The "mini brain": plasmacytoma in a vertebral body on MR imaging.

OBJECTIVE: Patients with solitary plasmacytoma in the spine frequently require a biopsy for diagnosis of their condition. We report an appearance of plasmacytoma in the spine, which is sufficiently pathognomonic to obviate biopsy. CONCLUSION: Identification of a "mini brain" in an expansile lesion in the spine is characteristic of plasmacytoma. It is important that radiologists note this characteristic because biopsy can be avoided in patients with this appearance. Although biopsy might still be required at many institutions, at our institution, surgeons find this appearance sufficiently path

Preoperative estimation of screw fixation strength in vertebral bodies.

STUDY DESIGN: Bone mineral density and bone cross-sectional area of human cadaveric vertebral bodies were investigated radiologically and histologically, respectively. After ventral instrumentation with ventral derotation spondylodesis screws, axial pullout force was measured and compared with radiologic and histologic data. OBJECTIVES: To elucidate how well ventral derotation spondylodesis screw fixation strength can be estimated before surgery by specified applications of dual-energy x-ray absorptiometry, quantitative computed tomography, T2*-relaxation time in magnetic resonance imaging, a

Gender differences in vertebral body sizes in children and adolescents.

PURPOSE: To determine whether differences in vertebral bone densities or sizes account for gender differences in skeletal mass during growth. MATERIALS AND METHODS: Quantitative computed tomography (CT) was used to measure the densities of cortical and cancellous bone and dimensions of the lumbar vertebral bodies in 196 healthy children and adolescents, ages 4-20 years. RESULTS: Neither cancellous nor cortical bone densities differed between boys and girls with age or level of sexual development. In contrast, the cross-sectional areas of the vertebral bodies were greater in boys than girls t

Fracture of the S1 vertebral body in a patient with ankylosing spondylitis.

We report an unusual case of spinal fracture in ankylosing spondylitis. The fracture occurred through the S1 vertebral body and resembled a traumatic spondylolisthesis. Treatment consisted of prompt operative intervention after extensive diagnostic evaluation. The case emphasizes the difficulty of diagnosing spinal fractures in those with ankylosing spondylitis, the necessity for being alert to the possibility of fracture in this group, and the value of early surgical intervention for sacral fractures with neural deficit.

[The intravertebral vacuum phenomenon or gaseous dissection of the vertebral body].

The intravertebral vacuum phenomenon is rather uncommon. It is determined by the collapse of the body of the vertebra with successive aspiration ex vacuo of gas (nitrogen) from the surrounding tissues, which collects under the endplates, causing them to appear disconnected. To date, the one who has reported the highest number of cases is Maldague, with sixty examples. Our report concerns four cases seen during the past five years. According to the most widely accepted pathogenetic theory, this phenomenon represents, together with vertebral collapse and increased bone density, a pathognomonic s

[Risk factors for the collapse of cemented vertebral bodies after the minimally invasive vertebral augmentation procedures].

OBJECTIVE: To evaluate the incidence rate, the independent factors, and the prevention measures of the collapse of cemented vertebral bodies after minimally invasive vertebral augmentation procedures including percutaneous vertebroplasty (PVP) and percutaneous kyphoplasty(PKP). METHODS: From January 2012 to June 2013, 154 patients with single segmental osteoporotic vertebral compression fractures(OVCF)were treated with PVP or PKP. There were 65 males and 89 females, ranging in age from 57 to 90 years old, with a mean of (76.20±9.35) years old. All the patients were followed up, and the durati

Vertebral artery location in relation to the vertebral body as determined by two-dimensional computed tomography evaluation.

STUDY DESIGN: This study analyzed the precise two-dimensional location of the vertebral artery within cervical vertebrae as determined by measurements obtained from axial computed tomographic images of the cervical spine. OBJECTIVE: To determine the margin of safety necessary to avoid vertebral artery laceration during central decompression and lateral nerve root decompression for cervical spinal stenosis. SUMMARY OF BACKGROUND DATA: Laceration of the vertebral artery is a rare but potentially catastrophic complication of anterior decompressive surgery of the cervical spine. METHODS: The me

Spinal augmentation for vertebral body fractures in the elderly population.

Osteoporotic vertebral body fractures (OVBFs) are a highly prevalent pathology in the United States elderly population and can result in height loss and kyphotic deformity. While minimally invasive surgical techniques such as percutaneous kyphoplasty (PKP) and vertebroplasty (PVP) are commonly used, the efficacy of these techniques in older individuals is unknown. By examining the current body of evidence, we aim to assess the efficacy of these procedures within the context of appropriate grading of levels of evidence. This narrative review was conducted by searching multiple databases, includ

Translocation of Cervical Vertebral Body Replacement Device into the Esophagus.

Expandable vertebrectomy devices are a key technology that has facilitated the adoption of minimally invasive approaches to spine oncology surgery. However, advanced technology still requires proper attention to surgical fundamentals. Here we illustrate a cage of a misplaced expandable vertebrectomy device causing esophageal perforation. Examination of the postoperative radiographs suggests that haptic feedback from the expandable technology may have given the false impression of bony engagement. This case highlights the need for proper mortise work and complete visualization of the segments t

Rate-dependent fracture characteristics of lumbar vertebral bodies.

Experimental testing incorporating lumbar columns and isolated components is essential to advance the understanding of injury tolerance and for the development of safety enhancements. This study incorporated a whole column axial acceleration model and an isolated vertebral body model to quantify compression rates during realistic loading and compressive tolerance of vertebrae. Eight lumbar columns and 53 vertebral bodies from 23 PMHS were used. Three-factor ANOVA was used to determine significant differences (p<0.05) in physiologic and failure biomechanics based on compression rate, spinal lev

Photoelastic analisys in the lower region of vertebral body L4.

OBJECTIVE: To analyze the shear forces on the vertebral body L4 when submitted to a compression force by means of transmission photoelasticity. METHODS: Twelve photoelastic models were divided into three groups, with four models per group, according to the positioning of the sagittal section vertebrae L4-L5 (sections A, B and C). The simulation was performed using a 15N compression force, and the fringe orders were evaluated in the vertebral body L4 by the Tardy compensation method. RESULTS: Photoelastic analysis showed, in general, a homogeneous distribution in the vertebral bodies. The she

Increase in vertebral body size in postmenopausal women with osteoporosis.

Bone geometry plays a prominent role in bone strength. Cross-sectional studies have shown that advancing age is associated with increasing diameter of long bones, related to both periostal apposition and endosteal resorption. However, there are few data provided by prospective studies, especially concerning the changes in vertebral body dimensions. The objective of this prospective study was to measure the changes occurring in the vertebral body size of women with postmenopausal osteoporosis. Three-year data from placebo groups of the SOTI and TROPOS trials, performed in women with postmenopau
