Chickenpox complicated by pneumococcal meningitis: a rare coinfection.

Bacterial complications, particularly skin superinfections, are common during chickenpox. However, reports of acute bacterial meningitis associated with chickenpox are unusual and amount to only a very few observations. For the most part, they are caused by Neisseria meningitidis or Streptococcus pyogenes. We report an infrequent occurrence of pneumococcal meningitis 2 days after the onset of a chickenpox rash in a 7-year-old previously healthy boy. Based on data from the literature, we attempt to understand the possible mechanisms resulting in bacterial complications, particularly meningitis,…

The 1727 St Kilda epidemic: smallpox or chickenpox?

An acute infectious epidemic almost eliminated the St Kilda community in 1727. An epic tale of survival in adversity followed. Contemporary records reveal atypical features, suggesting a speculative alternative.

Necrotizing fasciitis secondary to chickenpox infection in children.

BACKGROUND: Necrotizing fasciitis is an uncommon but serious complication of chickenpox infection in young children. Because many of these infections affect the musculoskeletal tissues, orthopedic surgeons are often the first caregivers to be involved in diagnosis and treatment. Our objective was to review the diagnostic features of necrotizing fasciitis and analyze treatment methods to control and eradicate the musculoskeletal infection. DESIGN: A review. SETTING: The Children's Hospital of Eastern Ontario, Ottawa, a major Canadian pediatric trauma and referral centre. PATIENTS: Five child…

Surgical implications of necrotizing fasciitis in children with chickenpox.

Varicella (chickenpox) affects approximately 90,000 children each year. Although most cases resolve, some develop necrotizing soft tissue infections secondary to group A streptococcus and staphylococcus. Delay in diagnosis is common. At the time of initial presentation, the need for surgical intervention is not always clear. The authors conducted a retrospective review of 30 patients with varicella (seen from December 1993 to June 1995) for whom there was clinical concern for necrotizing soft tissue infection. Various parameters were examined, including tachycardia, band count, temperature, an…

Acyclovir therapy for chickenpox in children with hematological malignancies.

Nineteen immunocompromised children with varicella were given intravenous acyclovir in dosages of 1000-1500 mg/m2 per day every 8 h for 5-10 days. The effect of treatment was compared with data from a group of patients previously treated with arabinoside cytosine (ara-c). The median times to cessation of new lesion formation, lesion crusting and lesion healing were shorter in the acyclovir-treated group than for ara-c. The time-to-event probability curves for cessation of new lesion formation, lesion crusting and lesion healing were significantly different for the acyclovir and ara-c group. Pa

[Risk of chickenpox during pregnancy and the neonatal period].

A newborn fell ill from congenital chickenpox eight days after delivery, a chickenpox rash having been noted in the mother two days before delivery. Administration of zoster-hyperimmune serum to the mother at the onset of the rash and to the child immediately after birth had no demonstrable effect on the course of the disease. Autopsy, the baby having died on the eleventh day post-partum, revealed focal necrotizing pneumonia of all the lobes. Varicella-zoster virus was isolated from lung material. On the basis of present knowledge the risk of varicella infection in the fetus and neonate is 1-7…

[Problems posed by measles, chickenpox and mumps in pregnancy].

In contrast to the risks and prevention of congenital rubella which are well-known in France, the problems that may result from measles, chicken-pox and mumps occurring in the course of pregnancy remain to be delineated. Our experience, based on a long-term prospective study of mothers and full-term infants born after preventive or curative administration of immunoglobulins has led us to the following conclusions. During pregnancy, as far as mumps and chicken-pox are concerned, the risk of fetal involvement seems much lower than is usually believed; however, the numerous documented reports in

Chickenpox encephalitis and encephalopathy: evidence for differing pathogenesis.

Retrospective assessment of hepatic and central nervous system involvement associated with chickenpox cases at a large metropolitan medical center reveals that 28 of 58 patients had biochemical, but not inflammatory, evidence of liver involvement. An additional 18 patients had biochemical liver abnormalities along with non-inflammatory encephalopathy (Reye syndrome) and 12 had clear evidence of central nervous system inflammatory involvement (encephalitis). There were no cases of solitary inflammatory liver involvement. Reviewed evidence suggests that the pathogenesis of hepatopathy and hepato…

A cluster of cases of Reye syndrome associated with chickenpox.

In the spring of 1980, four confirmed, and three possible cases of Reye syndrome, each associated with a chickenpox prodromal illness, were identified in Las Cruces, NM. One patient, a 5-year-old girl, died. Cases tended to occur in one section of the community, among children attending two of the 16 elementary schools. A telephone survey of parents with school-aged children (6 to 9 years old) suggested that a significantly greater attack rate of chickenpox occurred in children attending those two schools. Based upon this survey and the expected distribution of chickenpox among children less t…

Chickenpox and multiple sclerosis: A Mendelian randomization study.

Observational studies have suggested a suspected association between varicella-zoster virus (VZV) infection and multiple sclerosis (MS), but the connection has remained unclear. The aim of the present study is to evaluate the causal relationship between chickenpox which is caused by VZV infection and MS. We performed a two-sample Mendelian randomization analysis to investigate the association of chickenpox with MS using summary statistics from genome-wide association studies (GWAS). The GWAS summary statistics data for chickenpox was from the 23andMe cohort including 107 769 cases and 15 982 c…
