Spatial clustering of occupational injuries in communities.

OBJECTIVES: Using the social-ecological model, we hypothesized that the home residences of injured workers would be clustered predictably and geographically. METHODS: We linked health care and publicly available datasets by home zip code for traumatically injured workers in Illinois from 2000 to 2009. We calculated numbers and rates of injuries, determined the spatial relationships, and developed 3 models. RESULTS: Among the 23 200 occupational injuries, 80% of cases were located in 20% of zip codes and clustered in 10 locations. After component analysis, numbers and clusters of injuries cor

Does Neighborhood Disorder Predict Recovery From Mobility Limitation? Findings From the Health and Retirement Study.

OBJECTIVE: This research explores whether perceptions of physical neighborhood disorder predict recovery from mobility limitation over a 2-year period and examines whether psychosocial factors (i.e., depressive symptomology and mastery) and physical activity are salient mediators. METHOD: Using data from the Health and Retirement Study (HRS): Participant Lifestyle Questionnaire (2008-2010), odds ratio estimates of recovery were ascertained using binary logistic regression, and post hoc Sobel tests were conducted to formally assess mediation. RESULTS: Net of demographic characteristics and so

Worldwide Prevalence and Trends in Unintentional Drug Overdose: A Systematic Review of the Literature.

BACKGROUND: Drug overdose is an important, yet an inadequately understood, public health problem. Global attention to unintentional drug overdose has been limited by comparison with the scope of the problem. There has been a substantial increase in drug overdose incidence and prevalence in several countries worldwide over the past decade, contributing to both increased costs and mortality. OBJECTIVES: The aim of this study was to systematically synthesize the peer-reviewed literature to document the global epidemiological profile of unintentional drug overdoses and the prevalence, time trends

Quantifying Poverty as a Driver of Ebola Transmission.

BACKGROUND: Poverty has been implicated as a challenge in the control of the current Ebola outbreak in West Africa. Although disparities between affected countries have been appreciated, disparities within West African countries have not been investigated as drivers of Ebola transmission. To quantify the role that poverty plays in the transmission of Ebola, we analyzed heterogeneity of Ebola incidence and transmission factors among over 300 communities, categorized by socioeconomic status (SES), within Montserrado County, Liberia. METHODOLOGY/PRINCIPAL FINDINGS: We evaluated 4,437 Ebola cases

Interactive effects of individual and neighborhood race and ethnicity on rates of high-grade cervical lesions.

We estimated the main and interactive effects of individual race/ethnicity (black, Hispanic, white) and area race, ethnicity, and poverty (proportions of the female population black, Hispanic, and living below the federal poverty level at the census tract level, respectively) on rates of high-grade cervical lesions among young women. Using data from a statewide surveillance system during 2008-2011, we found a marginally significant interaction (P<0.05) between individual race/ethnicity and area race, with black and Hispanic women living in areas with ≥20% of the female population black having

Atlas of the clinical genetics of human dilated cardiomyopathy.

AIM: Numerous genes are known to cause dilated cardiomyopathy (DCM). However, until now technological limitations have hindered elucidation of the contribution of all clinically relevant disease genes to DCM phenotypes in larger cohorts. We now utilized next-generation sequencing to overcome these limitations and screened all DCM disease genes in a large cohort. METHODS AND RESULTS: In this multi-centre, multi-national study, we have enrolled 639 patients with sporadic or familial DCM. To all samples, we applied a standardized protocol for ultra-high coverage next-generation sequencing of 84

The effects of a therapeutic yoga program on postural control, mobility, and gait speed in community-dwelling older adults.

OBJECTIVE: To examine the effects of a 12-week therapeutic yoga program on gait speed, postural control, and mobility in community-dwelling older adults. DESIGN: Quasi-experimental study with a pretest/post-test design. Researchers evaluated changes over time (pretest to post-test) in all outcome measures. Paired t-tests were used to analyze normal and fast gait speed, Timed Up and Go test, and Timed Up and Go Dual Task. Wilcoxon signed-rank test was used to evaluate scores for the Mini-BESTest (MBT). SETTING: Yoga classes were performed at a local senior center. Blind examiners who were pre

Towards people-centred health systems: a multi-level framework for analysing primary health care governance in low- and middle-income countries.

Although there is evidence that non-government health system actors can individually or collectively develop practical strategies to address primary health care (PHC) challenges in the community, existing frameworks for analysing health system governance largely focus on the role of governments, and do not sufficiently account for the broad range of contribution to PHC governance. This is important because of the tendency for weak governments in low- and middle-income countries (LMICs). We present a multi-level governance framework for use as a thinking guide in analysing PHC governance in LMI

Transportation noise and annoyance related to road traffic in the French RECORD study.

