Giant cell arteritis is frequently an attendant complication of polymyalgia rheumatica. While involvement of other arterial beds is occasionally identified, this syndrome is most frequently recognized when symptomatic involvement of the temporal arteries occurs. We describe such a patient who despite initial treatment with high-dose intravenous and standard (5-day) orally administered corticosteroid therapy suffered a fatal myocardial infarction, which at autopsy was shown to be a consequence of granulomatous coronary arteritis. The literature is reviewed and the clinical implications of this …

Giant cell arteritis (GCA), also called as temporal arteritis, is a systemic vasculitis, primary affecting individuals over 50 years of age and mostly women. The disease can manifest with symptoms of systemic inflammatory response syndrome as well with symptoms of systemic vasculitis affecting mainly medium and large vessels, including branches of the carotid artery, first of all the ophthalmic and temporal. Pathogenetically, arterial damage in GCA is a combination of autoinflammatory and autoimmune processes, resulting in vascular wall remodeling with narrowing of the arterial lumen and subse…

Biological therapy in patients with Takayasu arteritis is frequently used, but the factors associated with its use have not been determined. The present study aims to describe and identify predictors of the need for biological therapy in this vasculitis. We conducted a retrospective, multicentre, descriptive study including all patients with Takayasu arteritis enrolled in the REVAS registry from Spain. We performed a descriptive and a multivariate logistic regression analysis to identify variables associated with the need of biological therapy requirement. The study included 77 patients, 65 (8…

A 63-year-old woman presented with a four-month history of bilateral hearing loss and three months of progressive, painful peripheral neuropathy. Her clinical course was complicated by deep vein thrombosis with pulmonary emboli and an incomplete response to initial empiric glucocorticoid therapy. Following glucocorticoid taper, she developed temporal headaches, jaw claudication, and scalp tenderness, raising concern for giant cell arteritis (GCA). Laboratory evaluation revealed markedly elevated inflammatory markers (erythrocyte sedimentation rate and C-reactive protein), microcytic anemia, mi…

BACKGROUND: Takayasu arteritis (TAK) is a rare granulomatous inflammatory vasculitis primarily affecting the aorta and its major branches. Data on childhood-onset TAK (c-TAK) remain scarce. This study retrospectively evaluates the clinical presentation, disease flares, treatment, and outcomes of c-TAK in a tertiary Canadian center.
METHODS: We identified all children under 18 years of age at disease onset with a clinical diagnosis of TAK seen at Alberta Children's Hospital, Calgary, Canada, between 2000 and 2024. Patients meeting the EULAR/PRINTO/Pres classification criteria for c-TAK were in…

Giant cell arteritis (GCA) and polymyalgia rheumatica (PMR) are closely related inflammatory diseases that predominantly affect individuals over 50 years of age. The pathogenesis of two diseases is unclear. Accumulating evidence indicates that aberrant innate and adaptive immune responses underlie the pathogenesis of GCA and PMR. Monocytes/macrophages play an important role in the inflammatory processes through producing proinflammatory cytokines and chemokines, modulating molecular expression, colony-stimulating factors, proteolytic enzymes, and growth factors, and activating the JAK/STAT pat…

We aim to better define the association between thoracic aortic aneurysm (TAA) and giant cell arteritis (GCA), thereby enhancing cross-diagnosis, monitoring, and therapy.Literature review: We used a two-step search approach to the available literature on the relationship between TAA and GCA. First, databases including PubMed, Web of Science, and Embase were searched. Additionally, relevant studies were identified through secondary sources including references of initially selected articles.Retrospective cohort study: We identified patients at our institution who were diagnosed with both TAA an…

Lingual necrosis is a rare but serious complication of giant cell arteritis (GCA). Diagnosis can be difficult due to its atypical presentation, particularly when it occurs without the usual GCA symptoms. We present the case of a 59-year-old female with a history of migraines and smoking, who developed severe tongue pain and neck discomfort. Glucocorticoid therapy was promptly initiated due to clinical suspicion of GCA. However, when the steroid dosage was reduced, the patient's symptoms worsened despite initial improvement. Tocilizumab was subsequently introduced, and by discharge, there was a…

Takayasu arteritis (TA) is a rare form of large vessel arteritis that predominantly affects the aorta and its major branches. This inflammation leads to thickening, fibrosis, and stenosis of the arterial walls, which may lead to thrombus formation. The resulting symptoms are typically due to ischemia of the end organs. Coronary artery involvement is uncommon and primarily affects the ostia of the arteries. Ostial involvement of the coronary arteries can have a dramatic course, including fatal outcomes. We present the case of a 16-year-old female with TA involving the ostium of the left main co…
