The Effects of Low-Dose Esketamine Combined with Paravertebral Block on Postoperative Hyperalgesia and Enhanced Recovery in Non-Intubated Video-Assisted Thoracic Surgery: A Randomized Controlled Trial.

PURPOSE: Non-intubated video-assisted thoracic surgery (NIVATS) reduces airway trauma but may lead to postoperative hyperalgesia and opioid dependence, contradicting enhanced recovery after surgery (ERAS) principles. We hypothesized that combining low-dose esketamine with a paravertebral block (PVB) may mitigate hyperalgesia, decrease opioid requirements, and improve recovery quality in NIVATS. PATIENTS AND METHODS: This prospective single-center, double-blind randomized controlled trial (RCT) enrolled 82 patients undergoing uniportal NIVATS. Patients were randomized into two groups: esketami…

Barriers to early rehabilitation and support needs among elderly hip fracture patients with kinesiophobia: a qualitative study.

PURPOSE: To explore the early rehabilitation experiences of elderly patients with postoperative kinesiophobia following hip fractures. METHODS: This qualitative inquiry adopted a phenomenological approach. 15 elderly patients with postoperative kinesiophobia were recruited 2-7 days after surgery to repair a hip fracture. Data were collected through semi-structured interviews and analyzed using Colaizzi's seven-step method. FINDINGS: The study identified five major themes and five sub-themes: (1) Fear is a barrier to progress in rehabilitation (fear of falling, fear of poor recovery, fear of

Tegileridine vs. sufentanil for postoperative analgesia in general surgery: A prospective, randomized, controlled, non-inferiority trial.

OBJECTIVE: Tegileridine, a novel G protein-biassed μ-opioid receptor agonist, may offer effective postoperative analgesia with fewer opioid-related adverse effects. This study evaluated its analgesic efficacy, safety and impact on surgical stress response vs. sufentanil for patient-controlled intravenous analgesia (PCIA) in general surgery patients. METHODS: In this single-centre, double-blind, non-inferiority trial, 120 adults scheduled for elective laparoscopic cholecystectomy, appendectomy or inguinal hernia repair were randomized 1:1 to 48-h PCIA with tegileridine (5 mg) or sufentanil (20

Pain and opioid use after colorectal resection for benign versus malignant disease: A single institution analysis.

BACKGROUND: Studies comparing opioid needs between benign and malignant colorectal diseases are inconclusive. METHODS: Single institution analysis of prospectively maintained colorectal surgery database. Multiple regression analyses done on perioperative numeric pain scores (NPS) and opioids prescribed at discharge. RESULTS: 641 patients in Benign and 276 patients in the Malignant group. Unadjusted comparison revealed significantly higher NPS for the Benign than the Malignant group preoperative and postoperative day 0 (after surgery), 1, 2, and 3 (all p ≤ 0.001). Opioids prescribed at disc

Preoperative prolonged fasting causes severe metabolic acidosis: A case report.

RATIONALE: Preoperative prolonged fasting may cause starvation ketoacidosis. Herein, we report of a case of starvation ketoacidosis due to l … of Gastroenterology for further therapy. LESSONS: This case suggests that although the concept of enhanced recovery after surgery (ERAS) has been adopted by most physicians because of its positive outcomes, the issue of prolonged fasting still exists, and such patients may be exposed to the risk of starvation ketoacidosis.

When Stones Escape: A Rare Radiologic Finding of Retroperitoneal Stone Extrusion Following American Association for the Surgery of Trauma (AAST) Grade IV Blunt Renal Trauma in a Chronically Hydronephrotic Kidney.

Traumatic injury of a hydronephrotic kidney is uncommon, and extrusion of calculi beyond the renal capsule is exceptionally rare. We describe a middle-aged man with severe left hydronephrosis who sustained a low-impact fall, resulting in an American Association for the Surgery of Trauma (AAST) grade IV renal laceration. Contrast-enhanced CT revealed urinary extravasation, inflammatory urothelial enhancement consistent with uretero-pyelitis, and multiple displaced calculi within the retroperitoneum. A double-J stent was inserted after the initial scan. Follow-up CT demonstrated a well-circumscr

A prospective study evaluating the feasibility and accuracy of very early postoperative translaryngeal ultrasonography in the assessment of vocal cord function after neck surgery.

