COVID-lateral damage: cardiovascular manifestations of SARS-CoV-2 infection.

Early in the pandemic, concern that cardiovascular effects would accompany COVID-19 was fueled by lessons from the first SARS epidemic, knowledge that the SARS-COV2 entry receptor (Angiotensin-converting enzyme 2, ACE2) is highly expressed in the heart, early reports of myocarditis, and first-hand accounts by physicians caring for those with severe COVID-19. Over 18 months, our understanding of the cardiovascular manifestations has expanded greatly, leaving more new questions than those conclusively answered. Cardiac involvement is common (∼20%) but not uniformly observed in those who require

SARS-CoV-2 Infection and Cardiovascular Disease: COVID-19 Heart.

Coronavirus disease (COVID-19) is a serious illness caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The symptoms of the disease range from asymptomatic to mild respiratory symptoms and even potentially life-threatening cardiovascular and pulmonary complications. Cardiac complications include acute myocardial injury, arrhythmias, cardiogenic shock and even sudden death. Furthermore, drug interactions with COVID-19 therapies may place the patient at risk for arrhythmias, cardiomyopathy and sudden death. In this review, we summarise the cardiac manifestations of COVID-19 i

HIV Infection and Incidence of Cardiovascular Diseases: An Analysis of a Large Healthcare Database.

Background People living with HIV ( PLWH ) experience higher risk of myocardial infarction ( MI ) and heart failure ( HF ) compared with uninfected individuals. Risk of other cardiovascular diseases ( CVD s) in PLWH has received less attention. Methods and Results We studied 19 798 PLWH and 59 302 age- and sex-matched uninfected individuals identified from the MarketScan Commercial and Medicare databases in the period 2009 to 2015. Incidence of CVD s, including MI , HF , atrial fibrillation, peripheral artery disease, stroke and any CVD -related hospitalization, were identified using validated

Cardiovascular involvement and manifestations of systemic Chikungunya virus infection: A systematic review.

BACKGROUND: In the last three years, chikungunya virus disease has been spreading, affecting particularly the Americas, producing more than two million cases. In this setting, not only new disease-related epidemiological patterns have been found, but also new clinical findings have been reported by different research groups. These include findings on the cardiovascular system, including clinical, electrocardiographic and echocardiographic alterations. No previous systemic reviews have been found in major databases about it. METHODS: We performed a systematic review looking for reports about c

Use of Abacavir and Risk of Cardiovascular Disease Among HIV-Infected Individuals.

BACKGROUND: Evidence is conflicting about the association of abacavir use and cardiovascular disease (CVD) among HIV-infected individuals. Previous studies may have been biased by the preferential initiation or continuation of abacavir in patients with renal dysfunction. METHODS: We conducted a cohort study in Kaiser Permanente California during 1998-2011, following HIV-infected adults initiating antiretroviral therapy until the earliest of CVD (ie, coronary heart disease or ischemic stroke), health plan disenrollment, death, or end of study. We used inverse-probability weighting to fit margi

Cardiovascular biomarkers in vertically HIV-infected children without metabolic abnormalities.

Early cardiovascular disease is a major concern for ART-suppressed vertically HIV-infected children; however, evidence is lacking regarding specific preventive measures. In this study, a complete panel of biomarkers was determined together with carotid intima-media thickness (IMT), in a cohort of 64 HIV-infected children and 30 controls. Mean age of participants was 14.1±5 years. HIV-infected patients showed normal lipid profile, with only slightly higher triglycerides, and no differences between groups were found regarding IMT. HIV-infected patients displayed higher levels of soluble CD14 (sC

Lipoprotein-associated phospholipase A2 and cardiovascular disease risk in HIV infection.

OBJECTIVES: HIV-infected patients on antiretroviral therapy (ART) have an increased cardiovascular disease (CVD) risk as a result of heightened inflammation and immune activation, despite at times having normal lipids and few traditional risk factors. Biomarkers are needed to identify such patients before a clinical event. Lipoprotein-associated phospholipase A2 (Lp-PLA2 ) predicts CVD events in the general population. This study investigated the relationship between Lp-PLA2 and markers of CVD risk, systemic inflammation, immune activation, and coagulation in HIV infection. METHODS: One hundr

[Morbimortality of infective endocarditis associated with permanent cardiovascular implantable electronic devices].

Infective endocarditis (IE) associated with permanent cardiovascular implantable electronic devices (CIEDs) is a complication of low frequency, but high mortality without adequate treatment. Progress on the knowledge of this disease and the development of therapeutic strategies such as early diagnosis, antibiotic management and better extraction techniques, among others, have improved the prognosis of these patients. The objectives of this study were to evaluate the in-hospital and out-of-hospital morbidity, and analyze some factors that explain the differences among the published mortality da

Helicobacter pylori infection is identified as a cardiovascular risk factor in Central Africans.

BACKGROUND: Helicobacter pylori is now incriminated in the pathogenesis of atherosclerosis. OBJECTIVE: To examine the importance of H. pylori infection as a cardiovascular disease (CVD) risk factor. METHODS: Two hundred five patients (128 with H. pylori infection [HP-seropositive] and 77 without) had a baseline assessment for other potential CVD risk factors and were followed prospectively for 10 years (1999-2008). They were assessed on a monthly basis for the outcomes of carotid plaque, angina pectoris, myocardial infarction, and stroke. In the HP-seropositive group, male sex and quartile 4

Interleukin 18 and cardiovascular disease in HIV-1 infection: a partner in crime?

