His Bundle and Left Bundle Pacing in Pediatrics and Congenital Heart Disease: A Single Center Experience.

Long-term right ventricular pacing is associated with left ventricular dysfunction and cardiomyopathy, particularly in pediatric patients and those with congenital heart disease (CHD). Research has shown that pacing-induced cardiomyopathy can be reversed with nonselective or selective His bundle pacing in adults, however, the information available about the use of this type of therapy in pediatrics and CHD is scarce. We performed a retrospective chart review of all the cases of His or left bundle pacing at the University of Minnesota, division of Pediatric Cardiology from January of 2019 to Ap

Voltage dependent conduction abnormalities in His bundle pacing in patients without His Purkinje system disease.

BACKGROUND: During His bundle (HB) pacing, the active fixation of HB lead may cause disruption of the enclosed conduction fibers and cause conduction delay. His Purkinje conduction delays have been shown to revert to normal with higher voltage pacing. OBJECTIVE: To determine if conduction delays seen with penetrative HB pacing are voltage dependent and can resolve with higher stimulus voltage. METHODS: In 17 patients undergoing HBP, the effect of voltage on a composite His Purkinje system-ventricular activation time (hVAT) as well as electrocardiographic (ECG) evidence of conduction delay, w

Intra-His bundle block in acute myocardial infarction. Report of two cases.

Two patients with acute intra-His bundle block, both secondary to myocardial infarction, are described. The diagnosis of the block was proved by means of His bundle recordings during the phase of complete atrio-ventricular (A-V) block. Two His bundle potentials, one (H) following each A wave and the other (H') preceding each QRS complex, were detected. In both cases, as well as in the few cases reported in the literature, the site of the infarction was in the inferior wall, an occurrence which shows the functional importance of the posterior blood supply to the His bundle. In both cases the bl

Pre-excitation of the ventricle associated with total intra His bundle block.

A case with total intra-His bundle block and intermittent pre-excitation syndrome is presented. During A-V conduction there was a P-delta interval of 130 msec. with a P-A interval of 20 msec., an A-H interval of 60 msec. and an H-V interval of 50 msec. During rapid atrial pacing the P-delta interval increased primarily due to an A-H1 prolongation and a Mobitz type 2 block and total A-V block occured at increasing rates showing H1 following every A spike. The escape beats showed a normal width of the QRS complexes with preceding H2 spikes. After administration of Ajmaline the bypass tract was b

Echocardiographic Evaluation of His Bundle Pacing in Patients with Prolonged PR Intervals.

BACKGROUND: Patients with PR intervals >240ms have atrio-ventricular (AV) dyssynchrony, which can increase risk of atrial fibrillation and all-cause mortality. When requiring pacing, long AV delays (AVDs) have been programmed to avoid ventricular dyssychrony. His bundle pacing (HBP) may provide improved AV synchrony in patients with prolonged PR. METHODS: 10 patients with sinus node dysfunction and prolonged PR who received HBP were studied. Real-time echocardiographic was performed with 3 pacemaker modes (RV septal, non-selective HBP, and selective HBP) using the following pacemaker settings

His bundle pacing after failure of cardiac resynchronization therapy: a case study.

His bundle pacing is a relatively new method of cardiac pacing. This method is used in patients with atrioventricular block to prevent heart failure associated with right ventricular pacing, and in patients with bundle branch block and cardiomyopathy. We report a patient with cardiomyopathy and left bundle branch block with failure of cardiac resynchronization therapy. Permanent His bundle pacing was associated with clinical improvement and improvement of parameters of cardiac function.

Long term performance and safety of His bundle pacing: A multicenter experience.

INTRODUCTION: Several single-center short-term studies have demonstrated the feasibility, safety, and positive clinical outcomes of permanent His bundle pacing (HBP). We performed a retrospective study to evaluate long-term technical and safety performances of HBP in a large population of pacemaker patients from two different centers. METHODS AND RESULTS: The analysis includes 844 patients (345 female, mean age = 75 ± 9 years) who underwent successful permanent HBP for pacemaker indications from 2004 to 2016. The main endpoints were long term electrical performances including pacing threshold

His Bundle Pacing: A New Frontier in the Treatment of Heart Failure.

