Bridging the gap: Spinal cord fusion as a treatment of chronic spinal cord injury.

Despite decades of animal experimentation, human translation with cell grafts, conduits, and other strategies has failed to cure patients with chronic spinal cord injury (SCI). Recent data show that motor deficits due to spinal cord transection in animal models can be reversed by local application of fusogens, such as Polyethylene glycol (PEG). Results proved superior at short term over all other treatments deployed in animal studies, opening the way to human trials. In particular, removal of the injured spinal cord segment followed by PEG fusion of the two ends along with vertebral osteotomy

[Effects of caudal traction of the spinal cord on evoked spinal cord potentials in the cat].

This study attempts clarify the mechanism of neurological deficits in tethered cord syndrome using evoked spinal cord potentials (ESCPs). ESCPs in response to both sciatic nerve (SN-ESCP) and spinal cord stimulation (SC-DESCP) were recorded from the dorsal epidural space. With a fixed degree of caudal traction on the spinal cord in ten cats for 2-4 hours, ESCPs were increased in amplitude in the N1 and N2 deflections of the SC-DESCPs to 158% and 154% at L5 and decreased to 91% and 76% after transient augmentation at L3. On the other hand, the amplitude in the N1 deflection of the SN-ESCPs at L

Significance of spinal cord swelling in the prognosis of acute cervical spinal cord injury.

Seven cases of acute cervical cord injury examined by CT-myelography are reported. Cord swelling was observed in 2 patients who developed complete lesions. Another 5 cases had an incomplete lesions. Spinal cord swelling indicated severe cord damage. External decompression seemed to have little effect on cord swelling. When cord swelling is absent, neurological improvement was expected though the initial neurological state showed severe deficits. A further 4 patients who had an acute cervical cord injury who were treated by myelotomy at the injury site are also reported. Two patients obtained s

Tethered thoracic cord resulting from spinal cord herniation.

Tethered cord syndrome (TCS) usually involves tethering of the lower cord at the conus medullaris from dural abnormalities, but may occur after spinal cord herniation. A tethered thoracic spinal cord is rare. We present an unusual case of a 30-year-old woman with a history of myelopathy presumed to be secondary to T6 cord compression resulting from T6-T8 arachnoid cyst. She continued to deteriorate after partial excision of the cyst. Repeat magnetic resonance imaging suggested recurrence of the presumed arachnoid cyst with cord compression and showed tethering at T6-T8. Surgical exploration re

A reassessment of spinal cord pathology in severe infantile spinal muscular atrophy: Reassessment of spinal cord pathology.

AIMS: Spinal muscular atrophy (SMA) is a life-limiting paediatric motor neuron disease characterised by lower motor neuron loss, skeletal muscle atrophy and respiratory failure, if untreated. Revolutionary treatments now extend patient survival. However, a limited understanding of the foundational neuropathology challenges the evaluation of therapeutic success. As opportunities to study treatment-naïve tissue decrease, we have characterised spinal cord pathology in severe infantile SMA using gold-standard techniques, providing a baseline to measure treatment success and therapeutic limitations

Electrical epidural stimulation of the cervical spinal cord: implications for spinal respiratory neuroplasticity after spinal cord injury.

Traumatic cervical spinal cord injury (cSCI) can lead to damage of bulbospinal pathways to the respiratory motor nuclei and consequent life-threatening respiratory insufficiency due to respiratory muscle paralysis/paresis. Reports of electrical epidural stimulation (EES) of the lumbosacral spinal cord to enable locomotor function after SCI are encouraging, with some evidence of facilitating neural plasticity. Here, we detail the development and success of EES in recovering locomotor function, with consideration of stimulation parameters and safety measures to develop effective EES protocols. E

The therapeutic window for spinal cord decompression in a rat spinal cord injury model.

OBJECT: There are no clinically based guidelines to direct the spine surgeon as to the proper timing to undertake decompression after spinal cord injury (SCI) in patients with concomitant stenosis-induced cord compression. The following three factors affect the prognosis: (1) severity of SCI; (2) degree of extrinsic spinal cord compression; and (3) duration of spinal cord compression. METHODS: To elucidate further the relationship between varying degrees of spinal stenosis and a mild contusion-induced SCI (6.25 g-cm), a rat SCI/stenosis model was developed in which 1.13- and 1.24-mm-thick spa

Understanding the Measurement of Pressure Within the Spinal Cord to Optimize Spinal Cord Perfusion Pressure Using a Porcine Model of Spinal Cord Injury.

Recent studies have reported that monitoring spinal cord perfusion pressure (SCPP) using a pressure probe to measure "intraspinal pressure" (ISP) within the subdural space at the injury site may improve the hemodynamic management of acute spinal cord injury (SCI) patients. This study aimed to investigate, within a pig model of SCI, the relationship between the ISP measured within the subdural space and the "spinal cord pressure" (SCP) measured within the spinal cord itself. Specifically, we sought to characterize the changes to ISP and SCP over time, both rostral and caudal to the injury epice

[A case of primary intramedullary spinal cord lymphoma diagnosed by spinal cord biopsy of long spinal cord lesions showing persistent gadolinium contrast enhancement].

