Development of a gastroretentive delivery system for acyclovir by 3D printing technology and its in vivo pharmacokinetic evaluation in Beagle dogs.

Gastroretentive (GR) systems are designed to prolong gastric residence time to allow sustained absorption and improve the oral bioavailability of drugs with a narrow absorption window in the upper part of the gastrointestinal tract. The present study aimed to develop a GR system for acyclovir using 3D printing technology and evaluate its in vivo pharmacokinetics after oral administration in Beagle dogs. The system consisted of a gastro-floating device, which can float in the gastric fluid, prepared by a fused deposition modeling 3D printer and conventional acyclovir sustained-release (SR) tabl

A simultaneous d-optimal designed study for population pharmacokinetic analyses of mycophenolic Acid and tacrolimus early after renal transplantation.

Mycophenolic acid (MPA) and tacrolimus (TAC) are immunosuppressive agents used in combination with corticosteroids for the prevention of acute rejection after solid organ transplantation. Their pharmacokinetics (PK) show considerable unexplained intraindividual and interindividual variability, particularly in the early period after transplantation. The main objective of the present work was to design a study based on D-optimality to describe the PK of the 2 drugs with good precision and accuracy and to explain their variability by means of patients' demographics, biochemical test results, and

A Comparative Study on the Distribution Pattern of Endocrine Cells in the Gastrointestinal Tract of Two Small Alpine Mammals, Plateau Zokor (Eospalax baileyi) and Plateau Pika (Ochotona curzoniae).

Endocrine cells can secrete a variety of gastrointestinal hormones that regulate gastrointestinal digestion and absorption, which, in turn, play an important role in animal growth, metabolism, and acclimation. The small alpine mammals, plateau zokor (Eospalax baileyi) and plateau pika (Ochotona curzoniae), live in a unique ecotope with cold, hypoxic environments and short plant-growing seasons, resulting in differential adaptive digestive strategies for foods. Studying the distribution pattern of endocrine cells in the gastrointestinal tract (GIT) of these two animals can lead to a better unde

Neurodegenerative diseases and environmental risk factors: an overview of the available scientific evidence.

Alzheimer's disease (AD), Parkinson's disease (PD), and amyotrophic lateral sclerosis (ALS) are among the most well-known and prevalent neurodegenerative disorders. These diseases result from an interaction between the environment and genetically predisposed individuals. This review examines the evidence available in the literature underlying this multifaceted interaction, focusing on various chemical substances such as metals, fertilizers, and herbicides, as well as toxic agents of microbiological origin, including cyanobacteria and their neurotoxins. In addition, the pathways through which t

Recent Advances in Cationic Nanoemulsions for Drug Delivery: Preparation, Properties, and Applications.

Nanoemulsions have gained popularity as drug delivery vehicles owing to the enhanced solubility of insoluble drugs, the augmented stability of photo- and thermosensitive substances, and the facilitation of transdermal permeation of efficacy substances. As the cell surfaces of the skin, cornea, gastrointestinal mucosa, and other cells in living organisms carry negative charges, cationic nanoemulsions (CNE) mainly promote drug absorption through electrostatic effects. In this review, a brief characterization of CNEs is provided, and the types of cationic agents and their roles in nanoemulsions,

Interaction of zein/HP-β-CD nanoparticles with digestive enzymes: Enhancing curcumin bioavailability.

The low bioavailability of polyphenolic compounds due to poor solubility and stability is a major challenge. Encapsulation of polyphenols in zein-based composite nanoparticles can improve the water dispersion, stability, targeted delivery, and controlled release of polyphenols in the gastrointestinal tract. In this study, we investigated the fluorescence properties, bioactivity, and microstructural characteristics of polyphenols during digestion, revealing that zein nanoparticles protect polyphenols from gastric degradation and promote their sustained release in the small intestine. The effect

Vitamin B12: For more than just the treatment of megaloblastic anemia?

Vitamin B12, or cobalamin, belongs to the group of water-soluble vitamins and is ingested through food of animal origin such as eggs, milk, red meat and poultry, fish, and shellfish. Its clinical indication is the treatment of hypovitaminosis B12 administered orally or intramuscularly in the form of hydroxocobalamin. Hypovitaminosis B12 is mainly caused by deficient dietary intake (individuals with malnutrition, vegetarians or vegans, older adults, pregnant people, individuals with alcohol use disorder); when intestinal absorption is reduced (atrophic gastritis, malabsorption syndrome, gastroi

Low-Dose Eltrombopag in a Patient with Chronic Idiopathic Thrombocytopenic Purpura Post Sleeve Gastrectomy.

Idiopathic thrombocytopenic purpura (ITP) is a disease in which the immune system attacks platelets and causes decrease in its number exposing the patient to risk of bleeding. It is diagnosed by exclusion. Eltrombopag is a thrombopoietin receptor agonist which is used as second-line treatment for patients with ITP. The usual starting dose is 25 mg daily and maintenance dose is 75 mg daily. Little is known about the dose of eltrombopag in patients with sleeve gastrectomy since reduction in the amount of functioning gastrointestinal tract after gastric bypass surgery leads to decreased time to d

Lymphatic Dysfunction, Leukotrienes, and Lymphedema.

The lymphatic system is essential for the maintenance of tissue fluid homeostasis, gastrointestinal lipid absorption, and immune trafficking. Whereas lymphatic regeneration occurs physiologically in wound healing and tissue repair, pathological lymphangiogenesis has been implicated in a number of chronic diseases such as lymphedema, atherosclerosis, and cancer. Insight into the regulatory mechanisms of lymphangiogenesis and the manner in which uncontrolled inflammation promotes lymphatic dysfunction is urgently needed to guide the development of novel therapeutics: These would be designed to r

Evidence for Using Doxazosin in the Treatment of Posttraumatic Stress Disorder.

