Combined cholecystotomy, retrograde hydropulsion, and choledochal stenting to treat extrahepatic biliary tract obstruction in 3 cats.

Extrahepatic biliary tract obstruction (EHBO) is uncommonly encountered in cats. Surgical treatment aims to decompress the biliary tract and insure bile duct patency. In veterinary medicine, cholecystotomy is not widely used in practice. The objective was to describe the use of cholecystotomy, retrograde hydropulsion of choleliths, and choledochal stenting to remove choleliths from the extrahepatic biliary tract back in the gallbladder. Three adult domestic shorthair cats were presented with anorexia, lethargy, and vomiting. Serum biochemistry revealed hyperbilirubinemia and increased hepatic …

Real-world outcomes among patients with advanced or metastatic biliary tract cancers initiating second-line treatment.

BACKGROUND: Limited data are available regarding second-line (2 L) treatment for advanced or metastatic biliary tract cancers (BTC) in the US real-world setting. This study explores the rapidly evolving and growing treatment landscape in the 2 L setting for advanced or metastatic BTC with a large cohort of patients treated in a community oncology setting. METHODS: Adult patients with BTC initiating 2 L treatment after a platinum-containing first-line between 1/1/10- and 6/30/19 were identified from the US Oncology Network electronic healthcare record database and followed through 12/31/19. Ba…

Synergistic combination of cytotoxic chemotherapy and cyclin-dependent kinase 4/6 inhibitors in biliary tract cancers.

BACKGROUND AND AIMS: Biliary tract cancers (BTCs) are uncommon, but highly lethal, gastrointestinal malignancies. Gemcitabine/cisplatin is a standard-of-care systemic therapy, but has a modest impact on survival and harbors toxicities, including myelosuppression, nephropathy, neuropathy, and ototoxicity. Whereas BTCs are characterized by aberrations activating the cyclinD1/cyclin-dependent kinase (CDK)4/6/CDK inhibitor 2a/retinoblastoma pathway, clinical use of CDK4/6 inhibitors as monotherapy is limited by lack of validated biomarkers, diffident preclinical efficacy, and development of acquir…

Circulating levels of soluble urokinase plasminogen activator receptor predict outcome after resection of biliary tract cancer.

BACKGROUND & AIMS: Surgical resection is the only potentially curative therapy for patients with biliary tract cancer (BTC), but 5-year survival rates after tumor resection have remained below 30%, corroborating the need for better stratification tools to identify the ideal surgical candidates. The soluble urokinase plasminogen activator receptor (suPAR) represents a mediator of inflammation and has been associated with distinct types of cancer. In this study, we evaluated a potential role of suPAR as a novel biomarker in patients undergoing BTC resection. METHODS: Tumor expression of uPAR wa…

NUC-1031/cisplatin versus gemcitabine/cisplatin in untreated locally advanced/metastatic biliary tract cancer (NuTide:121).

Gemcitabine/cisplatin is standard of care for first-line treatment of patients with advanced biliary tract cancer (aBTC); new treatments are needed. NUC-1031 is designed to overcome key cancer resistance mechanisms associated with gemcitabine. The tolerability/efficacy signal of NUC-1031/cisplatin in the Phase Ib ABC-08 study suggested that this combination may represent a more efficacious therapy than gemcitabine/cisplatin for patients with aBTC, leading to initiation of the global NuTide:121 study which will include 828 patients ≥18 years with untreated histologically/cytologically-confirmed…

A Multicenter Phase II Study of Gemcitabine plus S-1 Chemotherapy for Advanced Biliary Tract Cancer.

BACKGROUND: Gemcitabine (GEM) plus cisplatin (CDDP) chemotherapy has been used worldwide as the standard first-line treatment for advanced biliary tract cancer (BTC). A phase II trial has also suggested promising activity of GEM plus S-1 chemotherapy against advanced BTC. The aim of this study was to evaluate the efficacy and safety of GEM plus S-1 chemotherapy in patients with advanced BTC. PATIENTS AND METHODS: The eligibility criteria were as follows: histologically-proven BTC, unresectable or recurrent disease, ECOG performance status (PS) 0-1 regardless of previous treatment. Gemcitabine…

Neither neoadjuvant nor adjuvant therapy increases survival after biliary tract cancer resection with wide negative margins.

BACKGROUND: We investigated the role of neoadjuvant/adjuvant therapies on survival for resectable biliary tract cancer. We hypothesized that neoadjuvant and adjuvant therapy should improve the survival probability in these patients. METHODS: This was a retrospective review of a prospective database of patients resected for gallbladder cancer (GBC) and cholangiocarcinoma (CC). One hundred fifty-seven patients underwent resection for primary GBC (n = 63) and CC (n = 94). Fisher's exact test, Student's t test, the log-rank test, and a Cox proportional hazard model determined significant differen…

Recent advances in systemic therapy for biliary tract cancer: Evidence from Japan and recommendations in guidelines.

