Flow-synchronized nasal intermittent positive pressure ventilation for prevention of extubation failure in neonates: a review of literature and a case series of neonates with congenital diaphragmatic hernia successfully managed with this strategy.

BACKGROUND: Advances in neonatal medicine have improved survival rates in neonatal intensive care units, especially for high-complexity cases like congenital diaphragmatic hernia (CDH). Despite these advances, managing respiratory failure in CDH infants remains challenging due to lung hypoplasia, respiratory insufficiency, and pulmonary hypertension. Lung-protective ventilation strategies are crucial to minimize ventilator-induced lung injury, but weaning them from invasive mechanical ventilation remains complex, and extubation failure rates are high. CASE PRESENTATION: This retrospective cas

Heated Humidified High Flow Nasal Cannula (HHHFNC) is not an effective method for initial treatment of Respiratory Distress Syndrome (RDS) versus nasal intermittent mandatory ventilation (NIMV) and nasal continuous positive airway pressure (NCPAP).

BACKGROUND: Noninvasive respiratory support techniques are widely used to treat respiratory distress syndrome (RDS) in preterm infants, and the effectiveness of these methods should be compared. In the current study, nasal continuous positive airway pressure (NCPAP), nasal intermittent mandatory ventilation (NIMV), and heated humidified high-flow nasal cannula (HHHFNC) were compared. MATERIALS AND METHODS: In the current bicenter clinical trial, 109 preterm infants with RDS not treated with surfactant were randomly assigned to three groups: NCPAP, NIMV, and HHHFNC. The initial outcomes includ

Comparison of two fractions of inspired oxygen thresholds (0.25 vs 0.30) for surfactant administration in very preterm infants with respiratory distress syndrome under nasal intermittent positive pressure ventilation: study protocol for a multicentre randomised controlled, superiority trial.

INTRODUCTION: The combination of non-invasive ventilation, particularly nasal intermittent positive pressure ventilation (NIPPV), and exogenous surfactant therapy has been shown to significantly reduce mortality and morbidity in neonates with respiratory distress syndrome (RDS). The 2022 European Consensus Guidelines on RDS recommend a universal fraction of inspired oxygen (FiO₂) threshold of 0.30 for surfactant administration, irrespective of gestational age. However, this recommendation lacks validation through randomised controlled trials, and the optimal FiO₂ threshold remains uncertain. T

Comparing the Effect of Beractant (Beraksurf™) with That of Poractant Alfa (Curosurf®) on the Need for Intermittent Positive Pressure Ventilation in Neonatal Respiratory Distress Syndrome by Adopting a Semi-parametric Approach: Re-Analyzing Data of a Randomized Controlled Trial.

INTRODUCTION: Randomized controlled trial (RCT) data are analyzed by two challengeable adjusted and non-adjusted approaches. Performing appropriate adjusted analysis leads to a more interpretable and efficient estimation of treatment effects. Semiparametric adjustment approach modifies the estimating equations solved by the marginal treatment effect estimator by adding an augmentation function, which makes use of the baseline covariates and estimate the unbiased marginal treatment effect with improved precision. The effect of the intervention obtained using the semi-parametric adjustment metho

The Diaphragmatic Initiated Ventilatory Assist (DIVA) trial: study protocol for a randomized controlled trial comparing rates of extubation failure in extremely premature infants undergoing extubation to non-invasive neurally adjusted ventilatory assist versus non-synchronized nasal intermittent positive pressure ventilation.

BACKGROUND: Invasive mechanical ventilation contributes to bronchopulmonary dysplasia (BPD), the most common complication of prematurity and the leading respiratory cause of childhood morbidity. Non-invasive ventilation (NIV) may limit invasive ventilation exposure and can be either synchronized or non-synchronized (NS). Pooled data suggest synchronized forms may be superior. Non-invasive neurally adjusted ventilatory assist (NIV-NAVA) delivers NIV synchronized to the neural signal for breathing, which is detected with a specialized catheter. The DIVA (Diaphragmatic Initiated Ventilatory Assis

Inspiratory pressure support prevents diaphragmatic fatigue during weaning from mechanical ventilation.

Persistent inability to tolerate discontinuation from mechanical ventilation is frequently encountered in patients recovering from acute respiratory failure. We studied the ability of inspiratory pressure support, a new mode of ventilatory assistance, to promote a nonfatiguing respiratory muscle activity in eight patients unsuccessful at weaning from mechanical ventilation. During spontaneous breathing, seven of the eight patients demonstrated electromyographic signs of incipient diaphragmatic fatigue. During ventilation with pressure support at increasing levels, the work of breathing gradual

Clinical applications of body ventilators.

Interest has been increasing in providing ventilatory support in the home for patients with chronic respiratory failure, mainly with the use of positive pressure ventilation via a chronic tracheostomy. However, body ventilators that assist ventilation by applying intermittent negative or positive pressure to the thorax, abdomen, or airway without requiring an artificial airway, can offer distinct advantages for selected patients over systems requiring a permanent airway. These ventilators include the iron lung, portable lung (Portalung), pneumowrap, chest cuirass, pneumobelt, rocking bed, and

[General introduction to the clinical application of non-invasive respiratory aids].

