A 76-year-old patient presented with a recurrent mass of a malignant chondrosarcoma in the right infratemporal fossa and in the left maxillary sinus with orbital invasion. The patient was treated with a palliative intention with MR-guided laser-induced thermotherapy using a modified applicator technique. Following treatment clinical symptoms improved and MRI revealed complete laser-induced tumor necrosis.

Many surgical techniques are employed for access to midline lesions of the anterior and middle fossa. A convenient one is the midfrontobasal approach. For various indications this surgical procedure was performed in 105 patients. Its benefits are outlined and a method for dealing with the two main drawbacks, postoperative loss of the sense of smell and the presumed risk of infection related to the inevitable opening of the frontal sinus, are described. In 83% of the cases the authors were able to anatomically retain the olfactory nerves, with preservation of the sense of smell in at least 65%

BACKGROUND: Endoscopic endonasal approaches for pituitary adenomas are associated with improved clinical and quality of life (QOL) outcomes. However, the necessity of middle turbinate resection to optimize surgical exposure remains controversial, particularly regarding its potential impact on postoperative nasal and tumor-related QOL.
METHODS: This prospective cohort study included adult patients undergoing endoscopic endonasal transsphenoidal resection of pituitary adenomas between 2014 and 2021 at a tertiary center. Patients were divided into those undergoing bilateral middle turbinectomy a

A 52-year-old man presented with progressive gait disturbances. MRI demonstrated a large extra-axial tumor in the cerebellopontine angle compressing the brainstem and resulting in hydrocephalus. While taking the patient's history, episodes of uncontrollable crying with sudden onset occurred multiple times. In this case report, we present and discuss the differential diagnoses of episodes of uncontrollable crying without emotional affect in adults and the possible underlying etiology.

Treatment of refractory cerebrospinal fluid (CSF) leaks, particularly those associated with large skull base defects, is challenging. A variety of synthetic biomaterial-based systems have been investigated in experimental models and/or humans for reconstructing cranial base defects. A widely used dental composite (bisphenol A-glycidyl methacrylate [bis-GMA]) has been shown to be effective for reconstruction of anterior skull base defects in animal models. Here, we report 4 patients who underwent reconstruction of large anterior skull base defects (1405.8 ± 511 mm2) secondary to tumor resection

A 72-year-old woman had undergone a right mastectomy for breast cancer 3 years earlier. PET/CT revealed a tumor with bony destruction of the skull base and FDG accumulation (SUVmax, 7.86). MRI showed the well-circumscribed tumor in the sphenoid sinus. The possibility of bone metastasis could not be denied. Tumor removal surgery was performed; the tumor was separate from the normal pituitary gland, and an ACTH-producing ectopic pituitary adenoma was diagnosed pathologically.

RATIONALE: When removing the meningiomas of the sellar region, there is always a risk of visual impairment for various reasons, in particular, as a result of traction damage to the optic nerve. Decompression of the optic canal increases nerve mobility during tumor manipulation. In cases of meningioma growing into the canal, its decompression often seems necessary.
AIM: Evaluation of the effectiveness and risks of performing decompression of the optic canal.
MATERIALS AND METHODS: The study included patients with meningiomas of the parasellar location, who underwent surgical treatment at the

AIM: To visualize cranial nerves (CNs) using diffusion tensor imaging (DTI) with special parameters. This study also involved the evaluation of preoperative estimates and intraoperative confirmation of the relationship between nerves and tumor by verifying the accuracy of visualization.
MATERIAL AND METHODS: 3T magnetic resonance imaging scans including 3D-FSPGR, FIESTA, and DTI were used to collect information from 18 patients with skull base tumor. DTI data were integrated into the 3D slicer for fiber tracking and overlapped anatomic images to determine course of nerves. 3D reconstruction o

