Low-Frequency Hearing Preservation With Long Electrode Arrays: Inclusion of Unaided Hearing Threshold Assessment in the Postoperative Test Battery.

Purpose The goal of this work was to evaluate the low-frequency hearing preservation of long electrode array cochlear implant (CI) recipients. Method Twenty-five participants presented with an unaided hearing threshold of ≤ 80 dB HL at 125 Hz pre-operatively in the ear to be implanted. Participants were implanted with a long (31.5-mm) electrode array. The unaided hearing threshold at 125 Hz was compared between the preoperative and postoperative intervals (i.e., initial CI activation, and 1, 3, 6, 9, and 12 months after activation). Results Eight participants maintained an unaided hearing thre

Congenital Cytomegalovirus Testing Following Pennsylvania Hearing-Targeted Screening Legislation.

BACKGROUND: Congenital cytomegalovirus infection (cCMV) occurs in approximately 1/200 to 1/250 US newborns each year and can cause progressive sensorineural hearing loss. Since 2022, Pennsylvania law mandates cCMV testing for infants who fail newborn hearing screening. OBJECTIVES: To assess changes in cCMV testing rates and diagnosis before and after Pennsylvania hearing-targeted legislation. METHODS: Retrospective cohort study of all live births at 3 university-affiliated birth centers. We identified all infants tested for CMV at 21 days of age or younger and analyzed cCMV testing rates in

Non-invasive testing to diagnose the underlying auditory pathology of hearing impairment.

Of the three major classifications of inner ear pathology, sensory, neural and metabolic, it is relatively easy to distinguish a sensory pathology from a neural or inner hair cell pathology using current clinical tools. Deficiencies in both distortion product otoacoustic emissions (DPOAE) and auditory brainstem responses (ABR) suggest a sensory pathology involving outer hair cells. Normal DPOAEs with deficiencies in ABRs suggest a neural pathology, involving inner hair cells, auditory nerve synapses or neural deficiencies. However, there are currently no non-invasive methods to distinguish a m

Hearing Loss: Genetic Testing, Current Advances and the Situation in Latin America.

Congenital hearing loss is the most common birth defect, estimated to affect 2-3 in every 1000 births, with ~50-60% of those related to genetic causes. Technological advances enabled the identification of hundreds of genes related to hearing loss (HL), with important implications for patients, their families, and the community. Despite these advances, in Latin America, the population with hearing loss remains underdiagnosed, with most studies focusing on a single locus encompassing the GJB2/GJB6 genes. Here we discuss how current and emerging genetic knowledge has the potential to alter the ap

Evaluation of Hearing Loss: Understanding Audiologic Testing to Refine Image Interpretation.

The standard of reference for diagnosing and characterizing hearing loss is audiologic testing. The results of audiologic testing inform the imaging algorithm and the differential diagnosis for the underlying cause. Pure-tone audiometry tests the ability to hear tones across different frequencies, and the results are displayed as an audiogram. Tympanometry measures tympanic membrane compliance as a function of pressure to generate a tympanogram. Acoustic reflex testing helps differentiate third window lesions from other causes of conductive hearing loss. Clinical and audiologic assessment of s

Improved Sensitivity of Digits-in-Noise Test to High-Frequency Hearing Loss.

OBJECTIVES: Hearing loss is most commonly observed at high frequencies. High-frequency hearing loss (HFHL) precedes and predicts hearing loss at lower frequencies. It was previously shown that an automated, self-administered digits-in-noise (DIN) test can be sensitized for detection of HFHL by low-pass filtering the speech-shaped masking noise at 1.5 kHz. This study was designed to investigate whether sensitivity of the DIN to HFHL can be enhanced further using low-pass noise filters with higher cutoff frequencies. DESIGN: The US-English digits 0 to 9, homogenized for audibility, were binaura

A Hearing-Model-Based Active-Learning Test for the Determination of Dead Regions.

