Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) are caused by an exaggerated inflammatory response arising from a wide variety of pulmonary and systemic insults. Lung tissue is composed of a variety of cell populations, including parenchymal and immune cells. Emerging evidence has revealed that multiple cell populations in the lung work in concert to regulate lung inflammation in response to both direct and indirect stimulations. To date, the question of how different types of pulmonary cells communicate with each other and subsequently regulate or modulate inflammatory

Pulmonary complications after the use of extracorporeal circulation are common, and they range from transient hypoxemia with altered gas exchange to acute respiratory distress syndrome (ARDS), with variable severity. Similar to other end-organ dysfunction after cardiac surgery with extracorporeal circulation, pulmonary complications are attributed to the inflammatory response, ischemia-reperfusion injury, and reactive oxygen species liberated as a result of cardiopulmonary bypass. Several factors common in cardiac surgery with extracorporeal circulation may worsen the risk of pulmonary complic

Autoinflammatory syndromes are disorders with an exaggerated inflammatory response, mostly in the absence of an appropriate trigger. Prototypic autoinflammatory syndromes are FMF, hyper-IgD syndrome (also known as mevalonate kinase deficiency), TNF receptor-associated periodic syndrome and cryopyrin-associated periodic syndrome. The clinical phenotypes partly overlap (with fever and acute phase response), but also differ between the various syndromes (e.g. regarding fever pattern, episodic vs chronic inflammation and accompanying clinical signs). In recent years, the genetic basis of quite a n

INTRODUCTION: PSTPIP1-associated myeloid-related proteinemia inflammatory (PAMI) syndrome is a rare autoinflammatory disorder. Systemic inflammation, cytopenia, and skin lesions are classic features, accompanied by hypercalprotectinemia and hyperzincemia. Gastrointestinal manifestations such as colitis are infrequent. We present the first case in Thailand of PAMI syndrome presenting as refractory colitis.
CASE PRESENTATION: A 23-year-old male presenting with adult-onset refractory colitis and a history of a teenage perianal abscess. Despite the relatively late onset of intestinal symptoms, th

BACKGROUNDS: Both dietary preferences and systemic inflammation are associated with metabolic syndrome (MetS); however, the role of inflammation in the association between dietary preferences and MetS remains largely unclear.
METHODS: A total of 33,728 participants were included from the Henan Rural Cohort Study (n = 39,259). Food preferences were collected using a questionnaire. Generalized linear models were used to investigate the independent and joint associations of food preferences, specifically for sour or spicy foods, and inflammatory biomarkers [such as systemic immune-inflammatory i

UNLABELLED: Polymyositis (PM) is usually associated to other autoimmune or connective tissue diseases. The authors report the case of a 59-year-old man with pulmonary fibrosis, who presented with constitutional symptoms and gradually developed proximal muscle weakness, Raynaud phenomenon, and dysphagia. Besides creatine kinase (CK) elevation, he had positive anti-Polymyositis-Scleromyositis (PM-Scl) and anti-Sjögren's-syndrome A (SSA) antibodies. Nailfold capillaroscopy showed a scleroderma pattern and muscle biopsy revealed necrosis, regeneration of muscle fibers, and inflammatory infiltrate.

OBJECTIVE: It is not yet known whether systemic inflammatory indices affect the development of respiratory distress syndrome (RDS) in premature infants. We aimed to evaluate the relationship between systemic inflammatory indices obtained on the first day of life and the development of RDS in premature infants.
STUDY DESIGN: Premature infants with gestational age of ≤32 weeks were included in the study. Six systemic inflammatory indices involving neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), monocyte-to-lymphocyte ratio (MLR), systemic immune-inflammation index (SII

Coronavirus disease 2019 (COVID-19) has resulted in a global pandemic. Most COVID-19 patients are asymptomatic or have flu-like symptoms. However, around 15% of the patients may have severe disease, including unilateral or bilateral pneumonia with acute respiratory distress syndrome and progressive hypoxemia that may require mechanical ventilation assistance. A systemic inflammatory response syndrome occurs in the most severe forms of COVID-19, with multiorgan involvement which can be life threatening caused by a cytokine storm. Although what best characterizes COVID-19 are the manifestations …

Postoperative or trauma-induced intra-abdominal infections can result in sepsis and multiple organ dysfunction syndrome (MODS). Enteric bacteria and endotoxin released from the gut into the peritoneal cavity in response to injury can directly stimulate the inflammatory cascade responsible for the development of systemic inflammation and subsequent MODS. Therapeutic strategies, such as biologic modifiers that are aimed at blocking or enhancing specific mediators of the inflammatory response, have been developed and tested in animal models with varying efficacy in preventing mortality. Specific

Splanchnic and systemic low-grade inflammation has been proposed to be a consequence of long-term prehepatic portal hypertension. This experimental model causes minimal alternations in the liver, thus making a more selective study possible for the pathological changes characteristic of prehepatic portal hypertension. Low-grade splanchnic inflammation after long-term triple partial portal vein ligation could be associated with liver steatosis and portal hypertensive intestinal vasculopathy. In fact, we have previously shown that prehepatic portal hypertension in the rat induces liver steatosis