Systemic therapy for biliary tract cancer (BTC) has evolved substantially over the past two decades. Gemcitabine plus cisplatin was established as the global standard first-line regimen, followed by further optimization through alternative doublets, intensified triplet chemotherapy, and the recent introduction of immune checkpoint inhibitor-based combinations. In Japan, pivotal clinical trials have played a valuable role in refining treatment strategies, including the establishment of gemcitabine plus S-1, gemcitabine, cisplatin, plus S-1, and adjuvant S-1 after curative resection. Beyond cyto…

[Interpretation of "Guideline for quality control of laparoscopic radical resection for biliary tract cancer(2026 edition)"].

The publication of "Guideline for quality control of laparoscopic radical resection for biliary tract cancer(2026 edition)" marks the transition of laparoscopic radical resection for biliary tract cancer from technical exploration to a developmental stage centered on stratified indications, team credentialing, and whole-process quality control. The clinical value of laparoscopic radical resection for biliary tract cancer should not be evaluated solely by minimally invasive benefits, but should be established on the basis of strict patient selection, standardized margin assessment, adequate reg…

[Research progress on the role of tertiary lymphoid structures in the perioperative comprehensive therapy of biliary tract cancer].

Tertiary lymphoid structures (TLS), which are ectopic lymphocyte aggregates formed during chronic inflammation, are widely present in the tumor microenvironment of solid malignancies. By regulating anti-tumor immune responses, TLS is increasingly recognized as a key focus in cancer therapeutic research. However, systematic investigations into its role in biliary tract cancer (BTC) remain scarce. This essay outlines the fundamental characteristics, heterogeneity, and prognostic significance of TLS, and explores its potential role in perioperative comprehensive treatment and therapy sensitivity …

Hypoalbuminemia as a prognostic marker for survival in biliary tract cancer: associations with tumor type, treatment and sex.

BACKGROUND: Albumin in combination with other inflammatory markers has shown prognostic value in malignancy, including biliary tract cancer (BTC). This study aimed to evaluate the prognostic value of hypoalbuminemia alone in patients with BTC, with stratified analyses according to tumor type, treatment and sex. METHODS: A retrospective regional referral center cohort study was conducted, including consecutive patients with a measurement of preoperative albumin and intended resection of suspected BTC: intrahepatic cholangiocarcinoma (iCCA), perihilar cholangiocarcinoma (pCCA) or gallbladder ca…

A super-elderly patient with distal biliary tract cancer and tumor-associated granuloma: a case report.

BACKGROUND: Distal cholangiocarcinoma (dCCA) has a poor prognosis, and radical surgery is the only potential curative approach. However, elderly patients (≥80 years old) are often excluded from surgery due to high perioperative risks, and related reports are rare. CASE PRESENTATION: This article reports a 90-year-old female patient who presented with upper abdominal pain and distension. Imaging studies suggested a mass in the head of the pancreas. After undergoing pancreaticoduodenectomy, the pathology confirmed poorly differentiated cholangiocarcinoma, with negative margins (R0 resection). N

Gastrointestinal Basidiobolomycosis with Biliary Tract Involvement in the Early Postpartum Period: A Case Report and Literature Review.

Introduction: Gastrointestinal basidiobolomycosis is an uncommon invasive fungal infection caused by Basidiobolus ranarum, an environmental mould endemic to the arid south-west of Saudi Arabia and to a handful of other hot, dry regions. Its non-specific presentation routinely invites misdiagnosis as inflammatory bowel disease, tuberculosis or malignancy, and extension to the biliary tree is decidedly rare. Case Presentation: We report a 25-year-old Saudi woman who developed chronic watery diarrhoea and pronounced weight loss two months after her first delivery. Severe microcytic anaemia, leuko

Chemotherapy tolerance and continuity are prognostic for overall survival in patients with localized and advanced biliary tract cancer.

BACKGROUND/AIM: Biliary tract cancers (BTC) are relatively rare and have a poor prognosis in both localized and metastatic settings. Clinical trials tend to include patients who can tolerate treatments; however, chemotherapy eligibility, patterns, and survival may differ in the real world. The present study provides a 5-year overview of chemotherapy eligibility, patterns, tolerance, and survival in patients with resected and advanced BTCs. MATERIALS AND METHODS: Included in the study were patients with resectable or advanced BTC (excluding ampullary cancers) diagnosed between 2019 and 2024. T…