Intermittent positive pressure ventilation via tracheostomy has been the most common method of providing ventilatory support for patients with respiratory failure since the late 1950s. It is the standard procedure for individuals who suffer from acute respiratory failure, impaired consciousness, and severe restrictive lung disease. However, numerous adverse complications from these invasive techniques have been reported in those who are long-term ventilator users. This paper describes the evolution and current application of non-invasive respiratory aids in both acute and long-term settings. T

Characteristics of the Five Different Types of Intermittent Mandatory Ventilation.

Intermittent mandatory ventilation (IMV) has evolved substantially from its original form, developing into 5 distinct types intended to enhance patient safety, comfort, and liberation from mechanical ventilation. Each form of IMV has different potential advantages and disadvantages that should be considered when selecting the mode of ventilation. IMV(1): the set mandatory breath rate is always delivered. IMV(2): mandatory breaths may be suppressed by spontaneous breaths if their frequency is high enough. IMV(3): mandatory breaths may be suppressed by spontaneous breaths if they achieve some pr

Supporting all breaths versus supporting some breaths during synchronised mechanical ventilation in neonates: a systematic review and meta-analysis.

BACKGROUND: National Institute for Health and Clinical Effectiveness (NICE), UK, guideline published in 2019 recommends the use of volume-targeted ventilation (VTV). It recommends synchronised intermittent mandatory ventilation (SIMV) over the modes that support-all-breaths, for example, assist control ventilation (ACV). We conducted a systematic review and meta-analysis of the studies comparing SIMV mode with triggered modes supporting all breaths. METHODS: Patients: Neonates receiving mechanical ventilation. INTERVENTION: SIMV ventilation.Comparison: Modes that support-all-breaths: ACV, pr

Intermittent mandatory ventilation and weaning.

Oxygen, PEEP, and mechanical ventilatory therapy should be administered to patients in varying amounts and should be removed gradually and independently. The method of determining optimal PEEP, oxygen, and ventilation is not unlike that recommended for many other therapies. Nine years of prospective evaluation have demonstrated the numerous clinical advantages of this technique, and relatively few complications have been associated with it. Reduced FIO2 may promote resistance to atelectasis and allow rapid discontinuation of mechanical ventilation and PEEP. Similarly, optimal levels of PEEP ma

Life-threatening Mycoplasma pneumoniae infection.

Pneumonia due to Mycoplasma pneumoniae is common, but it is rarely life-threatening. A case is reported in a previously healthy adult whose illness was severe enough to require treatment with intermittent positive pressure ventilation (IPPV) for 14 days and which resolved after treatment with corticosteroids.

Recruiting collapsed lung through collateral channels with positive end-expiratory pressure.

We studied reinflation of collapsed parts in excised normal human lungs through both the ordinary bronchial route and through collateral channels. A model of atelectasis was made either by simple collapse or by applying a positive pressure to the pleura and a negative to the airway. Five different ventilatory patterns were used for reinflation: simulated normal breathing with and without continuous positive airway pressure (CPAP), simulated deep breathing and mechanical ventilation with and without positive end-expiratory pressure (PEEP). All methods, except normal breathing without CPAP, rein

Effects of intermittent negative pressure ventilation on effective ventilation in normal awake subjects.

RATIONALE: Previous studies have shown that an increase in inspiratory pressure during nasal intermittent positive pressure ventilation (IPPV) does not result in increased effective minute ventilation (E) due to glottic interference. STUDY OBJECTIVES: To test the consequences of increases in negative pressure ventilation (NPV) on V(E). MATERIAL AND METHODS: Eight healthy awake subjects underwent NPV delivered by an iron lung. First, NPV was started at a respirator frequency (f) of 15 cycles per minute with an inspiratory negative pressure (INP) of - 15 cm H(2)O (F15-P15). Then, f was increas

Techniques of ventilatory therapy in the adult respiratory distress syndrome (ARDS).

Techniques of respiratory support in ARDS are becoming more and more complex. New modes of ventilatory therapy like continuous positive airway pressure (CPAP), appeared during the last years and became popular; other ones are now under investigation, high frequency ventilation and extracorporeal CO2 removal, for instance. A lot of abbreviations are actually commonly used by physicians involved in intensive care as well as by ventilator industries. The purpose of this article is to review these different modes of respiratory assistance in ARDS, as well as their indications. They are grouped int

Alternatives to mouthpiece noninvasive ventilatory support to permit dental care.

Patients who are dependent on continuous noninvasive intermittent positive pressure ventilation for ventilatory support via angled mouthpiece interfaces during daytime hours often need dental interventions that are prevented by the presence of the mouthpiece. To permit dental interventions, however, the noninvasive intermittent positive pressure ventilation can most conveniently and safely be delivered via nasal interface using oximetry monitoring as long as both sedation and supplemental oxygen are avoided. Three mouthpiece continuous noninvasive intermittent positive pressure ventilation use
