Successful Use of OviTex® Bridging Repair in a Super Morbidly Obese Patient With Septic Shock, Perforated Diverticulitis, and Necrotizing Fasciitis of the Abdominal Wall.

Managing abdominal wall defects in contaminated surgical fields is particularly challenging in super morbidly obese patients with severe infection. We report the case of a 49-year-old man (BMI 54, 195 kg) who presented in septic shock with necrotizing fasciitis of the abdominal wall secondary to perforated diverticulitis and colocutaneous fistula. Initial management included exploratory laparotomy, extensive debridement of a 25 × 25 cm abdominal wall abscess, anterior resection left in discontinuity, and temporary abdominal closure with an AbThera™ wound vacuum (Solventum, Saint Paul, MN, USA)…

Accuracy of preoperative CT staging of acute colonic diverticulitis using the classification of diverticular disease (CDD) - Is there a beneficial impact of water enema and visceral obesity?

PURPOSE: We evaluated the accuracy of preoperative CT in staging colonic diverticulitis (ACD) by using the classification of diverticular disease (CDD) and investigated the diagnostic impact of water enema (WE) and visceral obesity. METHODS: In this retrospective study, the radiological and hospital information system was searched for patients who underwent CT for clinically suspected ACD prior to surgery between 2009 and 2019. From the initial population (n = 164), we included 155 patients (94.5 %) (85 women; mean age: 58 ± 13 years) matching the following inclusion criteria: i.) clinically …

Assessing the Optimal Surgical Approach for Complicated Diverticulitis in the Emergency Setting: Results from a Cohort Study Using National Surgical Quality Improvement Program Database.

Background: The optimal surgical approach for complicated diverticulitis in the emergency setting remains controversial. This retrospective cohort study aimed to investigate the clinical outcomes associated with Hartmann's procedure (HP) and primary anastomosis (with and without ileostomy) in patients diagnosed with complicated diverticulitis. Methods: Data were collected from the American College of Surgeons-National Surgical Quality Improvement Program participant user files from 2016 to 2021. A total of 25,458 patients with acute diverticulitis and emergency surgery were included. The patie…

Laparoscopic Surgery for Acute Right-colon Diverticulitis: Video Vignette and Systematic Review With Meta-analysis of Current Evidence of Minimally Invasive Versus Conventional Surgery.

BACKGROUND: Over the last decade, there has been growing diffusion of minimally invasive surgery in the setting of abdominal emergencies. However, right-colon diverticulitis is still mainly approached by conventional celiotomy. MATERIALS AND METHODS: A video vignette is presented showing the details of an emergent laparoscopic right colectomy as performed to treat a 59-year-old woman who presented with clinical signs of peritonitis, and radiologic findings suggestive of acute right-colon diverticulitis complicated by perforation of the hepatic flexure and periduodenal abscess. We also aimed t…

Short- and long-term outcomes for primary anastomosis versus Hartmann's procedure in Hinchey III and IV diverticulitis: a multivariate logistic regression analysis of risk factors.

PURPOSE: The management of perforated diverticulitis with generalized peritonitis is still controversial and no preferred standardized therapeutic approach has been determined. We compared surgical outcomes between Hartmann's procedure (HP) and primary anastomosis (PA) in patients with Hinchey III and IV perforated diverticulitis. METHODS: Multicenter retrospective analysis of 131 consecutive patients with Hinchey III and IV diverticulitis operated either with HP or PA from 2015 to 2018. Postoperative morbidity was compared after adjustment for known risk factors in a multivariate logistic re…

American Association for the Surgery of Trauma emergency general surgery guideline summaries 2018: acute appendicitis, acute cholecystitis, acute diverticulitis, acute pancreatitis, and small bowel obstruction.

In April 2017, the American Association for the Surgery of Trauma (AAST) asked the AAST Patient Assessment Committee to undertake a gap analysis for published clinical practice guidelines in emergency general surgery (EGS). Committee members performed literature searches to catalogue published guidelines for common EGS diseases and also to identify gaps in the literature where guidelines could be created. For five of the most common EGS conditions, acute appendicitis, acute cholecystitis, acute diverticulitis, acute pancreatitis, and small bowel obstruction, we found multiple well-referenced g…

Role of protective stoma after primary anastomosis for generalized peritonitis due to perforated diverticulitis-DIVERTI 2 (a prospective multicenter randomized trial): rationale and design (nct04604730).

BACKGROUND: Traditionally, patients with peritonitis Hinchey III and IV due to perforated diverticulitis were treated with Hartmann's procedure. In the past decade, resection and primary anastomosis have gained popularity over Hartmann's procedure and recent guidelines recommend Hartmann's procedure in two situations only: critically ill patients and in selected patients with multiple comorbidity (at high risk of complications). The protective stoma (PS) is recommended after resection with primary anastomosis, however its interest has never been studied. The aim of this trial is to define the …

Long-term outcomes of Hartmann's procedure versus primary anastomosis for generalized peritonitis due to perforated diverticulitis: follow-up of a prospective multicenter randomized trial (DIVERTI).

