Summating potentials in Menière's disease.

The summating potentials in response to tonebursts of 2, 4, and 8 kHz were studied in a group of 112 patients diagnosed as having Menière's disease, and the results were compared with recordings from 27 normal human ears. Subdivision of the Menière ears into a low-threshold (threshold less than 50 dB HL) and a high-threshold group (greater than 50 dB HL) proved to be functional in relation to the behaviour of the SP. The data of the low-threshold Menière group did not differ significantly from the normal data, whereas the high-threshold Menière group showed significantly smaller summating pote

Acoustical stress and hearing sensitivity in fishes: does the linear threshold shift hypothesis hold water?

Mammals exposed to loud aerial sounds exhibit temporary threshold shifts (TTS) that are linearly related to increases of sound pressure above baseline hearing levels. It was unknown if this relationship held true for aquatic ectotherms such as fishes. To test this linear threshold shift hypothesis (LINTS) in fishes, we examined the effects of increased ambient sound on hearing of two species differing in hearing capabilities: goldfish (Carassius auratus; a hearing specialist) and tilapia (Oreochromis niloticus; a hearing generalist). Fish were exposed to 1-28 days of either quiet (110 dB re 1

[Audiological observations of a patient with auditory agnosia].

A case is reported in which a patient was become unable to identify all kinds of auditory material, including speech, despite only subnormal audiometric thresholds (i.e. "auditory agnosia"). The following audiological disorders were shown: some fragility in the perception of brief sound stimuli, difficulties to apprehending prolonged series of stimuli (rhythms), defective sound localization, an increase in the homolateral and in the contralateral effects of masking sounds. On the other hand, differential thresholds for frequency and for intensity, as well as fusion thresholds, were little affe…

Tactile and proprioceptive dysfunction differentiates cervical dystonia with and without tremor.

OBJECTIVE: To determine whether different phenotypes of cervical dystonia (CD) express different types and levels of somatosensory impairment. METHODS: We assessed somatosensory function in patients with CD with and without tremor (n = 12 each) and in healthy age-matched controls (n = 22) by measuring tactile temporal discrimination thresholds of the nondystonic forearm and proprioceptive acuity in both the dystonic (head/neck) and nondystonic body segments (forearm/hand) using a joint position-matching task. The head or the wrist was passively displaced along different axes to distinct joint

Temporal predictions based on a gradual change in tempo.

Previous studies investigating sensitivity to step changes in tempo and prediction of tone onset time have generally utilized isochronous sequences. This study investigates subjects' ability to detect deviations from a gradual change in the tempo of a tone sequence (experiment 1) and their judgment of the perceptually optimal timing of this tone (experiment 2). In experiment 1, inter-onset-intervals within pairs of eight-tone sequences followed a geometric progression to create a gradual tempo change. In one sequence, the final tone was presented either earlier or later than specified by the p

Intracellular Vesicle Transport Impairment as a Candidate Systems-Level Bottleneck in Chronic Diabetic Foot Ulcers: Network Medicine Identifies KIF13A as a Potential Therapeutic Vulnerability.

Background: Growth factor therapy often fails in diabetic foot ulcers (DFUs). The reason remains unclear. Standard differential expression analysis may miss functionally critical genes with modest expression changes. Methods: We performed a secondary computational analysis of a longitudinal DFU transcriptomic dataset (Dryad; 17 patients, 117 serial biopsy samples, 12-week follow-up). Co-expression networks were built separately for healed (n = 37) and non-healed (n = 80) samples. Virtual gene knockout (VGK) was used to rank genes by topological impact on network cohesion. Single-cell analysis

Can local vibration alter the contribution of persistent inward currents to human motoneuron firing?

The response of spinal motoneurons to synaptic input greatly depends on the activation of persistent inward currents (PICs), which in turn are enhanced by the neuromodulators serotonin and noradrenaline. Local vibration (LV) induces excitatory Ia input onto motoneurons and may alter neuromodulatory inputs. Therefore, we investigated whether LV influences the contribution of PICs to motoneuron firing. This was assessed in voluntary contractions with concurrent, ongoing LV, as well as after a bout of prolonged LV. High-density surface electromyograms (HD-EMG) of the tibialis anterior were record

Tinnitus and normal hearing: a study on contralateral acoustic reflex.

