Doxapram use for apnoea of prematurity in neonatal intensive care.

Apnoea of prematurity is treated with noninvasive respiratory therapy and methylxanthines. For therapy unresponsive apnoea doxapram is often prescibed in preterm neonates. The duration, dosage and route of administration of doxapram together with its efficacy was evaluated in two Dutch neonatal intensive care. Outcome concerning short-term safety and neonatal morbidity were evaluated. During 5 years, 122 of 1,501 admitted newborns <32 weeks of gestational age received doxapram. 64.8% of patients did not need intubation after doxapram. 25% of treated neonates were <27 weeks of gestation. Fractures in a tertiary Neonatal Intensive Care Unit in Wales.

AIM: To explore associations with fractures reported in a tertiary neonatal intensive care unit (NICU). METHOD: Babies admitted to NICU from 1998 to 2007 in Cardiff, UK, with fractures during admission were identified. Clinical information was extracted from inpatient records. Results were reported as median (ranges). RESULTS: Seventy-one fractures were recorded in 27 (boys = 16) infants with a gestational age of 28 (23.6-40.4) weeks and birthweight of 920 g (485-4875). Fractures were recognized at 57 (1-128) days of age. Neonates with fractures without clear aetiologies (n = 17) were more p

Neonatal intensive care practices and the influence on skin condition.

BACKGROUND: Premature skin has a thinner epidermis with a poorly formed stratum corneum (SC) barrier compared to full term skin. Poor skin integrity increases the risk of exposure to irritants and infectious agents. Interventions that facilitate skin maturation are essential. OBJECTIVE: The objective was to examine the effects of prematurity and time from birth on SC maturation and to identify factors that impact skin condition. METHODS: A retrospective review was conducted among 130 NICU patients. Skin regions were evaluated for erythema, rash, integrity and function. The effects of gestati

Methicillin-resistant Staphylococcus caprae in a neonatal intensive care unit.

Staphylococcus caprae, a hemolytic coagulase-negative staphylococcus that is infrequently associated with humans, was initially detected in specimens from six infants in our neonatal intensive care unit due to phenotypic characteristics common to methicillin-resistant Staphylococcus aureus. These isolates were subsequently identified as S. caprae by the Automated RiboPrinter microbial characterization system. This prompted an 8-month retrospective investigation in our neonatal intensive care unit. S. caprae was the cause of 6 of 18 episodes of coagulase-negative staphylococcal bacteremia, was

Variations in mortality rates among Canadian neonatal intensive care units.

BACKGROUND: Most previous reports of variations in mortality rates for infants admitted to neonatal intensive care units (NICUs) have involved small groups of subpopulations, such as infants with very low birth weight. Our aim was to examine the incidence and causes of death and the risk-adjusted variation in mortality rates for a large group of infants of all birth weights admitted to Canadian NICUs. METHODS: We examined the deaths that occurred among all 19 265 infants admitted to 17 tertiary-level Canadian NICUs from January 1996 to October 1997. We used multivariate analysis to examine th

Endemic Pseudomonas aeruginosa infection in a neonatal intensive care unit.

BACKGROUND: Nosocomial infections due to Pseudomonas aeruginosa have been well described, but the environmental reservoir of the organism varies. We conducted an epidemiologic and molecular investigation of endemic P. aeruginosa infection among infants in a neonatal intensive care unit that was associated with carriage of the organisms on the hands of health care workers. METHODS: In August 1998, colonization or infection with P. aeruginosa was identified in six infants. Surveillance cultures for P. aeruginosa were obtained from the other 27 infants in the unit, and possible environmental res

Light and magnetic fields in a neonatal intensive care unit.

Effects of light and electromagnetic fields (EMFs) on pineal function could have implications for long-term risk of breast cancer, reproductive irregularities, or depression. Health-care workers in a neonatal intensive care unit (NICU) were interviewed to determine the tasks, work locations, and practices in their work environment as well as the care provided to the infants. After an initial visit, methods for measuring illuminance, luminance, and broadband resultant magnetic fields throughout the NICU were developed. Measurements were made of one nursery during a daytime (1:00 p.m.) and a nig

[Method of adjusting the netilmicin dosage in neonatal intensive care].