Biventricular pacing has revolutionised the treatment of heart failure in patients with sinus rhythm and left bundle branch block; however, left ventricular-lead placement is not always technically possible. Furthermore, biventricular pacing does not fully normalise ventricular activation and, therefore, the ventricular resynchronisation is imperfect. Right ventricular pacing for bradycardia may cause or worsen heart failure in some patients by causing dyssynchronous ventricular activation. His bundle pacing activates the ventricles via the native His-Purkinje system, resulting in true physiol

Restitution characteristics of His bundle and working myocardium in isolated rabbit hearts.

The Purkinje system (PS) and the His bundle have been recently implicated as an important driver of the rapid activation rate after 1-2 minutes of ventricular fibrillation (VF). It is unknown whether activations during VF propagate through the His-Purkinje system to other portions of the the working myocardium (WM). Little is known about restitution characteristic differences between the His bundle and working myocardium at short cycle lengths. In this study, rabbit hearts (n = 9) were isolated, Langendorff-perfused, and electromechanically uncoupled with blebbistatin (10 μM). Pacing pulses we

Catheter ablation of premature ventricular contractions originating from the His bundle region.

We describe a 35-year-old man with idiopathic frequent premature ventricular contractions (PVCs) originating from the His-bundle region, where a low frequency ventricular potential was recorded in the bipolar recording and QR pattern in the unipolar recording. Electro-anatomical mapping revealed a confined low voltage zone (<1.2 mV) in the His-bundle region, and the activation spread out radially to the entire right ventricle. The PVCs were successfully abolished without any atrioventricular block by a step-wise incremental radiofrequency energy application to the His-bundle region. The patien

The clinical value of the His-bundle electrogram in intraventricular conduction defects.

A study concerned with the clinical significance of the His-bundle ECG was carried out in association with upper atrial stimulation in 20 patients with intraventricular conduction defect. On the ground of the results the His-bundle ECG is regarded as a procedure of diagnostic value in intraventricular conduction defects by contributing to the accuracy of information on the severity and extent of the lesion. In the framework of published observations, including the present findings, questions relating to prognosis and to the necessity for pacing in bifascicular block are discussed.

The impact of His bundle location and direction on the efficacy and safety of ablation.

BACKGROUND: The impact of the His bundle location and distance from the ablation site on ablation efficacy and complication risk remains unexplored. We determined the correlation between age, height, body mass index (BMI), and the His bundle location, and whether the distance between the His bundle and ablation target (DHIS-ABL) affects ablation safety and efficacy. METHODS: Overall, 346 patients with atrioventricular nodal re-entrant tachycardia (AVNRT) and 96 with atrioventricular re-entrant tachycardia (AVRT) were retrospectively analyzed. The distance between the His bundle and the corona

QT Prolongation After His Bundle Pacing: What Is the Mechanism?

An 82-year-old woman with nonischemic cardiomyopathy underwent cardiac resynchronization therapy by the use of His bundle pacing. After the procedure, the patient had repolarization abnormality with severely prolonged QTc and anterior inferior T-wave inversions, likely resulting from memory T waves associated with the correction of long-standing left bundle branch block. These changes could be potentially arrhythmogenic. (Level of Difficulty: Intermediate.).

The phenomenon of concealed conduction in a case of His bundle pacing (HBP).

The phenomenon of concealed conduction in His bundle pacing (HBP) has rarely been reported. We report here a patient who had 2: 1 atrioventricular block and complete left bundle branch block was used because of HBP. The patient developed continuous right bundle branch block after the operation. After excluding mechanical damage, we speculate that the mechanism of right bundle branch block in this case was due to concealed conduction.