An 82-year-old man presented with subacute bilateral lower limb paralysis, deep sensory disturbance, and vesico-rectal disturbance. MRI of the spinal cord revealed a large gray matter-dominant lesion extending from the medulla oblongata to the lower thoracic spinal cord. The patient was treated with steroid-pulse therapy for myelitis, but without symptomatic improvement. A spinal cord biopsy was performed for treatment-resistant myelopathy, and histopathology revealed a diffuse large B-cell lymphoma, that was diagnosed as a primary intramedullary spinal cord lymphoma because systemic examinati

Spinal tophaceous gout encasing the thoracic spinal cord.

Gout rarely compresses the thoracic spinal cord. A 43-year-old man presented with lower limb paraparesis. MRI showed a soft tissue swelling at the level of T10/T11. He was managed with a laminectomy and evacuation of a presumed abscess and started on intravenous antibiotics. However, histology confirmed tophaceous gout.

Atypical spinal cord injury: spinal dural arteriovenous fistula.

Spinal dural arteriovenous fistulas (SDAVF) are vascular anomalies composed of intertwining arteries and veins with direct arteriovenous (AV) communication. It is presumed that the fistula is an acquired abnormality that produces an arterialization and increase in venous blood flow leading to venous hypertension, venous congestion with eventual hypo-perfusion, or ischemia of the spinal cord. Symptoms include progressive sensory and motor changes that commonly take place over a 2- to 3-year time span. Exacerbation of symptoms is frequently the reason for additional diagnostic work-up. Treatment

Bladder and urethral sphincter responses evoked by microstimulation of S2 sacral spinal cord in spinal cord intact and chronic spinal cord injured cats.

Urinary bladder and urethral sphincter responses evoked by bladder distention, ventral root stimulation, or microstimulation of S2 segment of the sacral spinal cord were investigated under alpha-chloralose anesthesia in cats with an intact spinal cord and in chronic spinal cord injured (SCI) cats 6-8 weeks after spinal cord transection at T9-T10 spinal segment. Both SCI and normal cats exhibited large amplitude reflex bladder contractions when the bladder was fully distended. SCI cats also exhibited hyperreflexic bladder contractions during filling and detrusor-sphincter dyssynergia during voi

Trunk-mediated spinal cord stimulation: A step toward clinical implementation after spinal cord injury.

In this issue of Med, Agnesi et al., show that residual trunk movements after spinal cord injury can modulate the effects of epidural spinal cord stimulation (SCS) to enable participants to volitionally initiate and control stepping.1 This trunk-mediated interface offers a practical strategy for bringing epidural SCS one step closer to widespread clinical adoption.

Spinal cord stimulation to manage autonomic dysfunction after spinal cord injury: a systematic review.

INTRODUCTION: Spinal cord injury (SCI) results in severe autonomic dysfunction, across cardiovascular, bladder, bowel and sexual domains, negatively affecting quality of life. Recently, there has been growing interest in the application of spinal cord stimulation (SCS) to restore or modulate autonomic functions in people with SCI. This review systematically evaluated the evidence for spinal cord and/or root stimulation, delivered via epidural (eSCS), transcutaneous (tSCS) spinal cord stimulation, or sacral (posterior) anterior root stimulation (SARS/SPARS) - in managing autonomic dysfunction a

UTX/Top2β axis mediated spinal cord microvascular endothelial cells senescence exacerbates spinal cord injury.

BACKGROUND: Cellular senescence plays a critical role in regulating angiogenesis. While UTX has been implicated in vascular regeneration following spinal cord injury (SCI), its underlying mechanisms remain incompletely understood. This study investigates how senescence mediates UTX-regulated vascular responses after SCI. METHODS: We assessed p16INK4a (a senescence marker) and UTX expression in spinal cord endothelial cells at 3, 5, 7 and 14 days post-SCI. By breeding UTXflox/flox mice with heterozygous Tek-Cre mice, we generated endothelial-specific UTX knockout mice. Cellular senescence and

Neurophysiological Effect of Transcutaneous Electrical Spinal Cord Stimulation in Chronic Complete Spinal Cord Injury.

BACKGROUND: Transcutaneous electrical spinal cord stimulation combined with activity-based therapy (TESCS-ABT) holds promising potential for motor rehabilitation of spinal cord injury (SCI). However, its effectiveness in individuals with chronic complete SCI remains largely unexplored, despite recent evidence suggesting that many of these individuals exhibit signs of neurological incompleteness. This study was a prospective, single-arm, open-label trial that investigated the neurophysiological effects of TESCS-ABT in chronic complete SCI and assessed whether any changes translate into function

Mapping Spinal Cord Stimulation-Evoked Muscle Responses in Patients With Chronic Spinal Cord Injury.

OBJECTIVES: Epidural spinal cord stimulation (eSCS) has shown promise for restoring some volitional motor control after spinal cord injury (SCI). Maximizing therapeutic response requires effective spatial stimulation generated through careful configuration of anodes and cathodes on the eSCS lead. By exploring the way the spatial distribution of low frequency stimulation affects muscle activation patterns, we investigated the spatial specificity of stimulation-evoked responses for targeted muscle groups for restoration after chronic SCI (cSCI) in participants in the Epidural Stimulation After N
