 326 (90.8%) believe that UDT should be treated according to national guidelines; 31 (8.6%) believe that UDT should be treated according to the parental wish, whereas only 2 (0.6%) tend to no treatment at all

Orchidopexy in children with Prader-Willi syndrome: Results of a long-term follow-up study.

INTRODUCTION: Prader-Willi syndrome (PWS) is a rare (1:20.000) genetic condition affecting both males and females. Among other features, in boys, the syndrome is characterized by cryptorchidism in 86-100% of cases, hypogonadism, delayed puberty and infertility. The aim of the present study is to appraise the results of orchidopexy in this selected population of children. STUDY DESIGN: A follow-up study of children with PWS treated for undescended testes at a single institution over a 20-year period was performed. Patients were identified from a National PWS registry and reviewed at a special

Journal of Pediatric Surgery-Sponsored Fred McLoed Lecture. Undescended testis: the underlying mechanisms and the effects on germ cells that cause infertility and cancer.

Testicular descent is a complex morphological process that occurs in at least 2 stages, with different hormonal control. Insl3 controls the first step of gubernacular enlargement, although the abnormality long gubernacular cord in persistent Műllerian duct syndrome remains unexplained. Androgens control inguinoscrotal migration, which may be triggered by local signalling from the mammary line, and which requires the genitofemoral nerve. However, there is still much to learn about this phase, which when abnormal frequently leads to cryptorchidism. Orchidopexy is being recommended in the first y

Impact of anatomical and socioeconomic factors on timing of urological consultation for boys with cryptorchidism.

PURPOSE: Cryptorchidism is a common finding in infants and young boys. Early repair lessens the extent of testicular injury. We hypothesized that anatomical and socioeconomic factors affect the timing of consultation and treatment for boys with cryptorchidism. MATERIALS AND METHODS: Under an institutional review board approved protocol we reviewed the records at a single institution of children who underwent exploration for unilateral or bilateral cryptorchidism. Demographic and anatomical factors were recorded. RESULTS: The median age of 677 boys at consultation and surgery was 20.3 and 28.

Inguinal approach for the management of unilateral non-palpable testis: is diagnostic laparoscopy necessary?

OBJECTIVE: During the last decade laparoscopy has increasingly been advocated as the primary investigative procedure for the management of the non-palpable testis. We reviewed the medical records in a consecutive series of boys with non-palpable testis to examine the contribution of the initial inguinal approach in the management of unilateral non-palpable testis. MATERIALS AND METHODS: Among the 183 consecutive patients with cryptorchidism from 2003 to 2012, there were 21 patients with unilateral and three with bilateral non-palpable testes. All unilateral patients then underwent inguinal an

A preliminary study on predicting the prognosis of testicular torsion based on indocyanine green-guided near-infrared fluorescence imaging.

BACKGROUND: Testicular viability is the primary basis for selecting the surgical approach during orchiopexy for testicular torsion (TT); however, there is currently a lack of methods that can objectively assess testicular viability intraoperatively. The purpose of this study is to evaluate testicular viability and predict postoperative testicular outcomes using indocyanine green-guided near-infrared fluorescence (ICG-NIRF) imaging during surgery for TT. METHODS: We retrospectively reviewed pediatric patients treated for TT at our hospital between January 2024 and December 2024. Intraoperative

Adenomatoid tumor of the testis in a patient with history of cryptorchidism, male breast cancer, and thymic B2 thymoma: a case report and literature review.

BACKGROUND: Adenomatoid tumor is a rare benign neoplasm of mesothelial origin, most commonly arising in the epididymis, vas deferens, and tunica vaginalis; primary occurrence in the testis is extremely rare. Cryptorchidism is recognized as a risk factor for testicular germ cell tumors, but its association with adenomatoid tumors remains hypothetical, as there is limited mechanistic evidence directly linking cryptorchidism to mesothelial tumorigenesis in the testis. Moreover, multiple primary malignancies may be related to impaired immune surveillance or genetic susceptibility, though these mec

Neonatal unilateral testicular torsion in the first 16 hours of life: A case report and literature review.

RATIONALE: Neonatal testicular torsion (NTT) is an uncommon but urgent urological condition that predominantly occurs in the prenatal or immediate postnatal period (up to 28 days of life). Diagnosis is often delayed due to subtle clinical signs and the inability of neonates to express discomfort. Early recognition is critical to prevent irreversible ischemic damage and to preserve future endocrine and reproductive function. This case emphasizes the importance of routine detailed neonatal physical examination, even when infants are hospitalized for unrelated conditions or otherwise normal. PAT

Stepwise mini-incision microdissection testicular sperm extraction in NOA patients with a history of cryptorchidism: a case-control study.