Vasoplegic syndrome, characterized by low systemic vascular resistance and hypotension in the presence of normal or supranormal cardiac function, is a frequent complication of cardiovascular surgery. It is associated with a diffuse systemic inflammatory response and is mediated largely through cellular hyperpolarization, high levels of inducible nitric oxide, and a relative vasopressin deficiency. Cardiopulmonary bypass is a particularly strong precipitant of the vasoplegic syndrome, largely due to its association with nitric oxide production and severe vasopressin deficiency. Postoperative va

Mutations in MECP2, encoding the epigenetic regulator methyl-CpG-binding protein 2, are the predominant cause of Rett syndrome, a disease characterized by both neurological symptoms and systemic abnormalities. Microglial dysfunction is thought to contribute to disease pathogenesis, and here we found microglia become activated and subsequently lost with disease progression in Mecp2-null mice. Mecp2 was found to be expressed in peripheral macrophage and monocyte populations, several of which also became depleted in Mecp2-null mice. RNA-seq revealed increased expression of glucocorticoid- and hyp

Despite its initial description as primarily a pulmonary disorder, the acute respiratory distress syndrome (ARDS) has been recognized as one component of a systemic disorder associated with microvascular permeability and widespread organ involvement that often afflicts the gastrointestinal tract. Although a healthy gut is an important defense for the body, its unique vascular anatomy and its predisposition toward ischemia and mucosal hypoxia put the gastrointestinal tract at high risk of injury during the course of the systemic inflammatory response syndrome. Once the mucosa is damaged and hos…

Sepsis is a severe systemic inflammatory response syndrome in which matrix metalloproteinase 9 (MMP9) plays a key role in inflammatory response and cell migration. In this study, we analyzed different sepsis phenotypes to explore the role of MMP9 in promoting the progression of sepsis, especially how it affects the inflammatory index and related pathogenic cell programs. A standardized cecal ligation puncture (CLP) model was used to evaluate the inflammatory response in sepsis. The experiments followed animal ethical principles and extracted proteins and performed SDS-PAGE analysis for mass sp…

BACKGROUND: Whole body ischemia and reperfusion injury after cardiac arrest leads to the massive inflammation clinically manifested in the post-cardiac arrest syndrome. Previous studies on the inflammatory effect on circulatory failure after cardiac arrest have either investigated a selected patient group or a limited part of the inflammatory mechanisms. We examined the association between cardiac arrest characteristics and inflammatory biomarkers, and between inflammatory biomarkers and circulatory failure after cardiac arrest, in an unselected patient cohort.
METHODS: This was a prospective

Lipopolysaccharide (LPS) is the major component of the outer membrane vesicles (OMVs) of Gram-negative bacteria. We hypothesized that OMVs-induced complement activation contributes to the development of systemic inflammatory response syndrome (SIRS). Yersinia enterocolitica O:3 (YeO3) variants synthesizing LPS of various chemotypes (S, Ra, Rd1, Re) were used as model microorganisms. OMVs activated complement more potently than parental bacteria or homologous LPS, independently of chemotype. A high molecular weight polysaccharide fraction, distinct from LPS, was recognized by serum mannose-bind…

Multisystem inflammatory syndrome in neonates (MIS-N) is a rare condition linked to perinatal exposure to SARS-CoV-2. We report a case of a term male neonate presenting at birth with severe respiratory failure and persistent pulmonary hypertension of the newborn. Initial evaluations, including chest X-ray and ECG, were unremarkable, but echocardiography showed elevated pulmonary pressures without structural abnormalities. Over the next few days, the infant developed diffuse skin peeling, acute kidney injury and hypertension, creating a perplexing clinical picture. Only after retrospective mate

BACKGROUND: Elevated intrarenal pressure during mini-percutaneous nephrolithotomy (mini-PCNL) may worsen systemic inflammation and affect stone clearance. This study compared stone-free rates and incidence of systemic inflammatory response syndrome (SIRS) between intelligent pressure-controlled and conventional mini-PCNL after propensity score matching (PSM).
METHODS: This retrospective cohort study included patients undergoing primary mini-PCNL for kidney stones (April 2022-April 2025). After PSM, patients were divided into a conventional mini-PCNL group (n = 136) and an intelligent pressure…

BACKGROUND: Systemic inflammatory response (SIR) occurs in normal pregnancy and is exacerbated in pre-eclampsia (PE). Most markers of SIR are expensive to measure, require trained personnel, and thus are limited for routine application in resource-limited settings. Thus, we aim to determine the clinical utility of hematologic markers of systemic inflammatory response in Preeclampsia in a resource limited setting.
METHODS: A cross-sectional comparative study was conducted in Enugu from April to October 2021. Forty-five women with pre-eclampsia (PE) and 45 matched normotensive (NT) pregnant wom
