Burns from a dust explosion.

Two cases of burn injury following a dust explosion are presented. It is important to (1) rule out other serious injuries and (2) continuously reassess the patient for respiratory injury. The mechanism of dust explosions is described and preventative measures are discussed.

[Rectal burns caused by enema].

The injuries incident to enemas may vary from oedema of the mucosa to gangrene of the large bowel. The burn of the rectum is very rare, and in mild cases it has an identical course to that of the caustic oesophagitis. The place of corticosteroids, administered by intravenous route or topically, in the therapy of te burn of the rectum is unproven. Healing of the lesion occurs by fibrosis and may result in structure formation, which usually need a treatment only when obstructive symptoms develops.

[Acute abdomen and thermal burns].

A review of the literature is made on the problem of the acute surgical abdomen under conditions of thermic shock. Such basic nosologic entities, as acute ulcers, acute cholecystitis, thrombosis and embolism of mesenterial vessels and hematogenous peritonitis, which may be directly related to thermic burns, are given detailed consideration. Thermic trauma raises complex and diverse problems; one of the pathways to reduce the lethal outcome in patients with burns is to face the acute abdomen problem with adequate approach to the pathogenesis, diagnosis and treatment of each nosologic entity.

Chemical burns of the oesophagus.

Chemical burns of the oesophagus caused by ingestion of corrosives present a difficult and potentially dangerous problem of management. Initial failure to recognize the seriousness of the injury and inexperience in the handling of the severer burn may result in consequences which are both life-threatening and life-long. Analysis of 95 patients who had ingested caustic substances is presented. Ninty-one sustained burns of the mouth, 39 of the hypopharynx or oesophagus, and 31 had laryngeal burns. Sixty-three were children of 15 years and under. Caustic soda, especially in children, was the most…

Childhood burns in Zaria, Nigeria.

From 1971 to 1980, 429 children with burn injuries were admitted to the Ahmadu Bello University Hospital, Zaria. These were major burns in 275 patients, moderate in 82 and minor in 72. Fourteen of the patients were neonates, 102 infants, 228 were 5 years old or younger and 85 were older. Socioeconomic factors contributing to the injuries included the use of firewood for cooking at ground-level and for warming the house and body during the cold season; loose indigenous garments; thatch-roofed huts and the post-partum rituals of mud-bed heating and hot baths. Flame burns exceeded scalds with a s…

Burns in motor vehicle accidents.

Burns received as a result of motor vehicle accidents (MVA's) create special problems in their care, as they are frequently severe and are often associated with other injuries. One hundred seventy-eight consecutive patients with burns sustained in an MVA were studied. The mean TBSA burn was 33.9%. The mortality was 24.7%, but the mean burn size in this fatal group was almost doubled at 63.9%. The injury most commonly associated with death was inhalation injury (in 36.3%). Thirty-six per cent of the patients sustained other injuries in addition to their burn, the most frequent of which was to t…

Improved survival after massive burns.

Sixteen patients with massive burns (exceeding 50% of total body surface) were treated at the University of California--Davis Burn Center in the period of 1980 and 1981. Fifteen had flame burns, and eight had inhalation injuries. Mean burn size was 72% total body surface (range, 51-94) with 20-81% full thickness. Mean age was 27 years. Survival results were compared with a similar group of 13 patients treated in 1978 and 1979, mean age 25, and burn size 65% total body surface. Fifteen of the 16 survived, compared with six of 13 in the early group. Substantial changes in therapy between the tim…

[Myositis ossificans following extensive burns].

A report on a case of myositis ossificans following 50-60% burns (second to third-degree) on the trunk and extremities with periarticular localization of the ossification at the hip and knee joints. In terms of its extent and localization the ossification is primarily similar to post-paraplegic ossification of soft tissues.

Heterotopic periarticular ossification in burns.

The pathogenesis of this rather rare complication of burn injury is discussed. Injury and bleeding as a result of enthusiastic physiotherapy are usually incriminated as the major cause of such bone formation. More recent work is quoted which suggests that increased nutritional support regimes, particularly in respect of protein, cause a calciuretic response in burn patients. This iatrogenic mobilization of calcium, together with the increased levels of calcium due to injury and bed rest, may precipitate this complication. The periarticular location usually seen in burns, as compared with the h…

"Barbiturate burns" caused by glutethimide.

A case of glutethimide overdose associated with skin lesions resembling burns is reported. These characteristic skin lesions are usually ascribed to barbiturates. Their aetiology, incidence, and association with other drugs and neurological disorders are discussed.
