A renal transplant recipient presented with pulmonary blastomycosis. Because of recent data suggesting a role for ketoconazole in the treatment of blastomycosis and concern for preserving her functioning renal transplant, she was started on therapy with ketoconazole, 400 mg daily. Forty-two women were administered ketoconazole, an orally absorbed antifungal agent. The symptoms of vulvar pruritus, burning, pain, and dysuria and the signs of vulvar erythema and fissures and vaginal erythema were associated with Candida albicans. Although ketoconazole relieved symptoms and signs seven days after therapy, a high recurrence rate occurred by 28 days after therapy.

A very rare case of Petriellidium boydii pachymeningitis is described. Rarer still is the stabilization of this case for a prolonged period. Miconazole and ketaconazole both seemed useful in the treatment, and their role is discussed.

Twenty six children with candidiasis, aged between 5 months and 14 years, were treated with different formulations and regimens of ketoconazole. Long-standing disseminated lesions of pityriasis versicolor (PV) were found in 101 patients. As they had failed to respond to various topical agents, we treated them with 200 mg/day of ketoconazole. All of them were under clinical and mycological surveillance at weekly intervals. In successive mycological examinations during the course of therapy, an increased ratio of filaments/spores was observed, and the last positive examinations invariably showed the presence of only a few filaments and no spores. Twenty-four paracoccidioidomycosis patients who completed a course of therapy with ketoconazole were followed for 12 months after treatment, and 10 of these patients were followed for 24 months. Only two of these patients relapsed; the remaining continued in remission. There were no fatalities. Immunologic tests showed decreased antibody activity and increased skin test reactivity to paracoccidioidin in comparison with the results obtained at the end of therapy. These findings indicate that ketoconazole therapy has improved the prognosis of patients with paracoccidioidomycosis.

Apart from the standard treatment of pityriasis versicolor with selenium disulphide preparations, the 30-day-therapy with ketoconazole has been considered an alternative so far. According to our investigations, the duration of treatment may be shortened to ten days. After discontinuation of the drug, the fungal elements are reduced and finally disappear. This effect is attributed to the active substance being deposited in the sebaceous glands and secreted onto the skin. An influence of the product on phagocytosis is also discussed.

A 46-year-old man with recurrent pulmonary histoplasmosis was given oral treatment with ketoconazole (a new imidazole derivative related to miconazole) in a dose of 400 mg/day for 3 months, followed by 200 mg daily for 6 months. Ketoconazole, a new oral imidazole derivative, was employed for the treatment of five patients with paracoccidioidomycosis. The response was excellent, with objective clinical improvement and healing of both mucocutaneous and pulmonary lesions. Mycological and serological tests, as well as delayed hypersensitivity, were assessed and found to correlate with clinical improvement. The diagnosis of an estrogen-secreting adrenal tumor was made based upon the finding of a 4-cm left adrenal mass, elevated levels of estradiol in peripheral and left adrenal venous blood, and increased urinary 17-ketosteroids. We report a patient with pulmonary blastomycosis who suffered relapse with dissemination of her disease after five months of apparent response to therapy with ketoconazole, 400 mg daily. Common causes of drug failure, including noncompliance and failure of absorption, were excluded, and there was no evidence of altered host immunity. Forty-two patients with active paracoccidioidomycosis were randomized to receive itraconazole (50-100 mg d(-1)), ketoconazole (200-400 mg d(-1)) or sulfadiazine (100-150 mg kg d(-1) up to 6 g d(-1)) for 4-6 months, followed by slow release sulfa until negativity of serological tests.
