Enhancing gesture decoding performance using signals from posterior parietal cortex: a stereo-electroencephalograhy (SEEG) study.

OBJECTIVE: Hand movement is a crucial function for humans' daily life. Developing brain-machine interface (BMI) to control a robotic hand by brain signals would help the severely paralyzed people partially regain the functional independence. Previous intracranial electroencephalography (iEEG)-based BMIs towards gesture decoding mostly used neural signals from the primary sensorimotor cortex while ignoring the hand movement related signals from posterior parietal cortex (PPC). Here, we propose combining iEEG recordings from PPC with that from primary sensorimotor cortex to enhance the gesture d

The Tale of the Residual Tail: Insights From Continuous Intracranial Monitoring From Post-Hippocampectomy Dynamics.

Subtotal hippocampal resection can leave residual hippocampal tissue, yet the immediate postoperative electrophysiologic evolution of such remnants is unknown. We describe a patient with drug-resistant temporal lobe epilepsy in whom a hippocampal remnant was continuously monitored using a responsive neurostimulator (RNS) following subtotal hippocampal resection. Postoperative electrocorticography demonstrated persistent electrographic status epilepticus for 3 days, followed by a transient 5-day quiescent period and delayed recurrence of electrographic and clinical seizures on postoperative day

Robust time delay estimation for measurement of ictal EEG traveling waves.

Objective.Seizures are often characterized by epileptiform discharges that appear to be highly synchronous across electroencephalogram (EEG) channels, yet closer analysis reveals small time delays indicative of fast traveling waves. Microelectrode studies suggest that a possible source of these waves is the ictal wavefront, a slowly advancing boundary emitting opposing waves; attempts to extend this analysis to macroelectrodes produced conflicting theories, suggesting both static and moving radial sources. Because variability in EEG measurement and analysis may contribute to conflicting result

Cortical evolutionary lineage as an organising principle for seizure semiology: implications for intracranial EEG and presurgical hypothesis generation.

BACKGROUND: Seizure semiology is usually interpreted through two main frameworks: anatomical localisation and network dynamics. Both are indispensable, but neither fully explains why certain cortical regions consistently produce particular classes of ictal phenomena, or why the same anatomical structure can generate different semiologies across seizures. PURPOSE: To examine cortical evolutionary lineage as a complementary level of explanation for seizure semiology, and to explore its implications for intracranial EEG, presurgical hypothesis generation, and epilepsy surgery. METHODS: This nar

Integration of epilepsy surgery and automatic seizure onset identification algorithm based on seizure patterns using SEEG.

Accurate identification of an epileptogenic zone (EZ) is crucial for successful epilepsy surgery. Stereotactic intracranial electroencephalography (SEEG) has contributed to improving surgical outcomes through assessing epileptic networks prior to surgery. However, SEEG data analysis has primarily relied on visual interpretation of epileptologists and clinical features of seizure types, leading to challenges in objective and quantitative surgical planning because it is difficult to visually evaluate High-Frequency Oscillation (HFO) or other epilepsy-related waveforms. Although the Epileptogenic

Causal Connectivity Network Analysis of Ictal Electrocorticogram With Temporal Lobe Epilepsy Based on Dynamic Phase Transfer Entropy.

Temporallobe epilepsy (TLE) has been conceptualized as a brain network disease, which generates brain connectivity dynamics within and beyond the temporal lobe structures in seizures. The hippocampus is a representative epileptogenic focus in TLE. Understanding the causal connectivity in terms of brain network during seizures is crucial in revealing the triggering mechanism of epileptic seizures originating from the hippocampus (HPC) spread to the lateral temporal cortex (LTC) by ictal electrocorticogram (ECoG), particularly in high-frequency oscillations (HFOs) bands. In this study, we propos

Brain-Computer Interface: Applications to Speech Decoding and Synthesis to Augment Communication.

Damage or degeneration of motor pathways necessary for speech and other movements, as in brainstem strokes or amyotrophic lateral sclerosis (ALS), can interfere with efficient communication without affecting brain structures responsible for language or cognition. In the worst-case scenario, this can result in the locked in syndrome (LIS), a condition in which individuals cannot initiate communication and can only express themselves by answering yes/no questions with eye blinks or other rudimentary movements. Existing augmentative and alternative communication (AAC) devices that rely on eye tra

Ripples reflect a spectrum of synchronous spiking activity in human anterior temporal lobe.

Direct brain recordings have provided important insights into how high-frequency activity captured through intracranial EEG (iEEG) supports human memory retrieval. The extent to which such activity is comprised of transient fluctuations that reflect the dynamic coordination of underlying neurons, however, remains unclear. Here, we simultaneously record iEEG, local field potential (LFP), and single unit activity in the human temporal cortex. We demonstrate that fast oscillations within the previously identified 80-120 Hz ripple band contribute to broadband high-frequency activity in the human c

Reflection of the Ictal Electrocorticographic Discharges Confined to the Medial Temporal Lobe to the Scalp-Recorded Electroencephalogram.

Objective: Previous reports on the simultaneous recording of electroencephalography (EEG) and electrocorticography (ECoG) have demonstrated that, in patients with temporal lobe epilepsy (TLE), ictal ECoG discharges with an amplitude as high as 1000 μV originating from the medial temporal lobe could not be recorded on EEG. In contrast, ictal EEG discharges were recorded after ictal ECoG discharges propagated to the lateral temporal lobe. Here, we report a case of TLE in which the ictal EEG discharges, corresponding to ictal ECoG discharges confined to the medial temporal lobe, were recorded. Ca

Fully implanted adaptive deep brain stimulation in freely moving essential tremor patients.

