External fixation in severe open tibial shaft fractures: indications, orthoplastic strategy, and infection-aware decision-making.

Complex fractures of the femoral shaft rise problems due to high energy trauma with major soft-tissues injuries and bone comminution so increasing operative difficulties, risk of infection and delayed union. Our aim was to appraise the outcome of these problems when using a closed intramedullary locked nailing. There were 52 men and 16 women. Aged 16 to 83 years. 52 patients had multiple-injuries. There were 17 open fractures. The 68 cases were subdivided according to a classification in 3 types. Stable fractures type A and B1 were excluded. The shaft was divided into 5 zones. Patients were in

Staged conversion after circular ring external fixation in adult open long-bone fractures: A systematic review.

BACKGROUND: Circular ring external fixation is frequently used in high-energy open long-bone fractures, yet prolonged frame retention is associated with pin-tract morbidity and patient burden. Staged conversion to internal fixation is therefore attractive, but concerns persist regarding deep infection and nonunion. We systematically reviewed the evidence on outcomes and timing of staged conversion following circular ring fixation in adult open long-bone fractures. METHODS: This review followed PRISMA 2020 guidelines and was registered in PROSPERO (CRD42021023282). PubMed, Embase, Cochrane Lib

Staged screw removal in external plate fixation for distal tibial fractures: a finite element study on adaptive biomechanics.

High-energy distal tibial fractures present treatment challenges due to poor soft tissue coverage and risk of delayed union. External locking plates offer stability while preserving biology, yet the optimal strategy to dynamically modulate fixation rigidity in accordance with healing progression remains unclear. We constructed finite element models of a comminuted distal tibial fracture incorporating callus at four healing stages (0, 1, 3, and 6 months). Seven screw configurations and the effect of an intact fibula were evaluated under 800 N axial load. Outcome measures included fracture site

Indications for temporizing knee-spanning external fixation in the treatment of knee dislocations: A multi-center retrospective case series.

BACKGROUND: The indications for temporizing knee-spanning external fixation (KSEF) in the setting of knee dislocation (KD) are poorly defined, leading to significant uncertainty and inconsistency in clinical practice. This study aimed to analyze and describe the documented indications for temporizing KSEF in a series of patients with KDs. METHODS: A retrospective, multi-center review was conducted at two level I trauma centers from January 2001 to May 2024, identifying patients with documented KD treated with KSEF. Data were extracted from operative records, imaging, and clinical notes, and r

Biocompatibility and healing patterns in experimentally induced canine tibial fractures using Pedicle screw-Rod external fixation.

BACKGROUND: Biological osteosynthesis preserves blood supply and promotes rapid healing by aligning fracture fragments without direct surgical exposure. Pedicle screws are primarily designed for internal fixation in spinal procedures. A key objective of many orthopedic studies is to assess the biocompatibility of implants with bone and adjacent soft tissue. This study aims to evaluate the biocompatibility and effects of the Pedicle screw-Rod configuration as a novel external fixation method in canine tibial osteotomy. METHODS: With ethics approval, eight healthy, intact male dogs, aged 10-12

Comparative Analysis of Dorsal Distraction Plating Versus External Fixation in the Management of Intra-Articular Distal Radius Fractures.

INTRODUCTION: Fractures located in close proximity to the lower end of the radius bone, which make up around 16% of fractures requiring emergency treatment require careful treatment to regain mobility in the wrist joint and the fingers. Although classic external fixation (ExFix) methods are widely used, newer procedures such as dorsal distraction plating are showing potential . This study investigates the concepts, debates, and results of several methods so that to determine the most effective fracture treatment. MATERIALS AND METHODS: The experiment will recruit individuals with intricate fr

Radiographic and clinical evaluation of external pedicle screw fixation as a definitive solution for selective acetabular fractures: a retrospective analysis.

BACKGROUND: Acetabular fractures typically require open surgery to restore hip joint function. Openness may lead to serious tissue damage, increased bleeding, and the risk of nerve and vascular damage. Minimally invasive closed reduction or percutaneous fixation aims to minimize additional harm to patients and provide reliable fixation to promote fracture recovery and functional rehabilitation. This study aimed to assess the radiographic and clinical effectiveness of pedicle screw external fixation as a definitive treatment approach for selective acetabular fractures. METHODS: The present stu

Optimizing Outcomes in Distal Tibial Deformity Correction: The Role of Supramalleolar Osteotomy with Computer-assisted Hexapod External Fixator.

OBJECTIVE: Distal tibial deformities can significantly impact patients if left uncorrected, often leading to pain, alterations in gait, and the eventual development of post-traumatic arthritis. The criteria for surgical correction in these patients continues to be a subject of debate, while supramalleolar osteotomy (SMO) is an effective method for correcting distal tibial deformities. The purpose of this study was to evaluate and compare the clinical results of SMO using internal fixation or using computer-assisted hexapod external fixator in the treatment of distal tibial deformity. METHODS:

Does minimally invasive external fixation of proximal humerus fractures provide adequate stability? A biomechanical in vitro study.

INTRODUCTION: Proximal humerus fractures are usually treated with rigid angle-stable plates or nails. As an alternative, semi-rigid, minimally invasive systems have been developed for supportive fixation. A new system uses this principle with 2.5 mm partially threaded pins and external fixation. AIM: This study aimed to compare the fracture stability of the fixator with an angular stable plate about whether the mechanical stability is sufficient for early postoperative mobilisation. MATERIALS AND METHODS: Human proximal humeri (paired, female, age ≥ 60, n = 8) were cut to a 3-part fracture a

Application of Hybrid External Skeletal Fixation with Bone Tissue Engineering Techniques for Comminuted Fracture of the Proximal Radius in a Dog.

A seven-month-old male Pomeranian presented with left forelimb lameness after a fall. Radiographic assessment confirmed proximal radial head and ulnar comminuted fracture. The initial surgical intervention involved the use of hybrid external skeletal fixation (ESF) to stabilize the radial head, concomitant with the application of a composite of bone morphogenetic protein type 2 (BMP-2)-loaded hydroxyapatite and gelatin microparticles at the fracture site. Although successful radial head healing was achieved, the ESF pinholes caused a defect in the proximal ulnar diaphysis. Subsequently, the ES

Is Using a Ring External Fixator in the Treatment of Congenital Pseudarthrosis of the Tibia Associated With Better Results or Using a Locking Plate?

BACKGROUND: Congenital pseudarthrosis of the tibia (CPT) is a rare disease. CPT is often unilateral and occurs between the middle and distal third of the tibia. Concurrent involvement of the fibula is present in more than half of cases. histologic studies indicate the presence of fibrous hamartoma tissue and a sick periosteum, which leads to recalcitrant bone fracture and, eventually, pseudoarthrosis. Although there are various surgical techniques, we intend to compare the 2 methods of external fixation versus internal plating. METHODS: Demographic data were collected from 26 patients with fr
