Multi-antibiotic resistant Gram-positive cocci, which include Staphylococcus aureus, the coagulase-negative staphylococcal group, Enterococcus faecalis and Enterococcus faecium, and other streptococci, represent emerging pathogens especially in the setting of the immunocompromised, hospitalized patients, in particular when surgery, invasive procedures, or prosthetic implants are of concern, patients are admitted in intensive care units, or underlying chronic disorders and immunodeficiency are of concern, and broad-spectrum antibiotics or immunosuppressive drugs are widely administered. During …

Despite significant advances in antimicrobial therapy and an enhanced ability to diagnose and treat complications, infective endocarditis is … pproaches need to be developed for the treatment of infective endocarditis caused by drug-resistant Gram-positive cocci, and some antimicrobial compounds recently introduced in clinical practice (such as streptogramins and oxazolidinones) may be an effective alternative, but further clinical studies are needed in order to confirm their effectiveness and safety. The effect of cigarette smoke on growth of those species of bacteria that are considered common potential human pathogens was examined in vitro. Smoke from both mentholated and nonmentholated cigarettes inhibited the growth of Gram-positive cocci to a greater degree than that of Gram-negative rods. Staphylococcus aureus, Streptococcus pneumoniae, and a variety of other streptococci were inhibited at a smoke solution dilution of 1:8. Enteric bacteria such as Klebsiella, Enterobacter, and Pseudomonas were not affected by a 1:1 dilution of the solution. As with the Gram-positive cocci, the Neisse…

Oral norfloxacin prevented Gram-negative bacilluria in female patients with hip fractures, who needed medium-term transurethral catheterization. This was shown in a placebo-controlled double-blind study of 34 patients. Seventeen of these received a suspension containing 200 mg norfloxacin and 500 mg amphotericin B, twice daily. In the placebo group, six cases of Gram-negative bacilluria had occurred by day 7, as compared with no cases during a median time of catheterization of 23 days in the group on medication. Bacteriuria, either by Gram-positive cocci or by Gram-negative bacilli, was observ…

A total of 18,639 clinical isolates in 19 species collected from 77 centers during 2004 in Japan were tested for their susceptibility to fluoroquinolones (FQs) and other selected antibiotics. The common respiratory pathogens, Streptococcus pyogenes, Streptococcus pneumoniae, Moraxella catarrhalis and Haemophilus influenzae showed a high susceptible rate against FQs. The isolation rate of beta lactamase non-producing ampicillin-resistant H. influenzae was approximately three times as large as those of western countries. Most strains of Enterobacteriaceae were also susceptible to FQs. The resist

The spectrum of activity and potency of doripenem, a broad-spectrum parenteral carbapenem currently in clinical development, was evaluated using 16 008 clinical bacterial isolates collected as part of an international surveillance project during 2003. Using reference broth microdilution methods, doripenem was found to be highly active against oxacillin-susceptible Staphylococcus aureus and coagulase-negative staphylococci (2705 and 297 isolates, respectively; MIC90s 0.06 mg/L), with a potency greater than that of other carbapenem antibiotics. Against enterococci (1474 isolates), with the excep

Sputum culture of patients at Siriraj Hospital, Bangkok was 49.84% positive for bacterial pathogens in 1994 and 40.95% in 1995. The average incidence of gram-negative rods was 3.11 fold more than the combination of gram-positive cocci and gram-negative cocci. The most common gram-negative rod was Pseudomonas aeruginosa, followed by either Klebsiella pneumoniae or Acinetobacter anitratus depending on year. The most common coccus was Staphylococcus aureus. From both years, the number of Haemophilus influenzae, Streptococcus pneumoniae, Burkholderia pseudomallei and Nocardia spp isolated were 122…

METHODS: Retrospective study of 111 eyes from 105 patients hospitalized from 2005 to 2006 for bacterial keratitis proven by microbiological assessment or suspected (favorable outcome after antibiotic treatment).
RESULTS: The main risk factors were contact lens wear (39.6%), ocular surface diseases (36.9%), a history of ocular surgery (27.9%), and ocular trauma (11.7%). Gram-positive cocci were found in 46.8% of cases, Gram-negative bacilli in 19.8%, Gram-positive bacilli in 7.2%, Gram-negative cocci in 2.7%…

Anaerobic bacteremia (116 cases) represented 5.4% of the total cases of bacteremia in the Hospital Nuestra Señora de Covadonga of Oviedo, Spain, during a five-year period (1981-1985). Microbiologic data for all 116 cases and clinical data for 63 patients were analyzed. A total of 129 isolates were identified as gram-negative bacilli (45.7%), gram-positive bacilli (38.0%), gram-positive cocci (14.0%), and gram-negative cocci (2.3%). Bacteroides fragilis and Clostridium perfringens were the most frequently occurring species. Anaerobic polymicrobial infection was detected in 21 patients. The most…

Three strains of vancomycin-resistant Gram-positive cocci, belonging to the family Streptococcaceae, were isolated from patient samples. Two were identified as Leuconostoc species, the other one as Enterococcus (Streptococcus) faecium. The clinical significance of vancomycin-resistant Gram-positive bacteria is discussed.

The in vitro activity of cefpirome (CPR) was compared with that of cefazolin (CEZ), cefotiam (CTM), flomoxef (FMOX), cefotaxime (CTX), ceftazidime (CAZ) and imipenem/cilastatin (IPM/CS) against 384 clinical isolates, for tested by the both microdilution method. The activity of CPR against Staphylococcus aureus (MIC50, 1 microgram/ml) and Enterococcus faecalis (MIC50, 8 micrograms/ml) was better than CTX and CAZ, but slightly less than IPM/CS. CPR and other antibacterial agents were not effect against methicillin resistant S. aureus and Enterococcus faecium. The activity of CPR against Enteroba

The correlation between thermal sensitivity and the microorganisms present in 29 deep carious lesions was studied. The numbers of lactobacilli and total Gram-positive rods in the carious lesions were found to be negatively related to the length of pain triggered by cold and heat stimulants. The presence of Gram-positive cocci and non-black-pigmented Bacteroides were positively associated with both cold and heat sensitivities. Black-pigmented Bacteroides, Streptococcus mutans, and total anaerobic colony counts were positively related to the heat sensitivity. Recovery of Fusobacterium nucleatum,…

Granulicatella elegans is a fastidious organism that is rarely implicated as a cause of infective endocarditis. Here, we describe a patient with mitral valve prolapse who developed G. elegans endocarditis. The organism was isolated from blood cultures and the patient had mitral valvuloplasty and repair, and completed a course of 6 weeks of intravenous antibiotics with no sequela.
