HIV and Syphilis Coinfection in Pregnancy and Adverse Birth Outcomes in Uganda.

BACKGROUND: Despite increasing global syphilis incidence, little is known about the combined impact of maternal HIV and syphilis coinfection. We evaluated effects of HIV/syphilis coinfection in pregnancy on birth outcomes. METHODS: We conducted 2 prospective birth cohort studies in Uganda from 2017 to 2023. Our primary outcome was birthweight. Our secondary outcome was a composite adverse birth outcome (birthweight <2.5 kg, stillbirth, early neonatal death, or 5-min APGAR<7). We compared outcomes by HIV and Treponema pallidum particle agglutination assay (TPPA) seropositivity and fitted multi…

Gastric Microbiota in Helicobacter pylori-Negative and -Positive Gastritis Among High Incidence of Gastric Cancer Area.

Helicobacter pylori (H. pylori) related chronic gastritis is a well-known major etiological factor for gastric cancer development. However, … a significantly increased risk for HpN (odds ratio 18.9 and 12.3, respectively) based on abundance. Treponema sp. was uniquely found in HpN based on occurrence. In this paper, we conclude that Streptococcus sp., Haemophilus parainfluenzae and Treponema sp. are candidate pathogenic bacterial species for HpN. These results if confirmed may have important clin …

A Case Report of Oral Syphilis Before and After Management.

Syphilis is a sexually transmitted infection caused by the spirochete bacterium Treponema pallidum, presenting with oral manifestations at various stages of the disease, including primary, secondary, and tertiary phases. Oral manifestations tend to be misdiagnosed as other lesions; therefore, the diagnosis and treatment may be delayed. This case reports the diagnosis and treatment of a 27-year-old female patient who presented to the Oral Medicine clinic with oral syphilis, emphasizing the clinical presentation before and after treatment.

Syphilitic Chancre of the Lip.

Syphilis is a sexually transmitted disease caused by Treponema pallidum. The primary stage of the disease (the chancre) mainly involves the genital areas. Extragenital areas are involved in 5%-14% of cases, many of which occur in the oral cavity. Among the extragenital locations, the lip is the most frequent; however, despite this, published cases of the chancre of the lip are few. We present a case of a chancre presenting on the lip of a nonimmunocompromised 55-year-old male patient with immunohistochemical confirmation.

Vesiculobullous syphilis: a case involving an unusual cutaneous manifestation of secondary syphilis.

The recent resurgence of syphilis mandates that clinicians maintain a heightened suspicion for Treponema infection, and that they be aware of the variety of cutaneous presentations that may mimic eczema, psoriasis, drug eruption, erythema multiforme, lichen planus, tinea versicolor, seborrheic dermatitis, mycosis fungoides, or other lichenoid lesions. In this report, we describe an unusual case of secondary syphilis in an adult woman, and briefly review the wide array of syphilitic dermopathy that could present to the foot and ankle surgeon.

Therapy of other bacterial infections.

Cutaneous bacterial disease continues to account for a significant proportion of clinical visits. The continuing emergence of strains that are resistant to available antibacterial agents creates a challenge for dermatologists, who need to keep abreast of current treatment strategies. In this article, antibacterial regimens are presented for skin infections caused by organisms such as Staphylococcus, Streptococcus, Pseudomonas, Neisseria, Haemophilus ducreyi, Treponema pallidum, Bacillus anthracis, Yersinia pestis, Pasteurella multocida, Vibrio vulnificus, Actinomyces, and Nocardia.

[Parenchymal neurosyphilis. Insidious onset (dementia) and acute onset (manic type) forms].

The increased rates in recent years of primary and secondary syphilis, with a renovated interest of central nervous system infection by the treponema pallidum, justify the presentation of two cases of classic General Paresis: Dementia and Mania. Both patients fulfilled sufficient criteria as to elaborate a process of differential diagnostic of physical illness in patients with mental disorders, which include a battery laboratory tests. The positive response to a protocol with penicillin improved both the clinical symptoms and the laboratory tests.

The many faces and phases of borreliosis. I. Lyme disease.

Lyme disease is increasingly being reported throughout the United States and many parts of the world. Borrelia burgdorferi, the etiologic agent of Lyme disease, is a spirochete that, not unlike the treponema of syphilis, can cause a spectrum of disease from the initial skin lesion, through widely varied symptoms and signs, to chronic neurologic and arthritic disability. The borrelial spirochete and Lyme disease are the subject of this review. A subsequent article will review other definite and possible cutaneous manifestations of borreliosis.

