Conjoint bicondylar coronal plane fracture is a rare orthopaedic injury, associated with high-velocity trauma. The proposed mechanism is axially directed shear forces in a flexed knee. To the best of our literature search, only 2 cases of conjoint bicondylar coronal fracture have been published in English literature-1 each in adult and paediatric age group. Conjoint bicondylar coronal plane fracture with incarcerated patella has not yet been reported and hence this is the first report of its kind. We report on the clinical presentation, management and outcome of such a complex injury, along wi

BACKGROUND: Several studies have found the 5-item modified frailty index (mFI-5) to be a reliable indicator of poor postoperative outcomes following various surgical procedures. This study aims to evaluate whether the mFI-5 continues to serve as a reliable predictor for patients undergoing patella fracture repair.
METHODS: The NSQIP database was queried to identify patients ages 50 or older who underwent surgery for patella fractures between 2006-2019. The mFI-5 was calculated based on the presence of the following 5 comorbidities: diabetes, CHF, hypertension, COPD, and dependent functional s…

OBJECTIVES: This study aimed to compare the biomechanical properties of three stabilisation techniques in feline patella transverse fractures and select the strongest method with potentially minimal complications.
METHODS: Patella fracture was simulated in 27 feline cadaveric pelvic limbs (mean weight of the cadavers 3.78 kg), and the limbs were randomly grouped to be stabilised with one of the three stabilisation methods. The modified tension band wiring technique with a single Kirschner wire (0.9 mm) and figure-of-eight wiring (20 G) was applied to group 1 (n = 9). Group 2 (n = 9) was stabi…

BACKGROUND: Inferior pole fracture of the patella (IPFP) has small and comminuted fracture blocks that are hard to immobilize, and early mobilization may lead to loss of fracture reduction and immobilization failure. Therefore, a difficulty of treatment is to achieve rigid immobilization with early functional exercise. Here, a new treatment method of tension-free external immobilization is put forward.
METHODS: The clinical data of 11 IPFP patients treated with tension-free external immobilization from May 2016 to June 2019 were retrospectively analyzed. There were six males and five females

OBJECTIVE: Inferior pole fractures of patella are notorious fractures where it is difficult to obtain rigid internal fixation by conventional methods. The objective of the study was to introduce the Hand Plating System (HPS), which was a novel surgical technique for inferior pole fractures of patella, and to report the radiological and clinical outcomes following the application of the surgical technique.
METHODS: The study was designed as a retrospective cohort study. Between July 2017 and December 2018, 30 patients who were diagnosed with inferior pole fracture of the patella without additi

OBJECTIVES: The purpose of this study was to compare the biomechanical attributes of patella fracture fixation with either anterior plating utilizing two parallel, longitudinal 2.0 mm plates technique versus a cannulated screw tension band technique.
METHODS: Five matched pairs (ten specimens) of fresh frozen cadavers were utilized. A transverse patella fracture (OTA 34C1.1) was fixed using either two 4.0 mm cannulated screw anterior tension band (CATB) or with two 2.0 mm stainless steel non-locking plates along the anterior cortex secured with 2.4 mm cortical screws traversing the fracture s…

Open reduction combined with external compressive skeletal fixation was used to treat twenty-seven patients who had a separated fracture of the patella. Fixation was obtained by the use of two compressive clamps applied to stainless-steel pins that were inserted just proximal and distal to the proximal and distal poles of the patella. Range-of-motion exercises for the knee were begun at two weeks and the pins were removed at three to four weeks. All of the fractures healed. Twenty-four patients regained a range of motion that was equal to that of the opposite knee. There was no evidence of chr

Thirty-one patients have been reviewed four and a half to thirteen years after total excision of the patella for fracture. This operation did not give the uniformly excellent results previously reported by some authors. The type of incision used was unimportant in the long term. Immobilisation in plaster-of-Paris for any period between one and eight weeks after operation had no adverse effect on the long-term results. There was no correlation between the amount of calcification or ectopic bone formation found in the patellar tendon and the degree of function or discomfort in the joint. There w

