Allergic Contact Dermatitis with Diffuse Erythematous Reaction from Diisopropanolamine in a Compress.

Compresses containing a nonsteroidal antiinflammatory drug (NSAID) are commonly used in Japan. However, this treatment may induce both allergic and photoallergic contact dermatitis from the NSAIDs and their ingredients. Here, we describe a case of allergic contact dermatitis with diffuse erythematous reaction due to diisopropanolamine in the applied compress. The absorption of diisopropanolamine might have been enhanced by the occlusive condition.

[Photoallergic eczema caused by musk ambrette].

This study reports a photoallergic contact dermatitis to musk ambrette in two male patients. Musk ambrette is a widely used perfume ingredient, but it is also included in food and drugs. Both patients reacted in the long ultraviolet light (UV-A) to musk ambrette and to the after shave lotions containing musk ambrette. The histopathologic finding in the photopatch test site revealed acute spongiotic dermatitis.

Five cases of photocontact dermatitis due to topical ketoprofen: photopatch testing and cross-reaction study.

BACKGROUND: In parallel with the popular usage of topical ketoprofen, the number of reported cases of ketoprofen-induced photoallergic contact dermatitis has been increasing. It is clinically important to know the cross-reactivity of ketoprofen in order to avoid cross-sensitization caused by several structurally similar non-steroidal anti-inflammatory drugs (NSAID) on the market. METHODS: To evaluate the spectrum of cross sensitization, photopatch testing was performed on five patients with ketoprofen-induced photoallergic contact dermatitis using ketoprofen and other structurally similar che

Gruppo Italiano Ricerca Dermatiti da Contatto ed Ambientali Italian Multicenter Study of Allergic Contact Photodermatitis: epidemiological aspects.

A total of 1,050 patients with histories and clinical pictures suggestive of photoallergic contact dermatitis were seen. All the patients underwent photopatch tests with haptens proposed by the Gruppo Italiano Ricerca Dermatiti da Contatto plus other substances suggested by each patient's history. Two hundred fifty-nine patients (24.6%) were positive to at least one test substance of the standard series or to substances added. Typical photoallergic reactions were seen in 198 subjects (259 minus 40 toxic photodermatitis to chlorpromazine and 21 to promethazine), which represents 18.8% of the to

Differentiation of hydrochlorothiazide-induced dermatitis from stasis dermatitis.

A woman in her 60s with a history of hypertension and stasis dermatitis presented to a primary care clinic with a bilateral, erythematous rash on the legs, stomach, and chest. Photosensitive rash and dermatitis may be caused by many conditions. Hydrochlorothiazide-induced dermatitis is a rare side effect of thiazide diuretics. Early identification of sulfa-sensitivity and photoallergic or phototoxic reaction is essential to accurate diagnosis and treatment of photosensitive dermatitis. Soliciting a targeted history is essential to delineating drug-induced dermatitis from stasis dermatitis. A t

[UV radiation as occupational environmental damage (author's transl)].

The authors treat the problem of the skin damages produced by the UV-rays as an occupational hazard. An antiopathogenetic parallel with the other light dermatoses is discussed. 6836 cases with solar keratoses and 3816 with skin cancer are described; 47% of solar keratosis and 43% with skin cancer can be related to the occupational environment. 160 cases with "Kusbas"-Dermatitis, 20 cases with toxic melanodermitis, six with photoallergic dermatitis caused by largactyl and nine with solar urticaria are discussed in connection with their pathogenesis.

Contact reactions to food.

Cutaneous adverse reactions to foods, spices, and food additives can occur both in occupational and nonoccupational settings in those who grow, handle, prepare, or cook food. Because spices are also utilized in cosmetics and perfumes, other exposures are encountered that can result in adverse cutaneous reactions. This article describes the reaction patterns that can occur upon contact with foods, including irritant contact dermatitis and allergic contact dermatitis. The ingestion of culprit foods by sensitized individuals can provoke a generalized eczematous rash, referred to as systemic conta

How to instruct patients sensitive to fragrances.

Patients who are sensitive to fragrances should either use fragrance-free cosmetics or undergo a repeat open application test to the cosmetic or perfume to determine sensitivity. Unusual reactions include systemic contact dermatitis due to balsam of Peru, benzyl alcohol, and menthol. Some responses involve pigmented eruptions due to phototoxic or photoallergic agents in perfumes and incense. Other reactions include consort dermatitis and reactions to toothpastes, gum and perfumes in paper products, sanitary napkins, ostomy pastes, and detergents.