An inflammatory pseudotumor is a rare benign disease presenting with non-specific chronic inflammation, and reported occurrences involving the skull base are relatively rare. A 65-year-old man became aware of pain around the right temporomandibular joint and mild trismus, and palsies of the cranial nerves III, IV, V, and VI were observed. A biopsy was performed under general anesthesia with an infratemporal fossa approach, and he was diagnosed with inflammatory pseudotumor of the infratemporal fossa. There was a rapid improvement in symptoms after the start of steroid administration, and 29 mo

The supraclavicular artery island (SAI) flap is a viable fasciocutaneous option for the reconstruction of head and neck defects. Although authors have reported success using SAI flaps for various reconstructive indications, concerns of a tenuous blood supply and distal ischemia have previously limited its use in the posterolateral skull base. This case series reports the outcomes of 5 consecutive patients receiving SAI flaps for posterolateral skull base reconstruction. All flaps were harvested in less than 1 hour with primary closure of all donor sites. A single patient developed superficial

Haemangiopericytoma is an uncommon vascular tumour of the sinonasal region with different clinical and pathological presentations. Given its origin from Zimmerman's pericytes around the capillaries, profuse bleeding during surgical manipulation is a characteristic sign. The open approach, with or without pre-operative embolisation, is therefore the usual technique for the resection of a large sinonasal haemangiopericytoma. We report on a case of haemangiopericytoma with massive unilateral involvement of the maxillary, anterior and posterior ethmoids, sphenoid, and frontal sinuses with extensio

We present the first case of ectopic ventricular recurrence of an epidermoid cyst in the middle fossa with confirming histological characteristics. A 51-year-old woman presented with an epidermoid cyst in the middle fossa and underwent complete resection. On the 6-month surgical follow-up MRI, a nodular lesion was detected in the frontal horn of the left lateral ventricle without evidence of recurrence at the primary site. The patient underwent transcortial tumor resection. Results of histological studies confirmed that the lesion was an epidermoid cyst similar to the primary lesion. This is a

PURPOSE OF REVIEW: Craniotomy created through the base of the skull has improved exposure of many types of extraaxial tumors and thus enhanced both tumor control and preservation of neural function. The purpose of this article is to review recent advances in this emerging field.
RECENT FINDINGS: Use of microscopes and endoscopes has allowed these procedures to become progressively less invasive. Electrophysiological monitoring has enhanced neural identification and preservation. The increasingly documented efficacy of stereotactic radiation for certain tumor types (e.g. meningioma, schwannoma

Skull base chordomas containing a sarcomatous component are extremely rare. Here the authors report two new cases in which a recurrent tumor with a sarcomatous component appeared after the patient had undergone charged-particle radiotherapy. Histological examinations performed in Case 1 revealed some retention of epithelial features in the sarcomatous component, whereas no such regions were observed in Case 2. Both patients had rapidly deteriorating clinical courses and died within 6 months after diagnosis of the recurrent tumor. The authors discuss the significance of the histological subtype

Garcin syndrome is characterized by a progressive ipsilateral involvement of cranial nerves, culminating in paralysis of all or at least seven of them, without sensory or motor long-tract disturbance, with no intracranial hypertension, and with osteoclastic involvement in the skull base on radiographic computed tomography. Giant cell tumor is a primary bone tumor rarely affecting the skull base. An 8-year-old female presented with a 3-month history of increasingly worsening right otalgia, tinnitus, hearing loss, right facial numbness, and diplopia. She was admitted with a 2-week history of swa

BACKGROUND: Although hearing loss is a common presenting symptom of jugular foramen schwannoma (JFS), recovery of hearing after tumor removal has rarely been reported. We report two cases of a large JFS presenting with severe hearing loss and recovering normal hearing after tumor removal.
METHODS AND RESULTS: Two patients complaining of progressive hearing loss each proved to have a large JFS involving the posterior fossa. The hearing losses, which exceeded 90 dB, were not associated with dysfunction of the 9th, 10th, or 11th cranial nerves. Schwannomas were removed by a suboccipital retrosig
