The causes of ipsilateral arm pain were analyzed in a consecutive series of 38 patients with breast cancer. A lesion of the brachial plexus was diagnosed in 17 patients, of whom 8 had tumor involvement, 5 had radiation fibrosis, 1 had lymphedema entrapment, and 3 had a probable transient neuritis of the plexus. In four patients, a cervical radiculopathy was found; two of these patients had a Horner's syndrome. A carpal tunnel syndrome was seen in four patients and could possibly be attributed to lymphedema in two patients. In five patients, a pericapsulitis of the shoulder joint was seen. Seve

After exploiting all causal therapies there are other therapeutic methods only for soothing, viz. anodyne drug therapy using peripheral or central efficient analgesics, opiates and opioids as well. Procedures with destructing the pain-sensitive nerve path (chemical neurolysis using a method like conduction anaesthesia) and blockage of the sympathetic trunk and plexus coeliacus are preferred against intrathecal rhizolysis. Operative: Percutaneous cervical chordotomy. If insufficient analgesia results by these methods patients may get a peridural long acting catheter for applying local anaesthet

When invasion of the brachial plexus occurs, tumours of the lung apex result in intolerable pain. On the basis of 13 cases, the authors show that surgical excision, whilst only palliative, is associated with immediate total or quasi-total disappearance of pain in all patients. This effect lasted for two to eighteen months. Operation gives marked relief in a painful situation not helped by medical treatment.

Cancer pain (CP) arises from a complex interplay between the tumour and its microenvironment. Many patients experience a mixed pain phenotype that encompasses nociceptive, neuropathic and neuroinflammatory mechanisms, and vary across tumour type and disease stage. Despite decades of intensive research, the mainstay of cancer pain treatment is still non-steroidal anti-inflammatory drugs (NSAIDs) and opioids. Recent advances in cancer neuroscience have provided novel insights into the neurobiology of cancer pain. The emerging picture of cancer pain is a disorder of aberrant crosstalk between the

OPINION STATEMENT: Cancer associated pain (CAP) is challenging to treat and may be complicated by type of cancer, location, presence of metastases, cancer therapies, medical co-morbidities, psychosocial factors, and barriers to treatment. Inadequate CAP management can lead to delays in receiving chemo/radiation therapy, and medications or interventions that provide analgesia. In this narrative review, we aim to provide an evidence-based overview of current and emerging interventional pain techniques used to manage CAP. Intractable visceral cancer pain refractory to conservative medical managem

Despite the unique and complex nature of cancer pain, the activation of different ion channels can be related to the initiation and maintenance of pain. The transient receptor potential vanilloid 4 (TRPV4) is a cation channel broadly expressed in sensory afferent neurons. This channel is activated by multiple stimuli to mediate pain perception associated with inflammatory and neuropathic pain. Here, we focused on summarizing the role of TRPV4 in cancer etiology and cancer-induced pain mechanisms. Many studies revealed that the administration of a TRPV4 antagonist and TRPV4 knockdown diminishes

This is a case report regarding a patient on maintenance therapy with methadone wth cancer pain. Minimal increase in methadone dose and a better modulation of administration intervals were effective, allowing the achievement of an optimal analgesia in a short time. This effect was maintained at home after discharge up the last follow-up 3 weeks after discharge. Existing literature is discussed and it is suggested to use the same drug, methadone, in increased doses.

Cancer-induced bone pain (CIBP) is a type of ongoing or breakthrough pain caused by a primary bone tumor or bone metastasis. CIBP constitutes a specific pain state with distinct characteristics; however, it shares similarities with inflammatory and neuropathic pain. At present, although various therapies have been developed for this condition, complete relief from CIBP in patients with cancer is yet to be achieved. Hence, it is urgent to study the mechanism underlying CIBP to develop efficient analgesic drugs. Herein, we focused on the peripheral mechanism associated with the initiation of CIB

Neuropathic cancer pain (NCP) is prevalent affecting up to 58% of those with persistent pain following cancer treatment. Neuropathic pain can develop from malignancy, after neural tissue insult during surgery and/or exposure to radiation or neurotoxic agents used as part of cancer treatment regimens. Pain following cancer treatment is commonly under-treated and one barrier identified is poor recognition of pain and inadequate assessment. Recognition of the presence of NCP is important to inform pain management, which is challenging to treat and warrants the use of specific treatments to target

