Free functional gracilis muscle opponensplasty for thenar reconstruction: Indications, technique, and long-term outcomes.

Thumb opposition is an essential component of human hand function, and loss of the thenar musculature creates substantial disability of the hand. Additionally, first web space contracture as a result of trauma is a substantial limitation in reconstruction of thumb opposition. Reconstruction of thenar function is most commonly performed via opponensplasty tendon transfers. Although beneficial, tendon transfers require cortical retraining and fall short of native function. These may be the only option in the case of median nerve palsy. However, at times, thenar function is lost owing to thenar m…

Talonavicular joint arthrodesis for paralytic post poliomyelitis forefoot instability.

A 17-year-old patient presented with paralytic valgus left foot deformity after poliomyelitis. Forefoot instability resulted from the transfer of peroneus brevis to the second metatarsal bone. This was secondary to a medial gapping of the talonavicular joint as a result of unbalanced pull of the transferred tendon. Talonavicular arthrodesis following a medial-based wedge articular resection has corrected the deformity.

[Problems in complex hindfoot corrections].

This article discusses the intraoperative and postoperative problems seen in the treatment of posterior tibial tendon dysfunction. Problems associated with tendon transposition procedures, osteotomy and arthrodesis are discussed. The preoperative, intraoperative and postoperative problems and complications and how to avoid or treat them will be addressed. The individual procedures are often part of other complex hindfoot reconstructions. For this reason the general and special aspects of treating posterior tibial dysfunction can often be transferred to avoiding and resolving problems in hindfo

Free microneurovascular muscle transfer in limbs to provide motor power.

The experience gained in a series of 7 free microneurovascular muscle transfers to the limbs over a seven-year period has led us to conclude that: free gracilis microneurovascular flaps can be safely transferred and can aid restoration of movement following severe injuries of the limbs, including post-replantation cases; inclusion of a cutaneous island with the muscle is safe, improves cover, and simplifies closure following muscle transfer; tendon adhesions and joint stiffness are the greatest obstacles to transferred muscle function, and early passive extension is recommended; additional pro

A rare complication of scaphoid nonunion: Multiple flexor tendon lesions. A case report and review of literature.

This case report describes a patient who presented with a complete rupture of the flexor pollicis longus (FPL) tendon and partial rupture of the flexor digitorum profundus of the index finger, secondary to scaphoid nonunion. This is a rare, late complication that deserves to be described because of the potential diagnostic confusion with anterior interosseous nerve palsy. No case has been reported since 1999 in the literature. The mechanism was an attrition rupture due to sharp osteophytes. The scaphoid osteophytes were removed and the FPL was repaired by tendon transfer. The results were sati…

Shoulder arthroplasty for the treatment of rotator cuff insufficiency.

Irreparable rotator cuff tendon tears result from chronic tears, failed cuff repairs, and fracture sequelae and occur in patients with rheumatoid arthritis. The management of patients with cuff tear arthropathy can be challenging. When pain is severe and function is poor, surgical options include hemiarthroplasty, bipolar arthroplasty, extended head arthroplasty, arthroplasty with tendon transfer, reverse shoulder arthroplasty, and fusion. A review of the literature shows good pain relief with hemiarthroplasty in carefully selected patients; however, the reverse prosthesis has been found to be…

Oerative treatment of the plantar-flexed inverted foot in adult hemiplegia.

From a population of 902 adult patients with acquired hemiplegia, thirty-two patients with extensor synergy were chosen for a reconstructive procedure designed to eliminate the need for an orthosis. Three additional patients underwent the procedure to eliminate severe spasticity which precluded orthotic fitting. The operative technique, first described by Mooney and associates, involved lengthening of the tendo achillis and tibialis posterior tendon, multiple toe-flexor tenotomies, and a split transfer of the tibialis anterior tendon. Satisfactory results were recorded for thirty-two patients.

One-stage reconstruction of post-electrical burn forearm and hand defects using microsurgical transfer of an ulnar neuromyotendinocutaneous unit.

Three cases of high-voltage electrical burns of the wrist and forearm were reconstructed, using vascularized ipsilateral ulnar nerve transfers to median-nerve defects. Two of these cases utilized composite ulnar neuromyotendinous flap transfers, with the muscle (flexor carpi ulnaris) bridging the gap between the flexor muscle bellies and tendons. Sensory recovery in the hand was excellent in all three cases.

Nerve Transfers for the Restoration of Wrist, Finger, and Thumb Extension After High Radial Nerve Injury.

High radial nerve injury is a common pattern of peripheral nerve injury most often associated with orthopedic trauma. Nerve transfers to the wrist and finger extensors, often from the median nerve, offer several advantages when compared to nerve repair or grafting and tendon transfer. In this article, we discuss the forearm anatomy pertinent to performing these nerve transfers and review the literature surrounding nerve transfers for wrist, finger, and thumb extension. A suggested algorithm for management of acute traumatic high radial nerve palsy is offered, and our preferred surgical techniq…

A simulation analysis of the combined effects of muscle strength and surgical tensioning on lateral pinch force following brachioradialis to flexor pollicis longus transfer.

Biomechanical simulations of tendon transfers performed following tetraplegia suggest that surgical tensioning influences clinical outcomes. However, previous studies have focused on the biomechanical properties of only the transferred muscle. We developed simulations of the tetraplegic upper limb following transfer of the brachioradialis (BR) to the flexor pollicis longus (FPL) to examine the influence of residual upper limb strength on predictions of post-operative transferred muscle function. Our simulations included the transfer, ECRB, ECRL, the three heads of the triceps, brachialis, and …
