OBJECTIVE: To treat chronic hypotony with decreased vision after filtration surgery.
DESIGN: After observation from 3 1/2 to 13 months with no spontaneous improvement, eyes were treated with the experimental therapy and followed up closely to determine the results.
PATIENTS: Four eyes with chronic hypotony, selected from a series of 125 eyes receiving trabeculectomy augmented by intraoperative mitomycin. All eyes had posterior chamber pseudophakia. All eyes had postoperative laser cutting of sutures holding the scleral flap of the trabeculectomy.

We describe 3 cases of unilateral congenital ectropion uveae associated with ptosis and glaucoma in a syndrome with no systemic associations. Early detection of glaucoma and timely surgical intervention is required to preserve productive vision in such patients. In our case series, trabeculectomy with mitomycin C was effective in controlling glaucoma progression over a follow-up period of 5-15 years.

Here we report a case of childhood glaucoma refractory to angle and trabeculectomy surgery. The patient was treated with an Ahmed™ drainage implant that was subsequently complicated by rapid-onset panophthalmitis and orbital cellulitis. Intravenous and intravitreal antibiotic therapy was initiated and the drainage tube was removed. The infectious process resolved within 3 weeks; however, phthisis bulbi developed subsequently.

We report 3 cases of ghost cell glaucoma all of which occurred in phakic patients with onset between 18 months and 4 years after vitreous haemorrhage. There appears to have been spontaneous disruption of the anterior hyaloid face in all 3 cases. Control of the glaucoma was achieved by medical treatment in one case and by trabeculectomy in two.

A patient with refractory glaucoma 1 year after cataract extraction and trabeculectomy had Molteno implant surgery. Three days after surgery a kissing choroidal effusion and retinal detachment adherent to the posterior chamber IOL were detected. Repeated choroidal taps were unsuccessful. Removal of the Molteno implant, vitrectomy, and silicone oil injection were required to reattach the retina.

Pseudophakic patients that require surgical management of glaucoma have many of the same options as phakic patients. Considerations when performing incisional glaucoma surgery (trabeculectomy with adjunctive antimetabolites and glaucoma drainage device implantation) and laser glaucoma surgery (laser trabeculoplasty and laser cyclophotocoagulation) in pseudophakic patients are reviewed here. The decision to operate and the choice of surgical procedure must be individualized for each patient.

CDI is showing continued promise as a reliable, reproducible method to assess the hemodynamics of a variety of ophthalmic diseases in which vascular perfusion abnormalities are suspect. Moreover, some patients with advanced POAG appears to have abnormalities of the vasculature of the retrobulbar circulation that is corrected by trabeculectomy. Continued CDI clinical correlations with longitudinal studies are now required to elucidate the significance of this technology and these results for individual patients.

A case of cutis marmorata telangiectatica congenita (CMTC) is described in association with congenital glaucoma. Approximately 68 cases of CMTC and only four cases associated with glaucoma have been reported. The type of glaucoma seems to be related to an abnormality of the filtration in the anterior angle, due to increased pressure in the episcleral vessels. The surgical treatment is trabeculectomy, and opening an alternative drainage pathway.

PURPOSE: To evaluate the criteria of diagnosis and management of secondary glaucoma in young nanophthalmic patients.
METHODS: Considering the anatomic features and clinical characteristics, 22 eyes of 11 patients were identified as bilateral nanophthalmos. Intraocular pressures (IOP) were checked with Goldmann applanation or Keeler pulse-air 2000 tonometer, and Nd-Yag laser was used for iridotomies.