We tried to adapt the dose of netilmicin in an intensive care unit on 41 newborns, under 8 days of age. Individual pharmacokinetic parameters were calculated after the first intramuscular dose administration and the initial dose (3 mg/kg/twice a day) was modified in 17 cases. Methodologic problems but mainly great variations in physiology and pathology explain the difficulties in predicting the serum concentration (peak and valley) on the 7th day. A decrease in the daily dose for the preterm infant, compared to the one used in full-term infant, and a drug concentration monitoring are advised.

Lung Ultrasound in the Neonatal Intensive Care Unit: Does It Impact Clinical Care?

A neonatal point-of-care ultrasound has multiple applications, but its use has been limited in neonatal intensive care units in the Unites States. An increasing body of evidence suggests that lung ultrasound performed by the neonatologist, at the bedside, is reliable and accurate in differentiating neonatal respiratory conditions, predicting morbidity, and guiding invasive interventions. Recent research has shown that a lung ultrasound can assist the clinician in accurately identifying and managing conditions such as respiratory distress syndrome, transient tachypnea of the newborn, and bronch

The Correlation Between Neonatal Intensive Care Unit Safety Culture and Quality of Care.

OBJECTIVES: Key validated clinical metrics are being used individually and in aggregate (Baby-MONITOR) to monitor the performance of neonatal intensive care units (NICUs). The degree to which perceptions of key components of safety culture, safety climate, and teamwork are related to aspects of NICU quality of care is poorly understood. The objective of this study was to test whether NICU performance on key clinical metrics correlates with caregiver perceptions of safety culture. STUDY DESIGN: Cross-sectional study of 6253 very low-birth-weight infants in 44 NICUs. We measured clinical qualit

Neonatologist-performed point-of-care functional echocardiography in the neonatal intensive care unit.

Functional echocardiography (fECHO) refers to a bedside, limited assessment of the ductus arteriosus, myocardial performance and pulmonary or systemic haemodynamics that is brief in nature and addresses a specific clinical question or management dilemma. This point-of-care ultrasonography is increasingly used internationally and locally among neonatal units to assist with management of neonatal haemodynamic conditions. This article intends to explain the modality, its indications, interpretation and implications for management, and how it impacts long-term outcomes, particularly in chronic lun

Communicating About Necrotizing Enterocolitis in the Neonatal Intensive Care Unit.

Necrotizing enterocolitis (NEC) is a serious, sometimes fatal complication of prematurity and a challenging diagnosis to discuss with parents. In this article, we will discuss communication topics and strategies before, during and after a diagnosis of NEC, including the protective role of mother's own milk, the inability to fully eliminate the risk of NEC, the possibility of fulminant NEC, and long-term sequelae. We argue for clear and transparent communication to partner with and empower families, especially at the time of NEC diagnosis and at key transitions of care throughout the neonatal i

AI-guided precision parenteral nutrition for neonatal intensive care units.

One in ten neonates are admitted to neonatal intensive care units, highlighting the need for precise interventions. However, the application of artificial intelligence (AI) in guiding neonatal care remains underexplored. Total parenteral nutrition (TPN) is a life-saving treatment for preterm neonates; however, implementation of the therapy in its current form is subjective, error-prone and resource-consuming. Here, we developed TPN2.0-a data-driven approach that optimizes and standardizes TPN using information collected routinely in electronic health records. We assembled a decade of TPN compo

Comparison of metrics of neonatal intensive care unit antibiotic use.

OBJECTIVE: To compare temporal trends, variation, and correlations between antibiotic use metrics across U.S. neonatal intensive care units (NICUs) and assess associations with mortality. METHODS: We conducted a retrospective cohort study of 438,156 infants admitted to 272 NICUs from 2017 to 2021 using the Premier Health Database. Antibiotic use rate (AUR), days of therapy (DOT), and antibiotic spectrum index (ASI) per 1,000 patient or therapy days were calculated both cumulatively by year and at the center level. Mixed-effects models adjusted for center-level characteristics were used for al
