[Keratoconus: biomechanics ex vivo].

BACKGROUND: Keratoconus is associated with an impairment in corneal biomechanics. Using nanoindentation, spatially resolved measurement of biomechanical properties can be performed on corneal tissue. The aim of this study is to assess the biomechanical properties of corneas with keratoconus in comparison to healthy controls. METHODS: 17 corneas with keratoconus and 10 healthy corneas unsuitable for transplantation were included in the study. After explantation, corneas were kept in culture medium containing 15% dextran for at least 24 h. Nanoindentation was then performed to a depth of 25 µm

Topography-Based Keratoconus Regression.

PURPOSE: To report 3 cases of keratoconic patients who had progressive corneal topographic flattening during a 2-year follow-up period. METHODS: Case series. RESULTS: Three patients with bilateral keratoconus, each operated for advanced keratoconus in one of their eyes, were observed for possible progression of the disorder for a period of 2 years. During the 2-year follow-up period, topographic examination showed progressive corneal flattening in the nonoperated eye. Mean keratometry decreased in all patients, whereas topography showed significant flattening at the apex of the cone. Best-sp

Comparison of a Scheimpflug imaging with other screening indices in diagnosing keratoconus and keratoconus suspect.

Keratoconus (KC) is an irreversible blinding eye disease; therefore, early screening of KC suspects (KCS) is crucial for protecting patients' quality of life. Scheimpflug imaging is a commonly used screening device in clinical practice. We aimed to evaluate the diagnostic ability of a Scheimpflug imaging device (Scansys) for KC and KCS and compared it with other Scheimpflug-based devices (Pentacam and Corvis ST). This prospective case-control study included 107 normal eyes, 72 KCS, and 57 KC. Scansys screening index Keratoconus probability (KCP) showed excellent performance in diagnosing KC at

Update on the Physiopathology of Keratoconus.

Keratoconus (KC) is a progressive corneal ectasia characterized by stromal thinning, steepening, and biomechanical instability. Although historically considered primarily a structural disorder, current evidence supports a multifactorial pathogenesis involving complex interactions among biomechanical, molecular, cellular, inflammatory, neurobiological, and environmental mechanisms. This narrative review provides an updated overview of KC pathophysiology, integrating current evidence across these interconnected domains. Focal reductions in corneal stiffness, altered viscoelasticity, collagen dis

[KERATOCONUS: DIAGNOSIS AND INNOVATIONS].

The importance of early detection of keratoconus lies in early treatment to prevent complications resulting from disease progression and in planning optimal follow-up or screening schedules for family members, considering the genetic component of the disease. In the past, with the absence of accessible, adequate imaging technology such as corneal topography to detect subtle manifestations of the disease, keratoconus was considered rare. This resulted in later detection when the disease had progressed to a more advanced, severe state, with many patients requiring corneal transplants. Currently,

Keratoconus: Diagnosis and Staging.

The advent of refractive surgery and corneal cross-linking for ectatic disease has further highlighted the need to recognize early or subclinical ectatic disease. Historical systems depend on anterior corneal changes that occur late in the disease course and are commonly associated with visual loss. Tomographic imaging allows for the acquisition of posterior corneal surface data and corneal thickness distribution. This led to the development of modalities to diagnose early or subclinical keratoconus and to screen patients for refractive surgery. This article reviews the modern methods for asses

Keratoconus: An updated review.

Keratoconus is a bilateral and asymmetric disease which results in progressive thinning and steeping of the cornea leading to irregular astigmatism and decreased visual acuity. Traditionally, the condition has been described as a noninflammatory disease; however, more recently it has been associated with ocular inflammation. Keratoconus normally develops in the second and third decades of life and progresses until the fourth decade. The condition affects all ethnicities and both sexes. The prevalence and incidence rates of keratoconus have been estimated to be between 0.2 and 4,790 per 100,000
