Prevalence of illicit substance use among patients presenting to the emergency department with acute behavioural disturbance: Rapid point-of-care saliva screening.

OBJECTIVE: To determine the prevalence of illicit substance use among patients presenting to one ED with acute behavioural disturbance using point-of-care saliva testing. METHODS: A prospective observational study was conducted. Acute behavioural disturbance was defined as any episode requiring a security response for unarmed threat (Code Grey). The setting was a single ED and tertiary referral centre located in metropolitan Australia. Participants were adults presenting to the ED requiring a Code Grey. Saliva was analysed for meth/amphetamine, cannabis, cocaine and opiates using a rapid poin

Overcoming analytical and preanalytical challenges associated with extragenital home collected STI specimens.

UNLABELLED: Home sample collection for sexually transmitted infection (STI) screening options can improve access to sexual healthcare across communities. For Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG), genital infections have classically been the focus for remote collection options. However, infections may go undiagnosed if sampling is limited to urogenital sites because some individuals only participate in oral and/or anal intercourse. Here we evaluated samples for CT/NG detection after several pre-analytical collection challenges. A paired provider to self-collection validatio

Offering HPV self-sampling kits: an updated meta-analysis of the effectiveness of strategies to increase participation in cervical cancer screening.

BACKGROUND: Human papillomavirus (HPV) testing on self-samples represents a great opportunity to increase cervical cancer screening uptake among under-screened women. METHODS: A systematic review and meta-analysis on randomised controlled trials (RCTs) were performed to update the evidence on the efficacy of strategies for offering self-sampling kits for HPV testing compared to conventional invitations and to compare different self-sampling invitation scenarios. Four experimental invitational scenarios were considered. Women in the control group were invited for screening according to existin

Measured and self-reported olfactory function in voluntary Norwegian adults.

PURPOSE: The lack of epidemiological data on the proportion of olfactory dysfunction (OD) using comprehensive olfactory assessment in healthy adults in Scandinavia motivated to the present study which aimed to explore the proportion of OD in voluntary healthy Norwegian adults, assessed by Sniffin' Sticks, and its correlation to self-reported olfactory function. Furthermore, sociodemographic and clinical factors associated with olfactory function were analysed. METHODS: The sample included 405 Norwegian participants, aged 18-78 years, 273 women and 132 men, who underwent olfactory testing with

COVID-19 testing and reporting behaviours in England across different sociodemographic groups: a population-based study using testing data and data from community prevalence surveillance surveys.

BACKGROUND: Understanding underlying mechanisms of heterogeneity in test-seeking and reporting behaviour during an infectious disease outbreak can help to protect vulnerable populations and guide equity-driven interventions. The COVID-19 pandemic probably exerted different stresses on individuals in different sociodemographic groups and ensuring fair access to and usage of COVID-19 tests was a crucial element of England's testing programme. We aimed to investigate the relationship between sociodemographic factors and COVID-19 testing behaviours in England during the COVID-19 pandemic. METHODS

Detection of Severe Acute Respiratory Syndrome Coronavirus 2 on Self-Collected Saliva or Anterior Nasal Specimens Compared With Healthcare Personnel-Collected Nasopharyngeal Specimens.

BACKGROUND: Nasopharyngeal specimens (NPS) are commonly used for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) testing but can be uncomfortable for patients. Self-collected saliva specimens (SS) or anterior nasal specimens (ANS) for SARS-CoV-2 detection are less invasive, but the sensitivity of these specimen types has not been thoroughly evaluated. METHODS: During September-November 2020, 730 adults undergoing SARS-CoV-2 testing at community testing events and homeless shelters in Denver provided self-collected SS and ANS before NPS collection and answered a short survey about

Comparison of the effects of abstinence on MDMA and cocaine self-administration in rats.

