Extracorporeal shock wave lithotripsy treatment in paraplegic patients with bladder stones.

BACKGROUND: Spinal cord injury patients are at increased risk of developing urolithiasis and many will require treatment, most commonly with extracorporeal shock wave lithotripsy (ESWL). METHODS: We examined 20 consecutive spinal cord injury patients (all male) undergoing ESWL for the treatment of bladder stones between April 1992 and May 2000. The mean number of shock waves administered in these 20 patients was 3600 and the shock power was 19 kW/s. All patients were treated without anesthesia. After ESWL, the bladder was irrigated with povidone iodine solution and stone fragments were evacua

Combination of pneumatic lithotripsy and transurethral prostatectomy in bladder stones with benign prostatic hyperplasia.

We present our experience with combined pneumatic lithotripsy and transurethral resection of the prostate (TURP) in 52 patients with bladder stone(s) and benign prostatic hyperplasia (BPH). All stones were fragmented with the pneumatic Swiss Lithoclast. Pneumatic lithotripsy and evacuation caused a mean increase of 16 minutes in operating time. No complications, other than mild hematuria, were observed intraoperatively because of pneumatic lithotripsy. We observed early and long-term complications related to the procedure in 13% of patients. The average hospital stay was 3.2 days. The combinat

Urological complications in 1,000 consecutive renal transplant recipients.

The urological complications in the first consecutive 1,000 renal transplants at our transplant center are reported with a minimum followup of 12 months. The kidney was implanted in the iliac fossa in all cases and in all but 3 the ureter was inserted into the bladder with a Politano-Leadbetter technique. Overall, there were 71 primary complications in 68 patients (7.1%), which included 36 ureteral obstructions, 25 ureteral or bladder leaks (including ureteral necrosis), 7 bladder outflow obstructions, 2 ureteral stones and 1 case of symptomatic vesicoureteral reflux. The use of high dose ster

Nonsurgical removal of urocystoliths in dogs and cats by voiding urohydropropulsion.

A technique called voiding urohydropropulsion has been developed that facilitates nonsurgical removal of urocystoliths. Voiding urohydropropulsion was performed in 11 dogs and 10 cats with urocystoliths. Urocystoliths were completely removed from 15 of 21 animals (5 female dogs, 3 male dogs, 5 female cats, and 1 male cat). The number of uroliths removed from any animal varied between 1 and 983. The mean time required to complete voiding urohydropropulsion in the 15 animals from which all uroliths were completely removed was 22 minutes. In 6 animals (2 female dogs, 3 female cats, and 1 male cat

Is the preliminary film necessary prior to the micturating cystourethrogram in children?

The aim of the study was to determine the value of the preliminary film in children undergoing a micturating cystourethrogram (MCU). The coded computer reports of 806 children undergoing MCUs in a 12-month period were retrospectively reviewed for abnormalities of the lumbar spine, hips, or for calcifications. Vesicoureteric reflux was present in 185 patients (23%). Four patients had renal calculi (0.5%), and in all cases the abnormality was evident on preceding imaging of the upper urinary tract. Spinal anomalies (other than known meningomyelocele) were present in four patients. The clinically

[Radiological exploration of hematuria].

Hematuria is defined as the presence of blood in urine. Its site of origin can be in any point of the urinary tract. Its discovery leads to clinical, biological and radiological investigations. The causes of hematuria are dominated, in terms of frequency, by urinary infection and lithiasis. The risk of associated lesions, especially of a tumor, must lead to regarding them as excluding diagnoses. Besides the so-called "medical" hematurias of glomerular origin (hematuria with proteinuria and cylinder casts) and hematurias occurring during a first episode of cystitis in women, the radiological ex

Bladder stone disease in children in Afghanistan.

Of 132 children with bladder stones seen in 1 year, 94% were boys and 73% were aged between 1 and 5 years. Significant malnutrition and evidence of vitamin deficiency were absent. Wheat bread was the dietary staple, while the intake of milk and dairy products, eggs and meat was very low. Analysis of 29 stones showed them to consist mainly of calcium oxalate and uric acid, with small amounts of calcium phosphate and magnesium ammonium phosphate in some cases. The composition of the surface and central parts of the stones, examined in 20 cases, was usually different. The nucleus was formed almos

Hypercalcemia in children with spinal cord injury.