BACKGROUND: Surgical management of Hinchey III and IV diverticulitis involves Hartmann's procedure (HP) or primary resection anastomosis (PRA) with or without fecal diversion. These procedures were evaluated in four randomized controlled trials. Early results from these trials demonstrated similar rates of complications but higher rates of colonic restoration after PRA than HP. Long-term follow-up has not been reported to date. The aim of this study was to analyze long-term outcomes and quality of life (QoL) in patients previously enrolled in a prospective randomized trial comparing HP and PRA…

Hartmann's procedure versus sigmoidectomy with primary anastomosis for perforated diverticulitis with purulent or faecal peritonitis (LADIES): a multicentre, parallel-group, randomised, open-label, superiority trial.

BACKGROUND: Previous studies have suggested that sigmoidectomy with primary anastomosis is superior to Hartmann's procedure. The likelihood of stoma reversal after primary anastomosis has been reported to be higher and reversal seems to be associated with lower morbidity and mortality. Although promising, results from these previous studies remain uncertain because of potential selection bias. Therefore, this study aimed to assess outcomes after Hartmann's procedure versus sigmoidectomy with primary anastomosis, with or without defunctioning ileostomy, for perforated diverticulitis with purule…

Short Occult Retractile Fibrous Scar Causing Exclusive Retrograde Stenosis of the Sigmoid Colon: An Incidental Diagnosis Nineteen Years After a Single Episode of Colonic Diverticulitis.

We report a rare case of purely retrograde stenosing stricture of the sigmoid descending colonic junction fortuitously diagnosed during the waning of a failed virtual colonoscopy in a 69-year-old patient. The rather asymptomatic patient was addressed to investigate a positive fecal occult blood test. He had suffered a single acute colonic diverticulitis episode 19 years before. A contrast-enhanced abdominal CT and complementary focused abdominal ultrasound fully diagnosed a short curvilinear contrast-enhancing "scar-like" tissue infiltrating the posterior colonic wall and developing retractile…

Complicated Diverticulitis With Abscess Managed Nonoperatively at Index Admission: A Longitudinal Analysis of State Inpatient and Emergency Department Databases From the Healthcare Cost and Utilization Project.

INTRODUCTION: In 2020, clinical practice guidelines for the management of complicated diverticulitis shifted from recommending interval elective colectomy to consideration of elective resection. However, population level evidence for long-term outcomes following nonoperative management of complicated diverticulitis with abscess is limited. METHODS: We performed a retrospective longitudinal analysis of hospital admissions for acute diverticulitis complicated by abscess from 2005 to 2015 from the State Inpatient and Emergency Department Databases from the Agency for Healthcare Research and Qual…

Analysis of the correlation between abdominal pain and long Meckel's diverticulitis on the mesenteric side: a case study in the context of mesenteric volvulus and internal hernias: a case report.

BACKGROUND: Meckel's diverticulum is a common congenital gastrointestinal malformation that affects approximately 2% of the population. While the majority of individuals with this condition remain asymptomatic, complications such as intestinal obstruction, hemorrhage, and inflammation can occur, leading to significant clinical implications. Despite its established prevalence, the pathophysiological mechanisms underlying Meckel's diverticulum and its associated complications are not well understood, presenting diagnostic challenges for medical professionals. CASE PRESENTATION: This report desc

Challenging a classic myth: pneumoperitoneum associated with acute diverticulitis is not an indication for open or laparoscopic emergency surgery in hemodynamically stable patients. A 10-year experience with a nonoperative treatment.

BACKGROUND: In patients presenting with acute diverticulitis (AD) and signs of acute peritonitis, the presence of extradigestive air (EDA) on a computer tomography (CT) scan is often considered to indicate the need for emergency surgery. Although the traditional management of "perforated" AD is open sigmoidectomy, more recently, laparoscopic drainage/lavage (usually followed by delayed elective sigmoidectomy) has been reported. The aim of this retrospective study is to evaluate the results of nonoperative management of emergency patients presenting with AD and EDA. METHODS: The outcomes of 39…

The DICA endoscopic score and the CODA clinical score may predict the severity of acute diverticulitis and the risk of hospitalisation: results from an international multicentre prospective cohort study.

BACKGROUND: We assessed whether baseline endoscopic and clinical scores of diverticular disease (DD) may predict the severity of acute diverticulitis (AD), hospital admission and length of hospital stay (HS). METHODS: We conducted a 3-year, multicentre, prospective cohort study involving 2215 patients. DD was scored according to the Diverticular Inflammation and Complication Assessment (DICA) classification and the Combined Overview on Diverticular Assessment (CODA) score. RESULTS: Higher baseline DICA and CODA scores were associated with increased risk of complicated AD [relative risk ratio…