This article describes a Bayesian active-learning procedure for estimating the edge frequency, fe, of a dead region, that is, a region in the cochlea with no or very few functioning inner hair cells or neurons. The method is based on the psychophysical tuning curve (PTC) but estimates the shape of the PTC from the parameters of a hearing model, namely fe, and degree of outer hair cell loss. It chooses the masker frequency and level for each trial to be highly informative about the model parameters in the context of previous data. The procedure was tested using 14 ears from eight subjects previ

Cochlear microphonic audiometry: a new hearing test for objective diagnosis of deafness.

CONCLUSIONS: Objective audiometric tests could constitute a valuable tool for detection of deafness. This could be especially useful in children (universal newborn hearing screening) and non-collaborative patients, who are especially difficult candidates for classic audiometry. The cochlear microphonic audiometry (CMA) technique offers the possibility of obtaining objective audiometric profiles, highly correlated with those obtained by pure tone audiometry (PTA). Therefore, CMA could be used as an alternative test to obtain the audiometric profile of these patients. OBJECTIVE: The main purpos

Factors affecting outcomes on the TEN (SPL) test in adults with hearing loss.

BACKGROUND: Recent work using the Threshold Equalizing Noise (TEN) test as a gold standard suggests that the presence of cochlear dead regions in persons with moderate-to-severe hearing loss may be quite common. In addition, previous data suggest that certain characteristics of hearing loss, such as severe-profound high-frequency hearing loss or steeply sloping configurations may be more commonly associated with positive TEN findings. These findings, however, are based largely on studies including a relatively small number of participants and/or participants that were included based on specifi

Adaptation of the HINT (hearing in noise test) for adult Canadian Francophone populations.

The HINT provides an efficient and reliable method of assessing speech intelligibility in quiet and in noise by using an adaptive strategy to measure speech reception thresholds for sentences, thus avoiding ceiling and floor effects that plague traditional measures performed at fixed presentation levels A strong need for such a test within the Canadian Francophone population, led us to develop a French version of the HINT. Here we describe the development of this test. The Canadian French version is composed of 240-recorded sentences, equated for intelligibility, and cast into 12 phonemically

External and middle ear effects on infant hearing screening test results.

This study investigated the relationship between external and middle ear factors and hearing screening results by automated auditory brain stem response (ABR) and transient-evoked otoacoustic emissions (EOAEs). The ears of 200 healthy newborns aged 5 to 48 hours underwent screening by ABR and EOAE, followed by otoscopic examination. The pass rates for ABR and EOAE were 91% and 58.5%, respectively. On otoscopic examination, 28% (112/400) ears had occluding vernix obscuring the view of the tympanic membrane. Cleaning of vernix was successfully performed in all but 2 ears that had occluding verni

Use of the distraction hearing test in children with congenital ocular motor apraxia.

Five of the 14 children with congenital ocular motor apraxia under Addenbrooke's Hospital Ophthalmology Department were assessed in tertiary audiology clinics. These referrals had been made after the children had failed to pass the 8-month hearing screen and second tier community-based assessment using distraction testing. In all cases they were found to have normal hearing on further assessment. The false positive response with the distraction test is likely to be due to the child's inability to initiate appropriate head and eye movements. We present a case report on a child with congenital o

A Revised Speech Intelligibility Rating (RSIR) test: listeners with normal hearing.

Two experiments were conducted to examine the intelligibility of 72 passages of connected discourse prepared by Cox and McDaniel in their development of the Speech Intelligibility Rating (SIR) test. Intelligibility was assessed with a method-of-adjustment (MOA) procedure in which listeners adjusted the level of a multi-talker babble until they could just understand 50% of a passage; the measure of intelligibility was the signal-to-babble ratio, dB S/B. The objective was to develop a Revised Speech Intelligibility Rating (RSIR) test that would comprise a large number of equivalent passages that

[Presentation of a new dichotic hearing test for the measurement of functional hemispheric asymmetries].

A newly developed dichotic test was tried on a clinical population of 197 primary school children. The results show a right ear advantage; that is better achievements in reproduction of verbal material presented to the right ear. This effect can only be reliably proved by double presentation of the material, once in inverse form. Changes in the test performance while doing the test; as well as the importance of the intervening variable, dominance of one word over the other within a pair simultaneously presented are discussed - first of all a statistical analysis was made to isolate the words t