Eighty-seven children less than 16 years of age with recent spinal cord injury were studied during a 7-year period (1970--1976). Serum calcium concentration was determined at least once in each of 76 of these patients, and in 18 (23.6%) it was above 11 mg%. Fifteen of the 18 patients had quadriplegia and 3 had paraplegia. In 5 patients, the first symptoms of acute hypercalcemia simulated those of an acute abdomen. Two of the 5 patients underwent exploratory laparotomy, with negative findings. The incidence of urinary stones was 55% (10 of the 18), a difference from the control population of on

The mechanism of urinary continence after prostatectomy.

An anatomical and neuropharmacological study of the urethra was performed in 19 patients after prostatetectomy to investigate the mechanism of continence. Anatomically, the urethral segment responsible for the postprostatectomy continence was located distal to the open prostatic fossa in the external sphincter which is comprised of intrinsic striated muscle and is called arbitrarily rhabdosphincter by the author. This unique striated muscle in the rhabdosphinecter appears to be primarily under the alpha-adrenergic influence in view of the change in the urethral pressure profile observed follow

Transurethral removal of bladder stone: the place of litholapaxy.

The surgical management of 652 cases of bladder stone, with particular reference to the value of litholapaxy, is discussed. The majority of cases can be treated by closed methods--litholapaxy and transurethral removal. Those cases treated by closed methods have a reduced postoperative complication and morbidity rate, a shorter operating time, and a reduced period of hospitalisation. Provided the necessary skill is acquired, it is concluded that litholapaxy is a safe and efficient method of dealing with bladder stones.

Ureteral pressure flow studies in difficult diagnostic problems.

Before a ureteral operation is undertaken for dilated, non-refluxing ureters it is essential to determine whether obstruction is present, since an operation is unnecessary and can be hazardous if there is no obstruction or infection. Obstruction is most accurately diagnosed by perfusing the upper tract at a known flow rate and measuring the resulting pressure. This test was performed on 5 patients in whom there was doubt as to the presence of obstruction from the radiographic evidence. In 4 of the 5 patients low pressure was found, the obstruction was excluded and an operation was avoided. In

Local factors compared with systemic factors in the formation of bladder uric acid stones.

AIM: The aim of this study was to evaluate the clinical characteristics of uric acid urolithiasis and how the presentation differs between urolithiasis of the upper urinary tract (UUT) compared with urolithiasis of the bladder. We attempted to evaluate the importance of local vs. systemic factors in the formation of bladder uric acid stones. PATIENTS AND METHODS: The medical records of 77 consecutive patients with uric acid urolithiasis from January 2004 to December 2006 were reviewed retrospectively. In all patients, histories were recorded, and physical examinations, renal function tests an

[Obstructive uropathy in adults].

Urinary tract obstruction is a common clinical problem. The obstruction of the urinary flow may be acute or chronic, partial or complete, unilateral or bilateral, and may occur at any site of the urinary tract. The major causes of urinary tract obstruction vary with the age of the patient. Anatomic abnormalities, e. g. ureteropelvic junction obstruction, account for the majority of cases in children. In comparison, calculi are most common in young adults, while prostatic hyperplasia or carcinoma, retroperitoneal or pelvic neoplasms, and calculi are the primary causes in older patients. Urinary

Clearance of Lower Renal Calyceal Residual Fragments Following Lithotripsy in a Patient With Spinal Cord Injury: A Case Report.

The clearance of residual fragments after lithotripsy is a problem of clinical relevance. Residual stone fragments can serve as nucleation centers for the deposition of calcified material, leading to recurrent formation of calculi. This case report presents a 63-year-old male with paraplegia of lower limbs who, through an empirical observation, managed to develop a protocol that resulted in clearance of residual calcified fragments of a lithotripsy performed 267 days ago. Preoperative imaging revealed the presence of kidney stones localized in the lower left renal calyces. The patient underwen

Controversies in the management of feline urethral obstruction.

OBJECTIVE: To discuss areas of differing opinion in the management of feline urethral obstruction and present current evidence to either support or refute common practices. ETIOLOGY: Urethral obstruction may occur as a result from a functional obstruction (idiopathic obstruction) or a physical obstruction, such as mucous plugs or calculi within the urethra. Potential risk factors for obstruction in cats include predominantly indoor status, decreased water intake, and increased body weight. DIAGNOSIS: The diagnosis is most commonly made based on history and initial physical exam-straining to

Ureteroileosigmoidostomy. Long-term results.

Ureteroileosigmoidostomy has been performed on 86 patients during the past twenty years, primarily for carcinoma of the bladder. Twenty-seven patients survived longer than five years. Severe complications occurred in 25 cases, and 11 per cent of the series required subsequent surgery. Hydronephrosis developed in 10 per cent. Ureteral reflux could be demonstrated by diatrizoate meglumine (Gastrografin) enema in 6 patients only.