CASE: A 12-year-old boy sustained a patella sleeve fracture of the superior pole, medial patellofemoral ligament tear, and lateral femoral condyle fracture after a direct contact sledding injury. He was managed nonoperatively with 5 weeks of cylinder cast immobilization with transition to a hinged knee brace and physiotherapy. By 3 months, he returned to sport activity without patellar instability.
CONCLUSION: This is the first case to describe simultaneous development of these 3 injuries in an adolescent. We recommend that patients with patella sleeve fracture undergo magnetic resonance imag

Fractures of the patella constitute approximately 1% of all skeletal injuries. The vertical pattern represents 12-17% and the open patella fracture represents 6-30%. We here represent a rare case constituting the presence of these two uncommon (vertical type patella fracture with depression of articular surface). A 22-year-old male had a close patella fracture after a road traffic accident. The fracture was classified as a vertical and comminuted pattern. A back slab above the knee was applied and then the patient was prepared for surgery. Open reduction and internal fixation of the patella by…

BACKGROUND: There has been no gold standard of the initial treatment strategy for acute patellar dislocation (APD) with osteochondral fracture (OCF). Hence the study aim is firstly, to review and compare clinical outcomes of patients who underwent conservative treatment for APD with or without OCF. Secondly, to characterize the location and size of fracture fragment.
METHODS: Sixty-nine consecutive patients who were retrospectively evaluated after first-time APD over a 2- year period were divided into two groups (group 1 (n = 24): APD with OCF and group 2 (n = 45): APD only). Magnetic resonan

This study evaluated complications associated with implant depth in headless compression screw treatment of an osteochondral fracture associated with a traumatic patellar dislocation in a 21-year-old woman. Computed tomography and X-rays showed one lateral fracture fragment measuring 25 × 16 mm. Osteosynthesis was performed with two headless compression screws. Five months later, the screws were removed because of patella-femoral implant friction. We recommend that the screw heads be embedded to a depth of at least 3 mm below the cartilage surface. Further clinical studies need to examine the

We present a simultaneous patella tendon avulsion fracture from both sides of its attachment in an ectopic ossified patellar tendon. Medially two thirds of the patellar tendon was detached from the inferior patella, and laterally one third of that from anterior tibial tuberosity with the patellar tendon split. Osteosynthesis was used to restore the extensor mechanism of the knee. The result at 46 months was good, even though there were some remaining ectopic ossifications in the patellar tendon.

A 57-year-old man was followed up for multiple myeloma with no lytic bone lesions. During a septicaema, he presented with a pathological fracture of the patella. Despite a poor haematological status, he received surgical treatment, which allowed diagnosis of haematogenous osteomyelitis of the patella. The outcome was favourable, with a 6-week plaster immobilization after cerclage and 4 months of intravenous antibiotic therapy. This case emphasizes the fact that one well-known cause of bone lesions may lead to an erroneous diagnosis. The successful clinical course suggests that accurate therape

UNLABELLED: Fractures of the Inferior pole patellar fracture (IPPF) affect the distal end of the bone, where the patellar tendon inserts, and are more common in young, active adults due to direct trauma or intense muscle contractions. This region, which is structurally more fragile and subjected to high tensile loads, becomes vulnerable to fractures that compromise the knee extensor mechanism, resulting in pain, functional limitation, and loss of mobility. Treatment can be conservative or surgical, depending on the degree of displacement and the integrity of the extensor apparatus, and involve

Impetigo is a pediatric skin infection characterized by the presence of pathognomonic "honey-crusted" lesions caused by either Staphylococcus aureus or Streptococcus pyogenes. The diagnosis of impetigo is largely based on clinical judgment, confirmatory skin cultures, and Gram staining. Surgical site infections following patellar surgery are a relatively uncommon occurrence, with the most common causative organisms being Staphylococcus aureus, Streptococcus pyogenes, Pseudomonas aeruginosa, and other gram-negative bacilli. Surgical site infections have a range of risk factors that largely depe
