Hypoparathyroidism-retardation-Dysmorphism (HRD) syndrome--a review.

Hypoparathyroidism, retardation, and dysmorphism (HRD) is a newly recognized genetic syndrome, described in patients of Arab origin. The syndrome consists of permanent congenital hypoparathyroidism, severe prenatal and postnatal growth retardation, and profound global developmental delay. The patients are susceptible to severe infections including life-threatening pneumococcal infections especially during infancy. The main dysmorphic features are microcephaly, deep-set eyes or microphthalmia, ear abnormalities, depressed nasal bridge, thin upper lip, hooked small nose, micrognathia, and small

Non-immune hydrops fetalis associated with impaired fetal movement: a case report and review.

Non-immune hydrops fetalis (NIHF) is due to many different causes. Fetal hypomobility has been alluded to as a possible cause. We present a preterm fetus with NIHF secondary to fetal hypomobility. Fetal movements were undetected after the 20th week of gestation. The infant was born 8 weeks later and was edematous, had pleural effusions, and no spontaneous movements. He died on day four of life. Diffuse massive central nervous system (CNS) destruction found on post-mortem examination was thought to be the origin of the hypomobility. As all other causes of NIHF were eliminated, we propose that t

Klippel-Trenaunay and Sturge-Weber syndromes with renal hemangioma and double inferior vena cava.

We describe a 3 1/2-year-old boy with the Klippel-Trenaunay and Sturge-Weber syndromes. The child had congenital superficial capillary hemangiomas, congenital glaucoma and mild hydrocephalus. During the first year of life he experienced intermittent hematuria. When he was 3 years old he presented with seizures and left hemihypertrophy first was noted. Several months later radiological examination of a large abdominal mass demonstrated its origin to be in the right kidney. Radical nephrectomy documented the presence of renal hemangioma with complicating perirenal hematoma. A double inferior ven

Transfer and persistence of viral antibody-producing cells in bone marrow transplantation.

Antiviral humoral reactivity was studied after bone marrow transplantation. Antiviral IgG--reflecting the presence of functional B cells--was found for a substantial period after transplantation. However, most patients unexpectedly stopped producing antibody after an extended period of observation (i.e., for more than three months and up to as long as 12 months). This cessation was noted whether the viral reactivity involved was of donor or recipient origin. The transferred viral antibody-producing cells thus usually persisted for only a moderate length of time and seemed to have a finite life

A miniaturized implantable electrochemical platform for continuous monitoring of metabolites in deep tissue.

The loss or suppression of local metabolic function in living tissues brought about by a lack of oxygen availability (ischemia) serves as the origin for myriad life-threatening conditions including stroke, heart failure, and peripheral ischemic injury. Despite the pressing clinical need to evaluate local tissue health, platforms that support such analysis remain elusive. To address this need, we present a wireless, minimally invasive, multianalyte, electrochemical probe supporting the continuous profiling of local metabolites including K+ and pH, and the semicontinuous profiling of lactic acid

Two Hurt More Than One: Severe Hyponatraemia and Rhabdomyolysis as Presenting Features of Addison's Disease.

Addison's disease (AD) is characterized by non-specific symptoms and electrolyte disorders, namely hyponatraemia and hyperkalaemia; rhabdomyolysis is uncommon. AD may manifest at onset with a life-threatening adrenal crisis which is triggered by stressful events. We describe the case of a young man who was hospitalized for severe myalgia and fatigue. Severe hypotonic hyponatraemia, rhabdomyolysis and hypotension were found; hormonal assessment unexpectedly revealed primary adrenal insufficiency. Saline infusion and intravenous hydrocortisone significantly improved the patient's condition and n

Acquired Dermal Macular Hyperpigmentation: An Update.

Acquired dermal macular hyperpigmentation (ADMH) is an umbrella term that includes disorders clinically characterized by small and large pigmented macules/patches and histopathologically showing an evidence of current or resolved interface dermatitis with pigment incontinence, without clinically significant prior inflammatory phase. The term intends to include diseases previously described in the literature as lichen planus pigmentosus, Riehl's melanosis/pigmented cosmetic dermatitis and ashy dermatosis/erythema dyschromicum perstans. The nomenclature and origin of these disorders have always

Neurofibromatosis 1 and massive hemothorax: a fatal combination.

Type 1 neurofibromatosis (NF 1), a rare genetic disease with autosomal dominant transmission, has typical dermatologic manifestations with pathognomonic Lisch nodules, and is rarely known for vascular alterations. Among these, aneurysmal dilatation is the most common form. We report a fatal case of massive hemothorax due to a spontaneous rupture of the left pulmonary artery branch micro-aneurysm in a NF 1 patient. Indeed, spontaneous rupture of these pathologic vessels is very rare in clinical practice and the literature, but, for its potentially life-threatening complications, there is the ne

Catastrophic presentation of anomalous origin of right pulmonary artery in a neonate--an interesting differential!

A 36-hour-old term neonate presented with refractory cardiogenic shock and multi-organ dysfunction. On suspecting coarctation of aorta, intravenous prostaglandin was started but without any benefit. Echocardiography revealed isolated anomalous origin of right pulmonary artery from ascending aorta causing massive aortic steal, supra-systemic pulmonary arterial hypertension (PAH), multi-organ dysfunction and right ventricular failure. The main pulmonary artery was solely continued as left pulmonary artery. Emergency surgery at 48 h was life-saving and mid-term follow-up is excellent. The authors

Granular cell tumor of the vulva: literature review and case report.