OBJECTIVE: Deep brain stimulation (DBS) is a safe and established treatment for essential tremor (ET) and several other movement disorders. One approach to improving DBS therapy is adaptive DBS (aDBS), in which stimulation parameters are modulated in real time based on biofeedback from either external or implanted sensors. Previously tested systems have fallen short of translational applicability due to the requirement for patients to continuously wear the necessary sensors or processing devices, as well as privacy and security concerns. APPROACH: We designed and implemented a translation-rea

Shared latent representations of speech production for cross-patient speech decoding.

Speech brain-computer interfaces (BCIs) can restore communication in individuals with neuromotor disorders who are unable to speak. However, current speech BCIs limit patient usability and successful deployment by requiring large volumes of patient-specific data collected over long periods of time. A promising solution to facilitate usability and accelerate their successful deployment is to combine data from multiple patients. This has proven difficult, however, due to differences in user neuroanatomy, varied placement of electrode arrays, and sparse sampling of targeted anatomy. Here, by alig

Mapping the migraine: Intracranial recording of cortical spreading depression in migraine with aura.

Migraine with aura is a common neurological disorder. Cortical spreading depression is the hypothetical pathophysiologic correlate, but it has never been directly recorded during migraine in a human. A 32-year-old female with a history of migraine underwent a presurgical workup for comorbid epilepsy. While implanted for stereotactic electroencephalography with 94 electrode contacts across bilateral hemispheres, she experienced a typical migraine with aura. Stereotactic electroencephalography demonstrated low-voltage suppression starting in the left mesial occipital cortex and propagating anter

Open multi-center intracranial electroencephalography dataset with task probing conscious visual perception.

We introduce an intracranial EEG (iEEG) dataset collected as part of an adversarial collaboration between proponents of two theories of consciousness: Global Neuronal Workspace Theory and Integrated Information Theory. The data were recorded from 38 patients undergoing intracranial monitoring of epileptic seizures across three research centers using the same experimental protocol. Participants were presented with suprathreshold visual stimuli belonging to four different categories (faces, objects, letters, false fonts) in three orientations (front, left, right view), and for three durations (0

Task-invariant anterior-to-posterior large-scale phase gradients of electrocorticogram oscillations during picture naming and wrist extension.

Electrocorticogram (ECoG) recordings during rest have shown that theta and alpha oscillations on the cortical surface form "cortical traveling waves" across large-scale brain networks, known as phase gradients. Theoretically, these large-scale phase gradients are thought to contribute to asymmetric coupling among cortical regions. During cognitive tasks, such large-scale gradients presumably support flexible information transfer between task-related cortices; however, this has not been extensively evaluated. Here, we examined the large-scale phase gradients of ECoG theta-alpha (7 Hz) oscillati

Interictal Functional Connectivity in Focal Refractory Epilepsies Investigated by Intracranial EEG.

Introduction: Focal epilepsies are diseases of neuronal excitability affecting macroscopic networks of cortical and subcortical neural structures. These networks ("epileptogenic networks") can generate pathological electrophysiological activities during seizures, and also between seizures (interictal period). Many works attempt to describe these networks by using quantification methods, particularly based on the estimation of statistical relationships between signals produced by brain regions, namely functional connectivity (FC). Results: FC has been shown to be greatly altered during seizures

Intracranial electroencephalography features of young and old mice under midazolam administration.

Understanding the electroencephalography features of young and old patients treated with anesthetic drugs is important to allow accurate drug use in elderly patients. This study aimed to monitor the intracranial electroencephalography (in the cortex and hippocampus) in free-moving young and old mice under midazolam administration. Behavioral assessment revealed that compared with young mice, old mice had a longer immobility time with a similar midazolam dose. In both young and old mice, midazolam significantly suppressed the total, δ (0.5-4 Hz), θ (4-8 Hz), and α (8-12 Hz) power, and thus indu

EMD-Based, Mean-Phase Coherence Analysis to Assess Instantaneous Phase-Synchrony Dynamics in Epilepsy Patients.

In this paper, an adaptive, non-linear, analytical methodology is proposed in order to quantitatively evaluate the instantaneous phase-synchrony dynamics in epilepsy patients. A group of finite neuronal oscillators is extracted from a multichannel electrocorticographic (ECoG) data, using the empirical mode decomposition (EMD). The instantaneous phases of the extracted oscillators are measured using the Hilbert transform in order to be utilized in the mean-phase coherence analysis. Finally, the dynamical evolution of phase-synchrony among the extracted neuronal oscillators within 1-600 Hz frequ

Focal extratemporal epilepsy: clinical features, EEG patterns, and surgical approach.

The objective of this review is a summary of the clinical and electrographic findings in those forms of epilepsy to which the term 'extratemporal' (ExT) can be applied. They form a group that differs in many ways from the better known temporal lobe epilepsies. Seizure foci are difficult to localize by clinical semiology alone but modern imaging now often allows a precise definition of the epileptogenic area. The most common causes of ExT epilepsy are tumors and cortical dysgenesis. The concept of 'dual pathology' implies the coexistence of two or more distinct lesions, typically mesial tempora
