OBJECTIVE: To examine the role of fetal size in neonatal clavicular fracture.
STUDY DESIGN: A retrospective, case-control study was performed on infants diagnosed with neonatal clavicular fractures and born vaginally between July 1997 and June 2000. For each index case, a control matched for gestational age (within one week) and birth weight (within 250 g) and delivered within the same 24-hour period was selected at random from the delivery suite registry. The maternal and intrapartum factors were compared and the results analyzed according to whether the infants were large for gestational ag

Accumulation of clavicular fractures among newborns on the authors' ward is reported, with reference to 1978. Twenty clavicular fractures were recorded from 1.6 per cent of all newborns, in the above period under review. They were related to cephalic presentations and to the upper shoulder in 90 percent. They were closely correlated to high birth weight. Positioning of the women concerned and medical supervision of delivery seemed to be of causative importance.

BACKGROUND An ipsilateral fracture of the midshaft clavicle with dislocation of the acromioclavicular joint (ACJ) is a rare combination injury and almost always occurs following high-energy trauma. Currently, there is no optimal surgical approach for this kind of rare injury. We present a 60-year-old man with a traumatic combined linear midshaft clavicle fracture and ACJ injury simultaneously treated with Knowles pin fixation. CASE REPORT A 60-year-old male patient presented a linear midshaft clavicle fracture in the emergency room (ER) due to a road traffic accident. A linear fracture progres

BACKGROUND: As achieving stable fixation in unstable far-lateral distal clavicle fractures is challenging, the optimal surgical treatment remains controversial. The aim of this study was to compare the clinical and radiological results of coracoclavicular fixation (CCF) versus anatomical locking plate fixation (ALPF) in patients with Cho type IIC fractures.
METHODS: Thirty-eight patients who underwent surgical treatment for Cho type IIC fracture were included. Open CCF using a suture button device was performed in 25 patients and ALPF was performed in 13 patients. The mean follow-up period wa

BACKGROUND: This study was performed to compare the operative clinical outcomes of helical plating, intramedullary nailing (IMN), and long straight lateral plating in the treatment of humeral shaft fractures extending into the proximal humerus, as well as to identify the optimal fixation strategy for managing such injuries.
METHODS: In total, 81 patients with humeral shaft fractures extending into the proximal humerus were divided into three groups based on treatment strategy: helical plating (Group A, n = 16), IMN (Group B, n = 12), and long straight lateral plating (Group C, n = 53). Preope

PURPOSE: This study was to test the hypothesis that intramedullary (IM) nailing fixation of midshaft clavicle fractures could result in better clinical outcomes and lower complications rates than plating fixation.
METHODS: PubMed, Embase, and the Cochrane Library database were used to search all English language published randomized controlled trials (RCTs) of midshaft clavicle fractures using plating versus IM nailing. The characteristics of the study participants were collected. Outcomes of postoperative shoulder functional measurements, operative data and complications rates were meta-anal

BACKGROUND: Posterior shoulder dislocation is an uncommon disruption of the glenohumeral joint. Risk factors include seizure, electric shock, and underlying instabilities of the shoulder joint.
CASE REPORT: A 27-year-old man with a history of recurrent posterior shoulder dislocation presented to the Emergency Department with sudden shoulder pain and reduced range of motion about the shoulder after abducting and internally rotating his arm. Radiographs did not show fracture or dislocation. The treating physician suspected an occult posterior shoulder dislocation, but wanted to avoid performing

A woman and her lover were accused of murdering the woman's husband. Five weeks after fracture/dislocations of both shoulders and a central fracture/dislocation of the right hip were diagnosed the woman's husband died of septicaemia consequent on a ruptured infected hydronephrosis. The shoulder and hip injuries and the ruptured hydronephrosis were attributed to a beating with a blunt instrument. A review of the medical records of the deceased, particularly his radiological examinations, led the authors to conclude that his joint injuries and ruptured kidney were not due to the alleged assault.