Cardiovascular disease has been frequent in HIV-infected patients both before and after the advent of antiretroviral therapy (HAART). The pathogenic basis for the increase of cardiovascular disease, in particular myocardial lesions, may involve HIV-1 itself or other mechanisms including endothelial dysfunction, activation of proinflammatory cytokines, and changes in platelets, which lead to atherosclerotic lesions of blood vessels. In the last decade, among the proinflammatory cytokines, interleukin 18 seems to play a central role in the inflammatory cascade, leading to development of atherosc

Highly active antiretroviral therapy and cardiovascular complications in HIV-infected patients.

Highly active antiretroviral therapy (HAART) has prolonged many patients' lives, but many cardiac sequelae of HIV are not affected by HAART and continue to develop even with treatment. In addition, HAART itself causes in a high proportion of patients a metabolic syndrome, characterized by lipodystrophy/ lipoatrophy, dyslipidemia and insulin resistance that may be associated with an increase in peripheral artery and coronary artery diseases. Careful cardiovascular evaluation in the course of HIV disease can identify cardiac complications early enough to treat. All HIV-infected patients who are

[Is there an increased risk for cardiovascular disease in HIV-infected patients on antiretroviral therapy?].

There is a growing concern about an increased risk for cardiovascular disease in HIV infected patients receiving antiretroviral therapy (ART). This risk could be related to metabolic abnormalities associated with long-term use of antiretroviral drugs. In fact, well recognized cardiovascular risk factors such as hypertension, dyslipidaemia, diabetes mellitus and central fat deposition are increasingly seen in HIV patients on ART. These factors can also be associated with non reversible risk factors, such as male sex, age greater than 40 years and family history of premature coronary artery dise

Cardiovascular and cerebrovascular events in patients treated for human immunodeficiency virus infection.

BACKGROUND: Metabolic abnormalities associated with human immunodeficiency virus (HIV) infection, including dysglycemia and hyperlipidemia, are increasingly prevalent, and there is concern about the possibility of an association with accelerated cardiovascular and cerebrovascular disease. METHODS: We conducted a retrospective study of the risk of cardiovascular and cerebrovascular disease among the 36,766 patients who received care for HIV infection at Veterans Affairs facilities between January 1993 and June 2001. RESULTS: For antiretroviral therapy, 70.2 percent of the patients received nu

Cardiovascular disease risk factors in HIV-infected patients in the HAART era.

HIV infection is accompanied by disturbances in lipid and glucose metabolism, which are further compounded by changes induced by antiretroviral drugs. There is increasing concern that these changes will lead to an epidemic of cardiovascular disease. Cardiovascular disease will no doubt increase, but current data indicate that the average absolute levels are likely to remain low, although patients with additional risks (smoking, hypertension, diabetes, age, family history, etc.) are certainly more susceptible. The complications of therapy need to be taken into account when deciding on the time

Cardiovascular and bacteremic manifestations of Campylobacter fetus infection: case report and review.

A case of bacteremia due to Campylobacter fetus subspecies fetus with concomitant pleuropericarditis in a previously healthy patient is presented. The organism is ubiquitous, but most commonly causes infection in patients with chronic underlying illnesses. The pathogenesis of human infection has not been definitively elucidated. Bacteremia is the most common clinical manifestation of this infection, although cases of thrombophlebitis, mycotic aneurysm, endocarditis, and pericarditis have also been reported. The treatment of choice for most infections is gentamicin, with chloramphenicol recomme

Cardiovascular involvement in gram-negative bloodstream infection: Timing, phenotypes, and mechanisms.

Gram-negative bloodstream infection (GN-BSI) is associated with a heterogeneous spectrum of cardiovascular manifestations extending beyond the hemodynamic consequences of sepsis. Epidemiological evidence identifies a transient period of heightened cardiovascular vulnerability, with myocardial infarction and ischemic stroke clustering close to BSI onset, alongside myocardial dysfunction, arrhythmias, thrombotic complications, and direct vascular or endovascular infection. These phenotypes likely reflect different interactions between systemic inflammation, endothelial dysfunction, immunothrombo

Pseudomonas stutzeri gastrointestinal infection in a patient with cardiovascular disease: a case report.

BACKGROUND: Pseudomonas stutzeri is a ubiquitous saprophyte and opportunistic pathogen capable of forming colonies and biofilms in various ecological niches, including soil and water. According to previous reports, this is a crucial virulence factor, enabling the bacterium to infect immunocompromised patients and individuals with a history of previous surgery, relapsed infective endocarditis after several years in patients with heart disease, trauma, skin infections, cardiovascular disease, or prosthetic devices. This report details an uncommon gastrointestinal infection resulting from P. stut

[Comment on the article "Severe coronavirus-2 infection background for cardiovascular complications"].

Current information is presented on a phenomenon that occurs in patients who had COVID-19 that could contribute to long-term cardiovascular risk. It deals with the mechanisms underlying cardiovascular and coagulation alterations that perpetuate cardiac symptoms caused by alterations induced by the virus or by myocardial inflammation, such as cardiac endothelial dysfunction, which could cause vascular microthrombosis. The intentional search for this in patients under follow-up becomes relevant since there is specific treatment and long-term complications, such as pulmonary hypertension and righ

Preparations for the Assessment of COVID-19 Infection and Long-Term Cardiovascular Risk.

Studies showing that coronavirus disease 2019 (COVID-19) is associated with an increased risk of cardiovascular disease continue to be published. However, studies on how long the overall cardiovascular risk increases after COVID-19 and the magnitude of its long-term effects have only been confirmed recently. This is partly because the distinction between cardiovascular risk as an acute complication of COVID-19 or post-acute cardiovascular manifestations is ambiguous. Long-COVID has arisen as an important topic in the second half of the pandemic. This term indicates that symptoms persist for mo