Syphilis.

Inoculation of Treponema pallidum during intimate physical contact is followed by development of a papule that rapidly erodes, yielding the syphilitic chancre. Hematogenous dissemination results in secondary syphilis, a multisystem disease characterized by skin rash, lymph node enlargement, hepatitis, arthritis, and central nervous system involvement. In the absence of treatment, a latent period is reached after which tertiary syphilis may appear in an increasing proportion of patients, depending upon the duration of the follow-up.

Reinfection primary, secondary, and latent syphilis: the serologic response after treatment.

An infection with Treponema pallidum does not confer solid immunity to reinfection. Consequently, it is not unusual for the physician to see a patient with reinfection syphilis. In this study there were 36 patients who, after having an initial infection with T. pallidum, became reinfected with the primary, secondary, or latent stage of syphilis. The titers of antibody to T. pallidum during reinfection were consistently higher than those during the first infection, and the serologic responses to treatment were always slower.

Prevalence of syphilis infection in Mozambican women with second trimester miscarriage and women attending antenatal care in second trimester.

OBJECTIVES: To elucidate whether recent syphilis infection is significantly more prevalent among women with mid-trimester miscarriage than among antenatal care attenders in midtrimester pregnancy. DESIGN: Two categories of pregnant women were compared regarding serological signs of syphilis. Rapid Plasma Reagin (RPR) analyses were done in Mozambique and Veneral Disease Research Laboratory (VDRL) tests in Sweden. In case of RPR and/or VDRL positivity, Treponema pallidum haemagglutination (TPHA) and Captia Syphilis-M were performed. SETTING: A suburban antenatal care clinic and the emergency w…

Secondary syphilis primarily presenting with multiple nodules on the scalp: Case report and published work review.

Syphilis, a sexually transmitted disease caused by Treponema pallidum, manifests with a broad spectrum of clinical presentations that usually involves skin. Nodular secondary syphilis occurs rarely, and mostly manifests as disseminated papulonodules or plaques over the trunk and limbs. We report an extremely rare case of nodular secondary syphilis that primarily presented with multiple nodules on the scalp, resembling Rosai-Dorfman disease or cutaneous malignant metastasis. Immunohistochemistry confirmed T. pallidum spirochetes microscopically.

[The key role of the pathologist in the diagnosis of syphilis: A case report].

Syphilis is a resurging sexually transmitted infection linked to Treponema Pallidum. Syphilitic lymphadenitis can be present during the 3 stages of the disease. Its histological diagnosis remains challenging for the pathologist given its possible misleading aspect and its ability to mimic as well as reactional as tumoral conditions. We report the case of an HIV-infected young patient referred for suspicion of an aggressive lymphoma. Histological and immunohistochemical analysis of cervical lymphadenopathy revealed secondary syphilis.

[Sero-epidemiological study in Mauritania (1985-1986): incidence of treponematosis, hepatitis B virus, HIV virus and viral hemorrhagic fevers].

A serological serosurvey was made in different ethnic groups of Mauritania in 1985. A very high prevalence of hepatitis B markers was found with more than 20% of HBs antigen carriers. Treponema specific antibodies in low-age classes observed is a reflect of endemic syphilis. The seroprevalence of antibody against HIV and viral haemorrhagic fever viruses (Rift Valley fever, Crimean-Congo haemorrhagic fever and haemorrhagic fever with renal syndrome) was very low.

Cobra venom factor abrogates passive humoral resistance to syphilitic infection in hamsters.

Cobra venom factor, an agent commonly used to deplete complement, lowered the resistance of hamsters to infection with Treponema pallidum subsp. endemicum , as shown by a more rapid development of cutaneous lesions in infected animals treated with cobra venom factor than in infected, untreated animals. Cobra venom factor also abrogated the passive transfer of resistance by injection of serum from syphilitic immune hamsters. These results indicate that complement influences the pathogenesis of treponemal infection.

[Neurosyphilis today].

Since there has been an increase in the number of patients suffering from syphilis during the last 2 decades, neurosyphilis has to be seriously considered in many unclear neurological cases. Atypical, often monosymptomatic clinical pictures make the diagnosis difficult. The exact analysis of the spinal fluid including antibodies against Treponema pallidum is necessary for the diagnosis. In contrast to former treatment schedules, now ultra high doses of penicillin are recommended and seem to be absolutely necessary.