Chronic actinic dermatitis: a retrospective analysis of 44 cases referred to an Australian photobiology clinic.

A retrospective study was performed to analyse the clinical and photobiological features and therapeutic outcomes of 44 patients with chronic actinic dermatitis who were evaluated over an 8.3-year period. The study population comprised 37 men and seven women with a mean age of 62.7 years (range 26-85 years). The most common abnormal phototest results were decreased minimal erythema doses to both UVA and -B (73.8%), and to UVA alone (14.3%). Twenty-six patients (78.8%) had at least one allergic, photoallergic or combined allergic/photoallergic reaction. A total of 139 positive contact or photoc

Photoallergic reaction in a patient receiving vandetanib for metastatic follicular thyroid carcinoma: a case report.

BACKGROUND: Novel targeted agents have been increasingly developed and tested in clinical trials over the past 5-10 years, many with unknown and unanticipated side effects. We describe here a case of a patient with a history of metastatic follicular thyroid carcinoma that we believe developed vandetanib-associated photoallergic dermatitis while enrolled on a phase 1 clinical trial. CASE PRESENTATION: A 51-year-old Caucasian female with poorly differentiated, metastatic follicular thyroid carcinoma presented with a cutaneous eruption that developed over 3 to 4 days while treated on phase 1 cli

An attempt to reduce the photosensitizing potential of chlorpromazine with the simultaneous use of beta- and dimethyl-beta-cyclodextrins in guinea pigs.

The effects of topically applied beta-cyclodextrin (beta-CyD) and heptakis(2,6-di-O-methyl)-beta-cyclodextrin (DM-beta-CyD) on photoallergic contact dermatitis due to chlorpromazine hydrochloride (CPZ) were investigated using the photomaximization technique in guinea pigs. From the gross and histological observations, the photosensitizing potential of CPZ with the simultaneous topical use of beta-CyDs was significantly lower than that of CPZ alone. The alleviating efficacy of DM-beta-CyD was greater than that of beta-CyD. beta-CyDs suppressed the penetration of CPZ into the skin through the fo

Psoralen photoallergy caused by plant contact.

A case of acquired photocontact allergy to furocumarins in plants is reported. Photopatch testing was performed with four psoralens [8-methoxypsoralen (8-MOP), 5-methoxypsoralens (5-MOP), trimethylpsoralen (TMP) and imperatorin (IMP)[. The use of serial dilutions of the test compounds made it possible to differentiate between photoallergic and phototoxic reactions. 8-MOP gave a positive eczematous test reaction down to a concentration of 0.0001%. The reactions to 5-MOP and imp also were positive, while that to TMP awas negative. Histopathological examination of a biopsy specimen from a positiv

Photocontact dermatitis.

Photocontact dermatitis is not a common condition, but neither is it rare. Both photo-irritant contact dermatitis (PICD) and photoallergic contact dermatitis (PACD) are seen by most dermatologists in general practice. PICD is diagnosed on clinical grounds and is usually caused by furocoumarins in plants like limes and celery. PACD is caused primarily by sunscreens but can also be the result of fragrances and antibacterial agents. PACD can only be diagnosed by photo-patch testing that most dermatologists, even those who patch test and give phototherapy in their office, do not perform. The proce

[Photocontact allergy caused by benzydamine (Tantum)].

Three women with a photoallergic dermatitis precipitated by benzydamine are reported. In one patient the cause of the photosensitization was a cream containing benzydamine (Tantum cream), whereas in the other two the reaction occurred after oral administration (Tantum drops). In all three cases the photosensitization was confirmed by a photopatch test that was positive for the active ingredient benzydamine.

First reported case of tenofovir-induced photoallergic reaction.

A 50-year-old man, a known case of human immunodeficiency virus infection for the past 1 year, was on antiretroviral therapy in the form of stavudine, lamivudine, and nevirapine. Three days after replacing stavudine with tenofovir, he developed redness on the face and neck and within 48 h the rash became generalized. Dermatological examination revealed involvement of photoexposed areas of the face in the form of erythema and ill-defined hyperpigmented plaques, with mild periorbital edema. There was specific involvement of V and nape of the neck. Extensive erythema and scaling were also present
