A new paradigm for the treatment of ulcerative colitis has recently been presented: Treatment of the mucosa with lidocaine (2%) enemas for prolonged periods. This therapy was introduced based on the hypothesis that hyperreactive autonomic nerves may play a pathogenetic role in the disease. Sixty-nine per cent of a group of children with chronic mucoid diarrhoea who would not normally be seen in a tertiary health care facility had endoscopically demonstrable abnormality in the distal bowel, especially in the distal rectum. Sigmoidoscopy and punch biopsy of the rectum are safe procedures which can be undertaken in primary health care facilities. The inclusion of such investigations will facilitate the elucidation of the aetiology of chronic mucoid diarrhoea.

In addition to proctocolectomy, new surgical techniques for treating ulcerative colitis include the Kock internal ileal reservoir and endorectal pullthrough of the ileum. In children three forms of Crohn's disease requiring operation have been identified. Ileocecal disease may be treated with resection and anastomosis; colorectal disease is best managed by primary proctocolectomy cutaneous ileostomy; and Crohn's disease of the small bowel should be managed by medical therapy in most cases.

Rectal prolapse was the presenting clinical finding in a group of juvenile emus (Dromaius novaehollandiae). Gross findings included severely thickened and rugose distal rectal mucosae. Perianal complications of Crohn's disease are fairly common in the adult and pediatric populations. Transrectal ultrasonography is effective for the diagnosis and follow-up of patients with anorectal abscesses and fistulas in Crohn's disease. Retrospective analyses were made of 171 of 200 patients on whom operations had been performed for anorectal abscesses and fistulas, between 1981 and 1985. Between 1968 and 1972, 33 patients with carcinoma of the colon or the rectum underwent a radical surgical procedure followed by adjuvant irradiation therapy. One month after surgery, 6,000 rads were delivered in 6 to 7 weeks. Five of these patients presented later a radiotherapy ileitis, colitis or rectitis.