BACKGROUND: Although the orchiopexy is recommended for cryptorchidism to preserve male fertility, non-obstructive azoospermia (NOA) may occur in adulthood. Fortunately, a great many of azoospermic men may obtain sperm by microdissection testicular sperm extraction (mTESE). Due to the potential injuries caused by testicular diagnostic biopsy and vascular damage at the time of orchidopexy, minimal invasiveness is particularly important during mTESE, aims to reduce the surgical damage and avoids secondary testicular failure. This comparative study aims to investigate the efficacy of stepwise mini

Post-operative complications following masculinizing genitoplasty in moderate to severe genital atypia: results from a multicenter, observational prospective cohort study.

INTRODUCTION: Differences of sex development (DSD) are congenital conditions in which there is atypical chromosomal, gonadal and/or phenotypic sex. A phenotype of severe genital atypia in patients raised as male is a relatively rare occurrence and standards for management are lacking. Decision making for early surgical planning in these rare cases includes, but is not limited to, degree of atypia, location of testes, and presence of Mϋllerian remnants. In this study we describe surgical approaches and short-term outcomes for masculinizing genitoplasty in moderate to severe genital atypia in yo

Avoidance of inguinal incision in laparoscopically confirmed vanishing testis syndrome.

PURPOSE: Nonpalpable testicles may be due to the vanishing testis syndrome, intra-abdominal position, examination obscured by obesity or scar tissue and rarely testicular agenesis. Laparoscopy is an excellent means of distinguishing these entities without the need for open abdominal exploration. We investigated whether laparoscopy affects the need for an inguinal incision and exploration when no testicle is palpable and the vas and vas deferens are visualized exiting the internal inguinal ring on laparoscopy. MATERIALS AND METHODS: In 34 boys 6 to 18 months old (mean age 41) physical examinat

No Detectable Association Between Virtual Setting for Vasectomy Consultation and Vasectomy Completion Rate.

INTRODUCTION: The COVID-19 pandemic has fueled widespread incorporation of telehealth into urology practices. Vasectomy consultation via telehealth is convenient and improves access to care for male contraception. However, it does not allow for physical examination, inherently leading to possible day-of-procedure cancellations due to unforeseen anatomic concerns. This study aimed to compare vasectomy completion rates between patients undergoing virtual vs in-person consultation. METHODS: All patients seen by a single provider at NYU Langone Health for vasectomy consultation between October 20

Torsion of the cryptorchid testis--can it be salvaged?

PURPOSE: The literature concerning undescended testis mainly concentrates on the increased risks of infertility and development of germ cell tumors. Yet the UDT also appears to be at higher risk for torsion compared to the normally descended testis, and this issue is relatively poorly addressed. We reviewed all cases of torsion of UDTs operated on at our hospital during the last 20 years in an attempt to characterize better this condition and its salvageability. MATERIALS AND METHODS: In this retrospective clinical study we reviewed and analyzed all cases of testicular torsion involving UDT o

Long-term impact of commonly performed operations in pediatric urology on reproductive and sexual health.

INTRODUCTION: Sexual dysfunction is highly prevalent among men of reproductive age. Clinical practice guidelines have been established to assist providers in identification and education of patients who are at increased risk for infertility and sexual dysfunction with certain congenital and acquired urogenital disorders. The authors sought to review the reproductive and sexual health implications of treating common childhood urological conditions with commonly performed surgical procedures. METHODS: To ensure the inclusion of influential and highly regarded research, we prioritized citations

Mesenchymal stem cells restore the sperm motility from testicular torsion-detorsion injury by regulation of glucose metabolism in sperm.

BACKGROUND: Testicular torsion is an urological emergency that may lead to infertility due to ischemic injury. Promptly surgical correction by orchiopexy is the only way to avoid infertility and no effective treatment for restoration of spermatogenesis. We previously reported that mesenchymal stem cells (MSCs), through local injection upon testicular torsion-detorsion, restored the spermatogenesis without differentiation into sperm. In this study, molecular mechanisms of MSCs in regulating germ cell activity induced by testicular torsion-detorsion were investigated. METHODS: Sixteen male Spra

Local injection of mesenchymal stem cells protects testicular torsion-induced germ cell injury.

INTRODUCTION: Testicular torsion is a urological emergency and infertility is a common complication due to ischemic injury. Surgical reduction and orchiopexy is indicated, but to date there is no effective method for restoration of spermatogenesis. The effects of mesenchymal stem cells (MSCs) on acute tissue injury have been demonstrated, and the abilities of paracrine support, differentiation and immune-modulation may benefit to testicular torsion-induced infertility. We investigate the therapeutic efficacy and the mechanisms of MSCs in testicular torsion-induced germ cell injury when injecte