There is evidence that doxazosin is effective in the treatment of posttraumatic stress disorder (PTSD). Doxazosin is a "me-too" drug of prazosin. Doxazosin has an improved absorption profile and this likely minimizes the risk for unintended adverse hypotensive effects. The availability of doxazosin in the gastrointestinal therapeutic system (GITS) form permits a higher initial daily dose (4 mg/day) while avoiding significant first-dose side effects. The treatment of PTSD with prazosin has several disadvantages due to its short duration of action (6-8 hours), which results in multiple doses bei

Intraperitoneal chemotherapy against peritoneal carcinomatosis: current status and future perspective.

Peritoneal carcinomatosis (PC), caused by advanced abdominal malignancies, such as those of the ovarian and gastrointestinal tracts, has an extremely poor prognosis. Intraperitoneal (IP) chemotherapy has been clinically applied for several decades, but its clinical efficacy has not been fully determined. An accumulating body of evidence suggests that cytoreductive surgery (CRS) plus hyperthermic intraperitoneal chemotherapy (HIPEC) is the optimal treatment for selected patients with ovarian and colorectal cancers with PC. Recent studies suggest that IP administration of taxane with systemic ch

Intestinal uptake of insulin nanoparticles: facts or myths?

The oral route is the most suitable and physiological delivery route. Oral insulin delivery would minimize the health hazard implied in repeated injection, surpass complications arising from the need for sterile techniques associated with parenteral formulations and provide better glucose homeostasis. However, it is limited by various physiological barriers and still remains a scientific challenge. The desire to deliver insulin by the oral route in a conveniently and effectively way has led to the intense investigation of new delivery systems. Nanodelivery systems have been proposed to enhance

Clinical features and diagnosis of celiac disease.

Celiac disease is a chronic enteropathy caused by intolerance to gluten. The true prevalence of this condition is much greater than previously recognized, with increasing numbers of silent cases being diagnosed. Population-based studies, using serologic screening, have indicated that the prevalence of celiac disease in Caucasian populations is .5%-1%. The pattern of incidence is changing, with a greater proportion of cases diagnosed later in adulthood. The pathologic lesion is characterized by a flattened small intestinal mucosa with a lymphocytic infiltrate, crypt hyperplasia, and villous atr

Growth hormone, glutamine, and modified diet for intestinal adaptation.

Many patients who undergo extensive resection of the gastrointestinal tract develop intestinal failure from short-bowel syndrome that results in significant malabsorption of fluid, electrolytes, and other nutrients. This may result in dependence on long-term parenteral nutrition. It has been almost a decade since Byrne and colleagues published their research demonstrating enhanced absorption of nutrients, improved weight gain, and reduction in parenteral nutrition requirements with the administration of a combination of growth hormone, glutamine, and a modified diet. Other researchers have con

The role of thiamine deficiency in alcoholic brain disease.

A deficiency in the essential nutrient thiamine resulting from chronic alcohol consumption is one factor underlying alcohol-induced brain damage. Thiamine is a helper molecule (i.e., a cofactor) required by three enzymes involved in two pathways of carbohydrate metabolism. Because intermediate products of these pathways are needed for the generation of other essential molecules in the cells (e.g., building blocks of proteins and DNA as well as brain chemicals), a reduction in thiamine can interfere with numerous cellular functions, leading to serious brain disorders, including Wernicke-Korsako

[Intestinal infections due to inhibition of gastric acid secretion in reflux disease].

Gastric acid stimulates the absorption of nutrients and is the most important non-immunological defence system against the constant bacterial invasion of our digestive tract. Patients with achlorhydria and resected stomachs have excessive growth of bacteria in the digestive tract and a much higher incidence of gastrointestinal infections. Modern treatment of reflux oesophagitis with acid secretion inhibitors creates a similar low acid state. Suppression of gastric acid secretion causes a dose dependent increased risk of a wide variety of intestinal infections especially for people over 65, imm

Amylin and gastrointestinal activity.

1. Amylin is a new pancreatic islet peptide with a role in the maintenance of glucose homeostasis. 2. Amylin is predominantly present in the beta cells of the pancreas and to a lesser extent in the gastrointestinal tract and in the nervous system, where amylin mRNA is also present along with specific binding sites. 3. Amylin given peripherally or centrally inhibits acid gastric secretion in a dose-dependent manner and has a protective effect against indomethacin- or ethanol-induced ulcers only when injected centrally. 4. Subcutaneous or central injection of amylin produces a dose-dependent inh

The rational use of methotrexate in rheumatoid arthritis and other rheumatic diseases.

Methotrexate's mechanism of action affects both the inflammatory and immunosuppressive aspects of response. Its kinetics are defined and include variable absorption, intracellular metabolism, and both renal and biliary excretion. Methotrexate is clearly effective in the treatment of rheumatoid arthritis and may be able to decrease the rate of formation of new bony erosions. It is also effective in psoriatic arthritis and is being used in a multiplicity of other rheumatic diseases. The most common toxicities ascribed to methotrexate are gastrointestinal (e.g. stomatitis) and central nervous sys

A mesalazine enema for ulcerative colitis.

Given by mouth, drugs based on 5-aminosalicylic acid (5-ASA) are often effective in inducing or maintaining remission in patients with ulcerative colitis. All of them depend on delaying the release of 5-ASA, so minimising its absorption in the small intestine and maximising delivery to the colon. With sulphasalazine (a chemical combination of 5-ASA and sulphapyridine) and olsalazine (a combination of two molecules of 5-ASA) cleavage of the azo bond by bacteria releases free 5-ASA in the colon. Oral formulations of mesalazine (the approved name for 5-ASA when given alone as a drug) use various