BackgroundCancer-induced Bone Pain (CIBP) is an important factor affecting their quality of life of cancer survivors. In addition, current clinical practice and scientific research suggest that neuropathic pain is a representative component of CIBP. However, given the variability of cancer conditions and the complexity of neuropathic pain, related mechanisms have been continuously supplemented but have not been perfected.Aim of reviewTherefore, the current review highlights the latest progress in basic research on the field and proposes potential therapeutic targets, representative drugs and u

Neuropathic cancer pain (NCP) is caused by nerve damage attributable to the cancer per se, and/or treatments including chemotherapy, radiotherapy, and surgery; the prevalence is reported to be as high as 40%. The etiologies of NCP include direct nerve invasion or nerve compression by the cancer, neural toxicity, chemotherapy, and radiotherapy. NCP is subdivided into plexopathy, radiculopathy, and peripheral neuropathies, among several other categories. The clinical characteristics of NCP differ from those of nociceptive pain in terms of both the hypersensitivity symptoms (burning, tingling, an

BACKGROUND AND AIMS: Cancer pain treatment has improved over the last decades. The majority of this population can be treated effectively with analgesics following the Guidelines of the original World Health Organisation (WHO). Unfortunately 10-15% of these patients still suffer from severe and refractory cancer pain, especially in the terminal phases of disease and require additional pain management modalities. Therefore, end-stage clinical interventions are particularly needed to minimize the perception of pain. With intrathecal therapy (ITT), drugs are delivered close to their site of actio

PURPOSE: Menstrual pain is associated with increased production of inflammatory molecules, such as prostaglandins. Inflammation is involved in pathogenesis of several cancers, including ovarian cancer. In this study, we examined the association between menstrual pain and risk of ovarian cancer.
METHODS: We conducted a case-control study with 2,028 cases of epithelial ovarian cancer and 2,091 age- and study center-matched controls. Women were asked to report the severity of menstrual pain during their twenties and thirties, when not using oral contraceptives or breastfeeding. We used an uncond

Cancerous cells can originate in a number of different tissues such as prostate, breast and lung, but often go undetected and are non-painful. Many types of cancers have a propensity to metastasize to the bone microenvironment first. Tumor burden within the bone causes excruciating breakthrough pain with properties of ongoing pain that is inadequately managed with current analgesics. Part of this failure is due to the poor understanding of the etiology of cancer pain. Animal models of cancer-induced bone pain (CIBP) have revealed that the neurochemistry of cancer has features distinctive from

Acrometastases is an unusual presentation of bone metastases, specially when they are the unique location of bone metastases. The authors report a case of a men diagnosed with a lung adenocarcinoma with a unique lytic acrometastases in his left foot. He has been treated with systemic chemotherapy with good local and systemic response. The most common sites of bone metastases are the vertebrae, pelvis, ribs, sternum and skull. Bone metastases in the distal regions of the extremities are unusual (from 0.007 to 0.3% of all patients with bone metastases), involving more frequently hands than feet.

Cancer-induced bone pain (CIBP) is a major clinical problem with up to 85% of patients with bony metastases having pain, often associated with anxiety and depression, reduced performance status, and a poor quality of life. Malignant bone disease creates a chronic pain state through sensitization and synaptic plasticity within the spinal cord that amplifies nociceptive signals and their transmission to the brain. Fifty per cent of patients are expected to gain adequate analgesia from palliative radiotherapy within 4-6 weeks of treatment. Opioid analgesia does make a useful contribution to the m

UNLABELLED: Upper-extremity pain is a common and debilitating problem for patients with breast cancer. Although there is considerable literature describing symptoms, little is available on the specific disorders responsible for pain and debility in these patients. Cervical radiculopathy, brachial plexopathy, neuropathy, rotator cuff tendonitis, adhesive capsulitis, epicondylitis, postmastectomy syndrome, swelling, and bone metastases are among the common disorders responsible for upper-extremity pain in breast cancer patients.

Combining pharmacological pain management to other therapeutic options relieves most of cancer pain. The pharmacological approach lies on the WHO analgesic three steps. The first step (paracetamol, NSAIDs) is used for weak pain, and the second (weak opioids) for moderate pain. The third one, involving strong opioids, is indicated in moderate to severe pain. If morphine is still considered the first choice, other opioids can be useful in case of unacceptable side effects, inadequate antalgy, or renal failure. The use of co-analgesics can further optimize analgesia in certain cases. Their approp