RATIONALE: 3,4-Methylenedioxymethamphetamine (MDMA) preferentially increases synaptic serotonin (5HT). This response was attenuated following repeated exposure but there was recovery as a result of abstinence. Effects of abstinence on self-administration of many drugs have been documented but the impact on MDMA self-administration is unknown. OBJECTIVE: This study compared the effects of abstinence on MDMA and cocaine self-administration. METHODS: Six-hour daily MDMA or cocaine sessions were conducted until a total of 350 mg/kg had been self-administered. Following this, rats were randomly a

Can online peer influencers accelerate HIV testing among young adult males living in Nairobi, Kenya? A study protocol.

BACKGROUND: Kenya continues to experience persistent gender disparities in HIV testing. Young men remain substantially less likely than young women to test for HIV, despite expansion of facility based, community based, and HIV self testing services. Although WhatsApp based and peer led interventions have shown promise individually, it remains unclear whether adding online peer influencer engagement to standard HIV communication further increases HIV testing uptake. OBJECTIVE: To determine whether adding an online peer influencer led WhatsApp intervention to standard FM radio and WhatsApp HIV

Self-monitoring of cardiac autonomic function at home is feasible.

BACKGROUND: Cardiovascular autonomic neuropathy (CAN) is associated with diabetes and may be related to the development of hypertension, ischemic stroke, and a number of other late complications. The earliest sign of CAN is a reduction of heart rate variability (HRV). Standard HRV tests for CAN include expiration-to-inspiration ratio, response to active standing (30:15), and the Valsalva maneuver. Because of the technical requirements for these tests, they are limited to the point-of-care office or a clinical laboratory setting. It is unknown if a "white-coat" phenomenon exists in autonomic ne

How often should self-monitoring of blood pressure be repeated? A secondary analysis of data from two randomized controlled trials.

BACKGROUND: Little evidence exists around the optimal frequency of self-monitoring of blood pressure (BP). Testing too frequently can lead to erroneous management changes due to random "noise" leading to raised measurements by chance. This study used recent trial data to evaluate self-monitored BP over time, aiming to determine how frequently patients should self-monitor. METHODS: Data from patients with home BP ≤135/85 mmHg on stable medication in the self-monitoring groups of two trials were analysed using a mixed effects model. The primary outcome was mean change in BP per month. Secondary

A Situation-Specific Theory of Self-Care Behaviors in People Living With an Intestinal or Urinary Ostomy.

The literature on self-care in people living with an ostomy pouch is expanding rapidly, yet a guiding theoretical framework remains lacking. This article aims to present the development of a situation-specific theory of self-care in people living with an ostomy pouch, encompassing both intestinal (colostomy and ileostomy) and urinary (urostomy) diversions. We describe the process of ostomy self-care, the key predictors of self-care behaviors, and their outcomes. Underlying assumptions and theoretical propositions are also outlined. Self-care includes actions directed toward maintaining the phy

Is there a difference in sample adequacy when vaginal HPV DNA samples are self-collected at home, a health post, or a primary care health center in rural Ethiopia? Implications for community cervical cancer screening.

BACKGROUND: Self-collection of HPV DNA samples is recommended as a cervical cancer screening method in areas with high barriers to clinical examination, such as Ethiopia. Self-collected sample adequacy in clinical settings is high compared to clinician collection, but less is known about self-collected sample adequacy in community settings. We evaluated sample adequacy differences when samples were taken at women’s homes, a local health post, or a primary care health center in rural Ethiopia. METHODS: Women either self-collected vaginal samples for HPV DNA at home (Arm 1, n = 100), at a health

Breaking the barrier: Transforming the diagnosis and management of penicillin and cephalosporin allergy in China.

OBJECTIVES: β-lactam allergy labels are common in routine care but are variably verified. This manuscript assessed prevalence, clinical features, verification practices, and penicillin-cephalosporin cross-occurrence across multiple hospitals in China. METHODS: We used multicenter cross-sectional questionnaires in outpatient clinics at 22 institutions (n = 6070). Adjudication relied on routine records and prior testing performed in care. "Clinically-confirmed" allergy required a chart-documented reaction and/or a previously positive skin test; indeterminate entries were coded "uncertain." Cros