Granular cell tumors are an uncommon neoplasm of neural origin. They occur throughout the body; only about 10% are found in the vulvar region. Granular cell tumors are most frequent in the third and fourth decade of life, the African-American race, and the female. Generally, they appear as small, firm, painless, slow-growing subcutaneous nodules. They are rarely encapsulated and recurrence is common. The malignant granular cell tumor is rare, representing 1% to 2% of cases. This form is highly aggressive, unresponsive to treatment, and ultimately fatal. A third type of granular cell tumor has

Duodenal webs in the adult.

Duodenal webs and diaphragms in the adult are congenital in origin and are rare. They are explained embryologically by the process of duodenal epithelial plugging and subsequent revacuolization. The symptoms may be insidious and appear late in life and consist of partial gastric outlet complaints. Two thirds of the patients will have other upper GI problems, some of which may be related to the antral distension and stasis produced by the web. Peptic ulcer disease is common but may resolve when the web is excised. Diagnosis can be made by contrast duodenography but is more definitive with endos

Integrated targeted sequencing reveals unique tissue-of-origin and donor-derived cell-free DNA signatures in stable organ transplant recipients.

Solid organ transplantation is a life-saving treatment for end-stage organ disease, yet current methods to assess graft health rely on invasive biopsies. We applied an integrated targeted deep sequencing assay to simultaneously quantify donor-derived and tissue-of-origin (TOO) cell-free DNA (cfDNA) in plasma from liver and kidney transplant recipients and healthy individuals. The assay sensitively detected cell-type-specific cfDNA and revealed that cfDNA composition differed markedly between stable transplant recipients and healthy individuals. Early after transplantation, dynamic cfDNA change

Tinea barbae following ruxolitinib cream application in a vitiligo patient: a case report.

Ruxolitinib 1.5% cream, a topical Janus Kinase (JAK) 1/2 inhibitor, represents a novel targeted treatment for non-segmental vitiligo, a common acquired depigmentation disorder of autoimmune origin clinically characterized by milky-white patches on the affected body areas. The efficacy of topical ruxolitinib in vitiligo has been demonstrated in clinical trials and real-life observation, with an overall safety profile. Mild-to-moderate reported adverse events include site-application acne, nasopharyngitis, and site-application pruritus, while increased susceptibility to viral skin infections, du

Utility of cardiac imaging in patients with ventricular tachycardia.

Ventricular tachycardia (VT) is a life-threatening arrhythmia that may be idiopathic or result from structural heart disease. Cardiac imaging is critical in the diagnostic workup and risk stratification of patients with VT. Data gained from cardiac imaging provides information on likely mechanisms and sites of origin, as well as risk of intervention. Pre-procedural imaging can be used to plan access route(s) and identify patients where post-procedural intensive care may be required. Integration of cardiac imaging into electroanatomical mapping systems during catheter ablation procedures can fa

Eruptive pseudoangiomatosis in adults with immune system disorders: A report of two cases.

Eruptive pseudoangiomatosis is a cutaneous disease of unknown origin, characterized by the sudden appearance of small, asymptomatic angioma-like erythematous papules surrounded by a pale halo that resolves spontaneously. It occurs due to transitory dermal blood vessel dilation and has been associated with viral infections and hematologic disorders. To this day, it remains a disease that affects mostly the lower spectrum of life. Most of the cases have been reported in children and few reports have described its association with immune system disorders. The diagnosis is based upon clinical pres

Successful Mesenchymal Stem Cell Application in Supraventricular Tachycardia-Related Necrotizing Enterocolitis: A Case Report.

Necrotizing enterocolitis (NEC) is one of the most important life- threatening diseases in neonates. Recurrent episodes of supraventricular tachycardia (SVT) can lead to gut ischemia, with subsequent reperfusion injury resulting in NEC. Mesenchymal stem cells (MSCs) are multi-potential cells which can differentiate into multiple cell types. Case report: A 22-day old male baby delivered to 34 yr old mother at 37-weeks' gestation via cesarean section, birth weight 3550 g, developed NEC 14 hours after being treated with cardioversion for SVT. At laparotomy, there was pan-NEC, and 60 cm of necroti

First Bulgarian case of citrin deficiency caused by one novel and one recurrent mutation in the SLC25A13 gene.

Citrin deficiency is an autosomal recessive disorder caused by mutations in the SLC25A13 gene and has three phenotypes: neonatal intrahepatic cholestasis caused by citrin deficiency (NICCD) in newborns, failure to thrive and dyslipidemia caused by citrin deficiency (FTTDCD) in older children, and recurrent hyperammonemia with neuropsychiatric symptoms in citrullinemia type II (CTLN2) in adults. NICCD presents in the first few weeks of life with cholestatic hepatitis syndrome, multiple aminoacidemia and hypergalactosemia. To date almost all reported patients were from East Asia and only few cas

Percutaneous vertebroplasty for thyroid cancer metastases to the spine.

BACKGROUND: Spinal metastases secondary to thyroid cancer of follicular and parafollicular cell origin are uncommon but may require stabilization of the compromised vertebrae to prevent fracture with spinal cord injury. Such treatment may also relieve pain and improve survival and quality of life. SUMMARY: Percutaneous vertebroplasty (PV) is a minimally invasive, radiologically guided procedure whereby bone cement is injected into a structurally weakened vertebra to provide immediate stability. The authors present two cases of thyroid cancer with spinal metastases. Both patients successfully

Intraoral tumor with rapid growing. Report of a case.

The appearance of an intraoral mass is common in our specialty. Most are benign lesions, but some are primary malignancies. Metastases account for less than 1% of all oral malignancies. An 86 year old woman was referred to our department with a large, asymptomatic, intraoral, fast-growing mass. She had no previous cancer history or other relevant physical findings. The radiology studies showed underlying bone erosion. The histological study showed a metastatic adenocarcinoma with a suspected origin in the abdomen. We were unable to identify it by non invasive diagnostic procedures. Given the p
