Severance of the ureter beyond the renal papilla causes a fall in urinary osmolality, which suggests that exchange of water or solute between urine and renal parenchyma normally occurs in the intact renal pelvis. Thirty-two cases of hydronephrosis caused by pelvi-ureteric junction obstruction, presenting to the Department of Paediatric Surgery in the last 6 years were reviewed. In both infants and older children a palpable abdominal mass was the most common mode of presentation. Anderson-Hynes pyeloplasty was performed in a total of 31 kidneys. The renal pelvic pressure/volume relationship was investigated in 8 pigs with a 10-week total unilateral obstruction. The pressure in the totally obstructed pelves was significantly increased to a mean value of 44 cm H2O (30.5-64). After emptying, the renal pelves were refilled continuously at a rate of 8 cc per minute to a maximal pressure of 80 cm H2O. We report on a patient who had had recurrent renal stones on the right side for 37 years. At the sixth lithotomy the diagnosis of cholesteatoma of the renal pelvis was discussed. In a case-control investigation of 154 women with cancer of the bladder and 440 female population controls spanning the same age range, the relative risk for this cancer was 2.6 in consumers of phenacetin-containing analgesics and 2.7 in tobacco smokers. The relative risks for cancer of the renal pelvis, determined for 31 cases, were 5.4 with phenacetin and 4.7 with tobacco. Thirty-five patients with transitional cell carcinoma of the renal pelvis or ureter of all stages and grades were studied for presence or absence of ABO(H) antigens utilizing an improved technique for staining and preserving the slides. Seventy per cent of the grade I tumors retained their antigens. Patients with antigen present had a longer duration of disease-free interval. Specific red cell adherence (SRCA) may predict the clinical course of patients with low-stage, low-grade transitional cell carcinomas and may be helpful in selecting patients for optimal therapy.

Since untreated intra-abdominal abscesses are associated with a high mortality rate, early diagnosis is important. In abscesses involving gas formation, plain radiographs of the abdomen in various projections are sufficient for diagnosis. In this article, differential diagnosis of cavities containing gas and fluids is discussed.

Computerized tomography has been used in the evaluation of 5 patients with radiolucent masses of the collecting system. There were 3 cases of renal calculi. 1 peripelvic cyst and 1 squamous cell carcinoma of the renal pelvis. The results of computerized tomography scanning are correlated with the results of excretory urography and ultrasonographic scanning. The diagnostic advantage of computerized tomography scanning for evaluation of radiolucent masses of the upper collecting system is elucidated.

Primary adenocarcinoma arising in the renal pelvis is a rare entity. Two cases are presented in which computed tomography (CT), ultrasound, and antegrade pyelography were performed in addition to conventional urographic methods. The percutaneous approach to the renal pelvis provides access for antegrade urography, percutaneous nephrostomy, or endourologic manipulation. Having performed 31 successful procedures, including the percutaneous placement of double-pigtail ureteral stents for ureterovesical obstruction, we advocate the combined use of ultrasonography and fluoroscopy to achieve optimal results.

Acetylcholine failed to change the frequency of spontaneous contractions in the proximal region of isolated rabbit renal pelvis, but significantly increased the contractile frequency in the middle and distal regions, which then reached similar levels to those of the proximal region. The characteristics of spontaneous contractility in the renal pelvis were examined in the unicalyceal kidney of the rabbit. In vitro isometric studies were done on whole as well as renal pelvic segments dissected longitudinally and radially. The results show that the contractile frequency of isolated segments was highest in the proximal region (4.188 +/- 0.162 min-1) and decreased distally to (2.178 +/- 0.330 min-1). Using a bipolar macroelectrode, simultaneous recordings of both pelvic and ureteral electromyograms (EMG) were carried out in dogs. The recordings were performed at 3 stages of urine flow; oliguric, increasing and diuretic stages. The results showed that the interval of the pelvic discharge was constant while its speed of propagation was slower than that of the ureter (1.32 cm/sec and 2.12 cm/sec, respectively).
