Primary hyperaldosteronism (Conns' syndrome) commonly presents with a combi-nation of clinical features of hypokalemia and hypertension. Atypical presentations like normo-tension, normokalemia and neurological ailments are described in few cases. We encountered two such cases, the first presenting with acute neurological complaint and second case having insig-nificant hypertension. Both the patients had a characteristic biochemical and imaging profile consistent with primary hyperaldosteronism and responded to surgical resection of adrenal adenoma.

SETTING: Outpatient dermatology practice at the Children's Hospital of Pittsburgh (Pa).
PARTICIPANTS: Children with culture-proven tinea pedis from 1987 until 1990.
MEASUREMENTS/MAIN RESULTS: Fungal cultures were used to identify 13 girls and 13 boys ranging in age from 17 months to 18 years with tinea pedis. A parent was the probable source of infection in at least 25% of cases.

Ionic regulation in the induction of exflagellation of Plasmodium berghei was investigated by culturing the parasites in various isotonic media. Of the salts tested, NaHCO3 exhibited the highest activity in inducing exflagellation, whereas KHCO3 showed no activity. In the absence of HCO3-, media containing monovalent cation (Na+, K+, Cs+, Rd+, choline+, lysine+, arginine+) and Cl- also induced exflagellation, but their activities were lower than that of NaHCO3. Studies were performed to assess the role of changes in the excretion of citrate, a metabolic precursor of bicarbonate, in acid-base balance in diuretic-induced metabolic alkalosis. Rats on a low-chloride diet with sodium sulfate added were studied during a base-line period, 3 days of furosemide administration, and 4 days post-furosemide. During the period of furosemide administration, net acid excretion and plasma bicarbonate concentration increased. In the post-furosemide period, net acid excretion remained higher than base line but plasma bicarbonate concentration did not increase further.

Between November, 1964 and January, 1983, 70 consecutive patients underwent primary repair of complex valvular heart disease, defined as repair or replacement of two or more cardiac values alone or with other concomitant cardiac procedures.