BACKGROUND: Early recognition of postoperative vocal cord palsy enhances postoperative care. Translaryngeal ultrasonography can assess vocal cord function accurately and noninvasively, but it is unclear whether it is feasible or accurate when done immediately after extubation in the recovery room owing to possible interference from laryngeal swelling. This study assessed the feasibility and accuracy of translaryngeal ultrasonography in this setting. METHODS: Consecutive patients undergoing neck operations were subjected to translaryngeal ultrasonography and flexible direct laryngoscopy 1 day

Comparison of reversal of rocuronium-induced neuromuscular blockade with sugammadex under remimazolam versus propofol anesthesia: a randomized clinical trial.

PURPOSE: The anesthetic management combining remimazolam and neuromuscular blocking agents with sugammadex is expected to enhance the speed and safety of recovery from anesthesia. However, the effect of remimazolam on reversal of neuromuscular blockade with sugammadex remains unclear. We hypothesized that sugammadex could reverse rocuronium-induced neuromuscular blockade even under remimazolam anesthesia, although the recovery might be delayed. In the present study, the recovery time from rocuronium-induced neuromuscular blockade after administration of sugammadex under remimazolam anesthesia

Loss of endogenous analgesia leads to delayed recovery from incisional pain in a rat model of chronic neuropathic pain.

BACKGROUND: Preoperative pain and impaired endogenous analgesia are risk factors of chronic postsurgical persistent pain (CPSP). A Chronic neuropathic pain model induced by spinal nerve ligation (SNL6W) shows impaired endogenous analgesia and delayed recovery from incisional pain. Repeated amitriptyline treatment can restore the endogenous analgesia, but its effects on delayed recovery are not clear. METHODS: A plantar incision was made on the side contralateral to the nerve ligation in SNL6W rats. Withdrawal thresholds were measured by von Frey filament test until 28 d after surgery. Amitrip

Prostatic and pelvic imaging parameters to predict post radical prostatectomy erectile function recovery: a systematic review.

With radical prostatectomy for prostate cancer, there may be associated long-term postoperative sequalae: urinary incontinence and erectile dysfunction (ED). It is important to predict the functional recovery of erections for better patient counselling and timely treatment of ED. This systematic review looks at imaging parameters to predict the recovery of erectile function (EF) after laparoscopic or robotic prostatectomy. A systematic search was performed to capture publications from January 2000 up to December 2023 (PROSPERO; Registrations ID CRD 42022359557). The considered studies applied

Shake and Fake: the Role of Interview Anxiety in Deceptive Impression Management.

BACKGROUND: Post-operative rehabilitation after posterior-approach total hip arthroplasty (P-THA) includes the use of standard hip precautions, defined as no hip flexion beyond 90°, hip adduction, or hip internal rotation for 6 to 12 weeks after surgery (sometimes for life). Since they were first implemented in the 1970s, subsequent advances may have made standard hip precautions no longer necessary, although little evidence supports that hypothesis. A modified set of precautions, a "pose avoidance protocol," could be effective in enhancing recovery, but its effectiveness on early dislocation

Ultrasound-guided bilateral erector spinae plane nerve blocks: a novel application for the management of acute postoperative pain in awake spine surgery. Illustrative case.

BACKGROUND: Postoperative pain management is a limiting factor for early ambulation and discharge following spine fusion surgery. Awake spinal surgery, when combined with minimally invasive transforaminal lumbar interbody fusion, is associated with enhanced recovery in well-selected patients. Some neurosurgeons have recently aimed to further improve outcomes by utilizing erector spinae plane block catheters, allowing for a continuous infusion of local anesthetic to improve the management of acute postoperative pain following minimally invasive transforaminal lumbar interbody fusion.
