Persistent left superior vena cava (PLSVC) is the most common congenital thoracic venous anomaly and is usually clinically silent. We report a critically ill patient in whom PLSVC was identified after left internal jugular central venous catheter placement. Chest radiography showed a vertically oriented catheter along the left mediastinum without expected midline crossover, raising concern for malposition. BACKGROUND: Bifid rib is a congenital abnormality occurring in about 0.6% of the healthy population.
CASE DESCRIPTION: A 5-year-old boy attended the Orthopaedics outpatient clinic with a swelling on the right of his sternum that was increasing in size. A chest x-ray did not reveal any abnormalities. However, on the CT scan that was then performed we saw a bifurcation of the fourth rib, for which we adopted a watchful waiting treatment strategy. With hindsight, the bifurcation was also visible on the x-ray image but we had not initially recognised it as such.

Peritoneal tuberculosis (TB) is still a major health problem in developing countries. We describe a 26-year-old woman with peritoneal TB presenting with lower abdominal pain and distention, weight loss, and night sweats. There are no pathognomonic clinical, laboratory, or radiologic findings for peritoneal TB. Therefore, it can be easily confused with peritoneal carcinomatosis and advanced ovarian carcinoma. Surgical repair of craniosynostosis carries a high risk with large blood losses. Over a 2-yr period, we have managed 115 patients undergoing craniosynostosis repair with peroperative haemodilution to achieve a final PCV of 0.28-0.35.
