The authors report a case of xanthogranulomatous pyelonephritis presenting with nephrocutaneous fistula. This case illustrates all of the typical features of this disease: late diagnosis, non-functioning affected kidney, staghorn calculi, urinary tract anomaly, perinephritis with fistulization. The authors review the diagnostic and therapeutic modalities of xanthogranulomatous pyelonephritis and discuss the other aetiologies of nephrocutaneous fistula. In view of the severity of this disease and its preoperative resemblance to renal cancer, nephrectomy is often the only available treatment opt…

The aim of the study was to analyse factors affecting the success rate of extracorporeal shock wave lithotripsy (ESWL) in children with rena … pt in the presence of unfavourable lower caliceal anatomy. Increased stone burden, multiple stones, staghorn calculi, narrow lower infundibulopelvic angle and long lower infundibulum are factors that adversely affect the clearance rate.

A group of 44 patients, aged from 10 months to 14 years, underwent surgery for urinary calculi over a 7-year period (1982-1989). Eleven patients had bilateral or multiple calculi (total number of stones = 55, 20 of which were staghorn). Metabolic disorders (n = 25) and Proteus urinary infection (n = 15) were the 2 factors most often associated with lithiasis. Of the 55 stones, 51 were removed by open surgery. Complete stone clearance was achieved in 29 of 36 kidneys. Follow-up periods in the remaining 7 renal units (with small residual fragments) ranged from 3.5 to 7.5 years (mean 6.2) and rev

Urinary cytological examination was performed on 1032 patients of urolithiasis at the Department of Urology, Chiba University Hospital betwe … re found, one at nephrectomy and the other at percutaneous nephrolithotripsy. These cases were with staghorn calculi with hydronephrosis. The significance of cytological examination in management of calculous diseases were discussed.

We reviewed 53 patients with stones in a solitary kidney who had undergone percutaneous nephrolithotomy. Previous surgery on that kidney had been performed in 35.8%, and 50.9% had other medical conditions including 26.4% who had impaired renal function. Staghorn or partial staghorn calculi were present in 52.9% and an additional 18.8% had multiple stones. Postoperative complications in 18.8% of the patients included sepsis, the need for transfusion and 1 death of bronchopneumonia. Percutaneous nephrolithotomy alone resulted in a 77.3% rate free of stone or fragments of 2 mm, or less. This rate…

During the past two years, from June 7, 1974 to August 9, 1976, the authors used the Polish tissue adhesive "Chirurcoll/Polfa" in 60 operated patients. The operations were performed on the solitary kidney, on the kidneys of staghorn calculi, in fixation of a floating kidney and, fixation and elevation of the bladder in women with urinary stress incontinence. In all cases the postoperative course was smooth and the results of operation were satisfactory. None of the patients were re-operated and histological examinations were not performed.

BACKGROUND: The coexistence of upper tract urothelial carcinoma (UTUC) and human immunodeficiency virus (HIV) infection poses unique diagnostic and therapeutic challenges. Literature on the perioperative management and outcomes of curative surgery for UTUC in people living with HIV (PLWH), particularly when complicated by conditions such as staghorn calculi, is exceptionally scarce.
CASE PRESENTATION: A 65-year-old male presented with a 1-week history of urinary frequency and urgency. Two months prior, he had undergone ureteroscopic laser lithotripsy for a left staghorn calculus (details unav…

We report on our first 6 months' experience with the extracorporeal shock-wave lithotripter (ESWL). During this period, 410 treatments were given to 350 kidneys and ureters. Our series was characterized by a particularly high proportion (40%) of multiple and large calculi and included 25 complete staghorns. The latter required multiple treatments (up to five), but no adjunct percutaneous nephrolithotomies were used in this series. A percutaneous nephrostomy was inserted in 20 cases. Thirty-six ureteric calculi above the pelvic level were treated in situ, with 95% success. The presence of a ure

Ten years after the first application, extracorporeal shock wave lithotripsy (ESWL) has gained world-wide acceptance as first choice therapy of urinary calculi. The introduction of different shock wave sources, shock wave coupling and imaging techniques have influenced treatment comfort, need of anesthesia, and costs of the device, but did not improve efficacy of treatment regarding complete disintegration of calculi without need for secondary treatments. Therefore the optimal device for lithotripsy providing high efficacy, no need for anesthesia, and inducing minimal tissue trauma has not bee

Urolithiasis in renal allografts is rare, but large or complex stones present unique surgical challenges due to altered pelvic anatomy and proximity to vascular and visceral structures. Percutaneous nephrolithotomy (PCNL) is the gold-standard treatment for staghorn calculi but is technically demanding in transplanted kidneys. This complexity is amplified in recipients of simultaneous pancreas-kidney (SPK) transplants, where both grafts drain into the bladder. We describe a man in his 40s with type 1 diabetes and end-stage renal disease who underwent SPK transplantation in 2018. He presented wi…

Ultrasound imaging was conducted in 30 patients during operation for removal of renal and ureteral stones, in most cases after contact radiography with small sterile films, and in all cases before opening the renal pelvis to avoid penetration of an air bubble into the intrarenal excretory tract. Each bubble may, because of its weak acoustic impedance, provoke total reflection of the waves simulating the image of a stone. Ultrasound provides data on cortical projection of calculi, the thickness of the parenchyma and the echoless avascular zones. All complex calculi (from Staghorn to caliceal ty

Xanthogranulomatous pyelonephritis (XGP) is a rare variant of chronic pyelonephritis. It is characterized by progressive parenchymal destruction caused by chronic renal obstruction due to calculus, stricture, or rarely tumor, resulting in kidney function loss. Herein, we describe the case of a 36-year-old female who presented with left loin pain, left lower limb pain, and dysuria. On contrast-enhanced computed tomography (CECT), multiple abscesses and an obstructive staghorn calculus were depicted in the left kidney with the classical appearance of "Bear Paw Sign." An abscess with calculi was

In 416 patients with renal and ureteric calculi other than large staghorns we found that when it was possible to identify the calyx of origin of the stones, the lower poles were involved in 36-6% of males and 40-4% of females. In the follow up of the 115 lower poles originally containing stones the recurrence rate was 17% in those who were merely observed, 56% in those treated by pyelo- or nephrolithotomy, and zero in those treated by partial neprectomy. But when these cases were looked at more closely it was found that recurrences were virtually confined to lower poles which had originally ha

BACKGROUND: The management of complete staghorn stones remains a challenge for urologists, owing to the high stone burden, low stone free rate, and high rate of complications. Hence, we aimed to evaluate the outcomes of a technique involving combination laparoscopy and nephrolithotomy in the same session in patient with complete staghorn stones and poor performance status.
METHODS: We retrospectively evaluated seven patients with complete staghorn stones who underwent a combination of laparoscopy and nephrolithotomy in the same session in our center between December 2016 and October 2019. The

Xanthogranulomatous pyelonephritis is a rare form of chronic pyelonephritis in which the involved areas of the kidneys are destroyed and replaced by foam cells. It usually occurs in immunocompromised middle-aged females with ureteral obstruction or chronic urinary tract infection induced by the formation of renal stones. We herein report the case of a 44-year-old woman, with a history of left kidney staghorn calculi and recurrent urinary tract infections. Abdominal computed tomography showed the 'bear paw' sign, typical appearance of XGP and patient underwent a nephrectomy with uneventful post…
