Decoupling between fundamental frequency and energy envelope of neonate cries.

BACKGROUND: The presence of decoupling, i.e. the absence of coupling between fundamental frequency variation and energy envelope during phonetic crying, and its extent, reflects the degree of maturation of the central nervous system. AIM: We hereby wanted to assess the existence and extent of decoupling in term neonates (neurodevelopmental relevance) and whether an association between decoupling and clinical pain expression could be unveiled (clinical relevance). STUDY DESIGN: To assess decoupling in healthy term neonates during procedural pain, newborns were videotaped and crying was record

A synthetic fraction of feline facial pheromones calms but does not reduce struggling in cats before venous catheterization.

OBJECTIVE: To evaluate whether a synthetic analogue of feline facial pheromone (FFP) calms cats before, and reduces struggling during intravenous catheterization. DESIGN: Block-randomized, 'blinded' clinical trial. ANIMALS: Seventy-seven healthy cats presented for elective surgery. PROCEDURE: Cats given glycopyrrolate and oxymorphone were assigned to one of four treatments: acepromazine and exposure to FFP (aceFFP); acepromazine and exposure to a placebo (acePlac); exposure to FFP only (FFP) and exposure to placebo only (Plac). The behaviour of cats was recorded on videotape for evaluation

A video method for the evaluation of antidepressant clinical and behavioural actions.

Measuring efficacy and behavioural actions of new antidepressants (ADs) is greatly enhanced by having videotaped records of assessment interviews. This study describes a revised version of the Video Interview Behaviour Evaluation Scales (VIBES), shortened to make it more applicable to clinical trials. The method focuses on physically expressive, motor and social behavioural aspects of the depressive disorder. The Brief version permits juxtaposing of baseline and outcome interviews during the same viewing session thereby reducing the role of memory in the rater's observations. The method provid

The safety and efficacy of oral dextrose for relieving pain following venepuncture in neonates.

The objective of this study was to assess the efficacy and safety of oral 30% dextrose during venepuncture in neonates. Neonates admitted in the Special Care Nursery for jaundice from September 200 to January 2001 were recruited for this double-blind randomised controlled trial. The intervention consisted of administration of either 2 ml of oral 30% dextrose or 2 ml of sterile water 2 minutes before venepuncture. The primary outcome measure was the cumulative Neonatal Infant Pain Scale (NIPS) score at 3 minutes after venepuncture and the duration of cry assessed from a videotaped recording. Tw

Dystonia in corticobasal degeneration.

OBJECTIVE: To characterize the clinical features, particularly dystonia, in patients with clinically diagnosed or pathologically proven corticobasal degeneration (CBD). BACKGROUND: Although dystonia has been reported in many neurodegenerative disorders, it has not been studied in CBD. Dystonia, often accompanied by painful rigidity and fixed contractures, is one of the most disabling features of CBD. METHODS: The medical records, imaging studies, and videotapes of 66 patients who satisfied the clinical criteria of CBD, evaluated between 1988 and 1998, were reviewed. The occurrence, nature, a

Epilepsy surgery in developing countries.

Epilepsy surgery (ES) is a well-accepted treatment for medically intractable epilepsy patients in developed countries, but it is highly technology dependent. Such technology is not usually available in developing countries. For presurgical evaluation, magnetic resonance imaging (MRI) and electroencephalogram recording while videotaping the patient have been important. High technology equipment will, in conjunction with MRI, identify approximately 70% of ES candidates. Introducing ES into developing countries will require determining the candidates that are appropriate for the existing medical

Awareness and responsiveness during partial seizures.

We examined the impairment of consciousness during partial seizures (PS) arising from various brain sites according to the operational definition of the international classification, i.e., altered awareness and/or responsiveness. The subjects were 142 patients who underwent intracranial EEG evaluation and subsequent resective surgery. First, the patients were examined to determine whether they usually had been partially or completely aware of their seizures. Second, spontaneous habitual seizures that had been videotaped with simultaneous intracranial EEG recording were reviewed to determine re

In vitro modeling of spinal anesthesia. A digital video image processing technique and its application to catheter characterization.

BACKGROUND: Maldistribution of intrathecal local anesthetic has recently been implicated as a contributor to neurotoxic injury. In vitro modeling can be used to understand the distribution of anesthetic agents within the subarachnoid space. We describe an in vitro modeling technique that uses digital video image processing and its application to catheter injection of local anesthetic. METHODS: A clear plastic model of the subarachnoid space, including a simulated spinal cord and cauda equina, was filled with lactated Ringer's solution. Phthalocyanine blue dye of known concentration was inject

Accuracy of two-dimensional echocardiography in the diagnosis of congenital heart disease.

To assess the accuracy of 2-dimensional (2-D) echocardiography in the evaluation of cardiac anatomy in patients with congenital heart disease, 2-D echocardiograms were performed in 126 infants and children before cardiac catheterization and angiocardiography. The segmental echocardiographic analysis included determination of intracardiac, great artery, systemic venous and pulmonary venous anatomy. The 126 patients had 259 separate cardiovascular abnormalities, of which 226 (87%) were prospectively identified by 2-D echocardiography. There were 8 false-positive diagnoses. The most common lesion

Exploring the unspoken: Maternal implicit attitudes and mother-child emotional availability.

The quality of the mother-child bond is vital to children's socioemotional development. A mother's attitude toward her child might be especially important in this regard; however, assessing such attitudes is difficult given strong reporting biases in close relationships and especially mother-child relationships. Implicit attitudes-thought to reflect the summary of evaluative associations developed through personal experiences with a (close) other-may be one way to circumvent such biases. We investigated the link between mothers' implicit attitudes about her child and the quality of the mother-

Deep Phenotyping of Musicians' Upper Limb Dystonia.

BACKGROUND: Focal task-specific dystonia of the musicians' arm (FTSDma) is an unusual and challenging disorder, often causing significant disability with loss of performing careers. The etiology and optimal management of this disorder remains unclear. METHODS: We reviewed records and videos of 173 patients with FTSDma, 50 patients with writer's cramp (WC), and 16 with other forms of arm dystonia (OD), evaluated by a single examiner in clinical practice over a 25-year period. Detailed analysis of clinical features and videotaped examinations in slow motion (what we call "deep phenotyping") all

The association between childhood trauma and motor structure in obsessive-compulsive rituals: An ethological and evolutionary approach.

OBJECTIVE: Ethological models have highlighted a specific motor structure of compulsions in obsessive-compulsive disorder (OCD), based on the rigid repetitions of acts, and with the adaptive significance of facing unpredictable conditions. Such an evolutionary mechanism might explain the robust association between childhood traumatic experiences (CTEs) and OCD. However, a relationship between CTEs and the motor structure of compulsions has not been investigated yet. The first objective of the study was to confirm a specific motor structure of OCD compulsions with respect to control behaviors;

Couples communication and cancer: Sequences and trajectories of behavioral affective processes in relation to intimacy.

Cancer poses a set of physical and emotional challenges to the patient, spouse, and their relationship. One challenge for couples is discussing cancer-related concerns in a manner that facilitates intimacy. Current couple-based interventions have been shown to have mixed efficacy, and little is known about how they bring about improvements. This study aims to expand our understanding of dyadic communication and intimacy to adapt and/or develop more effective interventions for couples coping with cancer. To accomplish this goal, the present study examined affective and behavioral processes asso