Road traffic and related noise is a major source of annoyance and impairment to health in urban areas. Many areas exposed to road traffic noise are also exposed to rail and air traffic noise. The resulting annoyance may depend on individual/neighborhood socio-demographic factors. Nevertheless, few studies have taken into account the confounding or modifying factors in the relationship between transportation noise and annoyance due to road traffic. In this study, we address these issues by combining Geographic Information Systems and epidemiologic methods. Street network buffers with a radius o

Parks as a tool for HIV management.

Access to parks improves overall health outcomes in the general population. Given that HIV infection has become a chronic disease to manage, among populations engaged in medical care, parks may be promoted as physical activity opportunities in order to manage chronic comorbid conditions. We conducted a cross-sectional examination of the relationships between sociodemographic and biomedical characteristics to park proximity among 635 individuals receiving outpatient HIV care. The data collected included HIV-related biomarkers, depression, and diagnoses of other chronic diseases. The total acres

Residential pesticide use is associated with children's respiratory symptoms.

OBJECTIVE: To characterize the association between residential pesticide application and acute and chronic respiratory symptoms in children, focusing on the location of the pesticide application in and around the home and the type of pesticide applicator (professional vs nonprofessional). METHODS: We used 1999 to 2004 National Health and Nutrition Examination Survey results to investigate this association in children younger than 18 years (N = 14,065). RESULTS: Overall, pesticide use in the home was not associated with wheezing (odds ratio [OR], 1.15; 95% confidence interval [CI], 0.91 to 1.

Neighborhood socioeconomic conditions are associated with psychosocial functioning in older black and white adults.

We examined neighborhood socioeconomic status (NSES) in relation to depressive symptoms, perceived stress, and hostility in 5770 community-dwelling older black and white adults (mean age=73 years; 62% female) from 3 contiguous neighborhoods covering 82 census block groups in Chicago, IL. NSES was an average of z-scores of four Census 2000 block-group variables: % public assistance, % households earning <$25,000 annually, % with >college degree, and % owner-occupied dwellings valued >$200,000. NSES was inversely related to hostility (beta=-0.305), stress (beta=-0.333), and depressive symptoms (

The impact of depression, seizure variables and locus of control on health related quality of life in a community dwelling sample of older adults.

Few studies have examined the impact of epilepsy on the quality of life of older people, although epilepsy is one of the most common neurological disorders of old age. This study investigated the association of depression, seizure type and frequency and locus of control on health related quality of life in community dwelling adults aged over 60 years. Sixty-four participants were administered a clinical diagnostic interview to assess depression and dysthymia, and completed measures of HRQOL (QOLIE-31), locus of control and provided information on seizure variables. Depression, dysthymia and mo

Restricted randomization of ZAMSTAR: a 2 x 2 factorial cluster randomized trial.

BACKGROUND: A small number of clusters and substantial variation between clusters increase the chance of unbalanced randomization in cluster randomized trials. Baseline imbalances between groups may distort intervention effects. When adjusting for imbalances in the cluster-level analysis, this results in loss of degrees of freedom. Variance reduction that can be achieved through stratification and blocking is limited. Restricted randomization is an alternative approach that ensures balanced allocation. PURPOSE: We present the randomization scheme used in the ZAMSTAR trial of tuberculosis cont

The problems of relative deprivation: why some societies do better than others.

In this paper, we present evidence which suggests that key processes of social status differentiation, affecting health and numerous other social outcomes, take place at the societal level. Understanding them seems likely to involve analyses and comparisons of whole societies. Using income inequality as an indicator and determinant of the scale of socioeconomic stratification in a society, we show that many problems associated with relative deprivation are more prevalent in more unequal societies. We summarise previously published evidence suggesting that this may be true of morbidity and mort

The Social Phobia Inventory: validity and reliability in an adolescent community sample.

Social anxiety disorder and social anxiety are highly prevalent psychological conditions in youth. Because of the known risks associated with the spectrum of social anxiety in youth, early detection with valid and reliable assessment measures is imperative. Self-report measures have become the most widely used method of assessment for child and adolescent social anxiety. Because research to date on self-report measures of social anxiety disorder in adolescents is limited, the primary objective of the present study was to contribute to the literature by examining the validity and reliability of

Review: the wider social environment and schizophrenia.

Rates of schizophrenia differ significantly between groups defined at the social level, eg, urban/rural comparisons, neighborhoods, and ethnic minority status. While earlier studies were not able to determine if the social environment influenced the development of schizophrenia (causation) or whether individuals at risk aggregated in adverse social environments (selection), the recent development of multilevel modeling should inform this debate. To date, there are few examples of multilevel analyses in schizophrenia research; however, the small number of studies suggest that there may be a nei