PURPOSE: The aim of this study was to evaluate the difference of the contralateral acoustic reflex (AR) threshold between adult subjects with hearing within clinically normal limits, with and without tinnitus. METHOD: The study sample in this exploratory, descriptive, and comparative study comprised 40 female subjects who were evaluated: 20 had tinnitus and 20 formed the control group. The contralateral AR threshold was evaluated at the frequencies of 500, 1000, and 2000 Hz. RESULTS: Elevated or absent AR thresholds were found only in subjects with complaints of tinnitus. At all frequencies

Pacing threshold testing induced ventricular fibrillation following acute rate control of atrial fibrillation.

BACKGROUND: A properly placed stimulus on the T-wave during ventricular repolarization can result in ventricular fibrillation (VF). Initiation of VF with pacing on T-wave is a rare event with a few reported cases in the literature. We present a unique case of induced VF attributed to a pacing stimulus on T-wave during ventricular pacing threshold testing of a permanent pacemaker. CASE REPORT: A 64-year-old woman with persistent atrial fibrillation (AF) and a permanent pacemaker for tachycardia-bradycardia syndrome presented with symptomatic AF with rapid ventricular response. Acute rate contr

The wavelength of light causing photophobia in migraine and tension-type headache between attacks.

OBJECTIVE: To ascertain the wavelength of light that patients with migraine and tension-type headache find uncomfortable between attacks. BACKGROUND: Photophobia is an abnormal perceptual sensitivity to light experienced by most patients with headache during and, also, between attacks. METHODS: We examined the discomfort threshold to light of low, medium, and high wavelengths in a group of patients with migraine (n=21), patients with tension-type headache (n=19), and healthy controls (n=21). RESULTS: The results indicate that the migraine group had significantly lower discomfort thresholds

Impact of trophoblast penetration through the basal membrane on the efficacy of drug therapy in tubal pregnancies.

Concentrations of beta-human chorionic gonadotrophin (HCG) of 2500 IU/l are generally considered to be maximal for successful drug therapy of tubal pregnancies [instillation of prostaglandin-F2 alpha (PGF2 alpha) or hyperosmolar glucose]. The purpose of our study was to ascertain if there was an association between the significantly higher failure rates above this threshold value and the histologically determined anatomopathological substratum. We therefore evaluated the impact of trophoblast penetration through the basal membrane of the Fallopian tube on the efficacy of drug therapy. Pre-oper

Tuning and suppression in auditory nerve fibers of aged gerbils raised in quiet or noise.

Mongolian gerbils were reared either in quiet or in a continuous noise field (85 dBA, 500-4000 Hz). The gerbils began the noise exposure at 8 months of age and, after the exposure, spent the remainder of their lives in the quiet vivarium with the quiet-aged group. The duration of the noise exposure was between 365 and 724 days. At the terminal experiment the ages of the animals varied between 24 and 43 months, with a mean age of about 36 months, an age representing the average life span of a gerbil in our colony. During the terminal experiment, tuning curves and boundaries of two-tone rate sup

A sleep-induced apneic threshold and its consequences.

We have emphasized the mechanisms and consequences of sleep state effects on the manifestation of a sensitive apneic threshold. In the absence of the stabilizing influences of wakefulness, even the healthy person is vulnerable to instabilities and ventilatory control as maintenance of a rhythmic breathing pattern becomes overwhelmingly dependent on CO2. This sleep-induced unmasking of the depressant effects of hypocapnia contrasts with the relatively minor effects of sleep on the ventilatory response to a wide variety of other acute or chronic ventilatory stimuli or inhibitors. This combinatio

Therapeutic value of nerve growth factor in promoting neural stem cell survival and differentiation and protecting against neuronal hearing loss.