Incidence and severity of shoulder lesions are increasing especially in alpine skiing accidents. In case of a shoulder dislocation there is always the question of a safe and atraumatic reduction technique if possible without analgetics or even general anesthesia. We present a new reduction technique with the help of the patient's own body weight under muscular relaxation. Even in the presence of displaced fractures of the greater tuberosity or a depression fracture of the humeral head successful reductions were obtained. After the reduction, the arm is immobilized in a Gilet. The patient is ab

Growth plate injuries (Salter-Harris type 1 or physeal fractures) of the long bones in the newborn are easily misdiagnosed as joint fractures with dislocations due to their nonossified epiphyses on plain radiographs. Diagnosis with musculoskeletal ultrasound (US) is advantageous due to its ability to visualize the nonossified epiphysis. We present two cases of humeral growth plate fractures in newborns, one at the shoulder (proximal humerus) and the other at the elbow (distal humerus). These cases emphasize the importance of quick and noninvasive diagnosis with US to avoid unnecessary interven

Most humeral shaft fractures are amenable to nonoperative treatment. According to shoulder and elbow functions, humeral shaft malunions are well tolerated with deformities up to 30 degrees of varus, 20 degrees of anterior bowing and 15 degrees of internal rotation. Limitations to nonoperative treatment do exist. Open fractures with extensive soft-tissue lesions, penetrating open fractures with neurological or vascular impairment are best managed with immediate stabilization. However the appropriate treatment strategy has to be adapted for each patient. Patient expectations, fracture propensity

Complications of operative treatment of closed fractures of the shaft of the humerus occur in about 12 per cent of cases. Fixation of the arm in some form of cast gives better results but the complication rate is still 5.5 per cent and there is a risk of shoulder-hand-finger syndrome. In order to minimize these complications the author treated patients with humeral shaft fractures by supporting the hand only, in a simple sling. The method is described and its advantages discussed. All the fractures, including transverse ones, united in 3 to 6 weeks without any serious complications.

OBJECTIVE: We wanted to investigate and compare the reliability of two measuring methods for glenoid bone loss, one linear- and one surface area method, in acute glenoid fractures after dislocation. This was an experimental study.
METHOD: Thirty CT-shoulder scans in patients with an acute glenoid rim fracture were measured in a lateral"en face" view in 2D and 3D by two independent assessors. In 10 of 30 patients contralateral shoulder CT-scan was obtained for reference circle placement. Intra- and interrater reliability was analysed by Intra Class Correlation. Monteggia fractures are uncommon ulnar fractures with associated radial head dislocation, which are classified into four Bado types. Type IV lesions, involving both forearm fractures with anterior radial head dislocation, are particularly rare. This case describes an exceptionally rare variant in a 23-year-old male patient following a motor vehicle accident: a Bado Type IV Monteggia fracture with posterior radial head dislocation, accompanied by an ipsilateral Ideberg type III glenoid fracture, which was initially missed. Despite prior open reduction and internal fixation (ORIF) of the forearm

INTRODUCTION: Pedicle screws are durable and safe instruments frequently used to treat vertebra injuries and deformities. There is also a possibility of medulla spinalis and visceral organ injury when the pedicle screws are delivered in unsuitable positions or dimensions. In this case, the authors want to draw attention to one of the visceral organ injuries during the thoracic pedicle screw placement.
PRESENTATION OF CASE: A 31 years old man underwent posterior instrumentation and fusion for T4 vertebra fractures. The patient was not symptomatic in the postoperative period. Tracheal pressure

To assess whether pulsed electromagnetic field therapy during cast immobilization of distal radius fractures has beneficial effects on pain and limb function, the study included 52 patients (mean age 60.8 ± 15.0 years) with distal radius fractures treated with cast immobilization. Patients were allocated to a pulsed electromagnetic field group (n = 27) or a control group (n = 25). Pain; forearm and arm circumference; range of motion; disabilities of the arm, shoulder, and hand score; and touch sensation were evaluated on the day of the plaster cast dressing and 3 and 6 weeks after.