Nerve growth factor (NGF) is a neurotrophic factor that modulates survival and differentiation of neural stem cells (NSCs). We investigated the function of NGF in promoting growth and neuronal differentiation of NSCs isolated from mouse cochlear tissue, as well as its protective properties against gentamicin (GMC) ototoxicity. NSCs were isolated from the cochlea of mice and cultured in vitro. Effect of NGF on survival, neurosphere formation, and differentiation of the NSCs, as well as neurite outgrowth and neural excitability in the subsequent in vitro neuronal network, was examined. Modeling the suppression dynamics of Aedes mosquitoes with mating inhomogeneity.

A novel strategy for controlling mosquito-borne diseases, such as dengue, malaria and Zika, involves releases of Wolbachia-infected mosquitoes as Wolbachia cause early embryo death when an infected male mates with an uninfected female. In this work, we introduce a delay differential equation model with mating inhomogeneity to discuss mosquito population suppression based on Wolbachia. Our analyses show that the wild mosquitoes could be eliminated if either the adult mortality rate exceeds the threshold [Formula: see text] or the release amount exceeds the threshold [Formula: see text] uniforml

Models of bovine babesiosis including juvenile cattle.

Bovine Babesiosis in cattle is caused by the transmission of protozoa of Babesia spp. by ticks as vectors. Juvenile cattle (<9 months of age) have resistance to Bovine Babesiosis, rarely show symptoms, and acquire immunity upon recovery. Susceptibility to the disease varies between breeds of cattle. Models of the dynamics of Bovine Babesiosis transmitted by the cattle tick that include these factors are formulated as systems of ordinary differential equations. Basic reproduction numbers are calculated, and it is proved that if these numbers are below the threshold value of one, then Bovine Bab

Different TMS patterns of intracortical inhibition in early onset Alzheimer dementia and frontotemporal dementia.

OBJECTIVE: To investigate putative changes in cortical excitability of patients affected by early-onset mild dementia by means of transcranial magnetic stimulation (TMS) and to verify whether a peculiar neurophysiological profile may contribute to characterise Alzheimer's disease (AD) vs frontotemporal dementia (FTD). METHODS: Motor threshold and intracortical inhibition (ICI) and facilitation (ICF) after paired-pulse TMS (inter-stimulus intervals from 1 to 20 ms) were studied in two groups of early-onset demented patients with a neuropsychological profile suggestive of AD (n = 12) and FTD (n

Implantable cardioverter defibrillator with and without defibrillation threshold testing.

BACKGROUND: Defibrillation threshold (DFT) testing at the time of implantable cardioverter defibrillator (ICD) insertion is performed routinely. This practice is being reconsidered due to doubts about its ability to improve ICD efficacy and evidence that survival may not be affected by the test. OBJECTIVES: To compare the outcome of ICD recipients who underwent DFT testing and those who did not. METHODS: A total of 213 eligible patients were implanted with an ICD between 2004 and 2009. DFT testing was performed in 80 of them. We compared total mortality, appropriate and inappropriate ICD sho

Sensitivity to interaural time differences with combined cochlear implant and acoustic stimulation.

The interaural time difference (ITD) is an important cue to localize sound sources. Sensitivity to ITD was measured in eight users of a cochlear implant (CI) in the one ear and a hearing aid (HA) in the other severely impaired ear. The stimulus consisted of an electric pulse train of 100 pps and an acoustic filtered click train. Just-noticeable differences (JNDs) in ITD were measured using a lateralization paradigm. Four subjects exhibited median JNDs in ITD of 156, 341, 254, and 91 mus; the other subjects could not lateralize the stimuli consistently. Only the subjects who could lateralize ha

Accuracy of the central vein sign in diagnosis of multiple sclerosis: a systematic review and meta-analysis.

BACKGROUND: The central vein sign (CVS) is a promising imaging marker of multiple sclerosis (MS). We performed a systematic review and meta-analysis to evaluate the diagnostic accuracy of CVS-based rules in the differential diagnosis of MS and to identify the best cutoffs for these rules. METHODS: PubMed, Embase, and Scopus were systematically searched for available evidence. Data extracted were entered into Bayesian models recommended by the Cochrane network. Summary sensitivity and specificity of CVS-based diagnostic rules across different positivity thresholds were calculated. A meta-regre
