Computational electrophysiology of the coronary sinus branches based on electro-anatomical mapping for the prediction of the latest activated region.

This work dealt with the assessment of a computational tool to estimate the electrical activation in the left ventricle focusing on the latest electrically activated segment (LEAS) in patients with left bundle branch block and possible myocardial fibrosis. We considered the Eikonal-diffusion equation and to recover the electrical activation maps in the myocardium. The model was calibrated by using activation times acquired in the coronary sinus (CS) branches or in the CS solely with an electroanatomic mapping system (EAMS) during cardiac resynchronization therapy (CRT). We applied our computat…

Technical considerations for CRT-D implantation in different varieties of persistent left superior vena cava.

PURPOSE: The persistent left superior vena cava (PLSVC) is usually asymptomatic and creates a challenge when detected incidentally during cardiac resynchronization therapy defibrillator (CRT-D) implantation. The purpose of our cases is to show different anatomical variables of PLSVC and different strategies used for CRT-D implantation. METHODS: Four cases of PLSVC were presented. Pre-procedural bilateral venography was done to define anatomical variant of PLSVC. The side of approach and vein of approach were chosen according to the anatomical variant. Major challenges, electrical parameters, …

The effects of right ventricular apical pacing on ventricular function and dyssynchrony implications for therapy.

Cardiac pacing is the only effective treatment for patients with sick sinus syndrome and atrioventricular conduction disorders. In cardiac p … is directly related to acutely induced left ventricular dyssynchrony. The upgrade from RV pacing to cardiac resynchronization therapy may partially reverse the deleterious effects of RV pacing. It has even been suggested that selected patients with a conventional pacemaker indication should receive cardiac resynchronization therapy to avoid the deleterious effects. This review wil …

His Bundle Pacing for Cardiac Resynchronization.

Cardiac resynchronization therapy with left ventricular coronary sinus or epicardial pacing is an established therapy for patients with systolic heart failure and intraventricular conduction system delay, particularly left bundle branch block. Despite large-scale randomized trials demonstrating the efficacy of this therapy, clinical and echocardiographic nonresponse rates remain as high as up to 40%. His bundle pacing (HBP) has reemerged as a means to provide physiologic resynchronization in patients with bundle branch block by correcting QRS by direct capture of the His-Purkinje system. This …

Common questions about pacemakers.

Pacemakers are indicated in patients with certain symptomatic bradyarrhythmias caused by sinus node dysfunction, and in those with frequent, … neurocardiogenic syncope may benefit from pacemakers, but most patients with this disorder do not. Cardiac resynchronization therapy improves mortality rates and some other disease-specific measures in patients who have a QRS duration of 150 milliseconds or greater and New York Heart Association class III or IV heart failure. Patients with class II heart failure and a QRS of 150 …

Biventricular pacing as bridge to rapid recovery in infancy.

The authors present the unique case of an 8-month-old baby diagnosed with severe left ventricular failure of unknown etiology. Due to a lack of organ availability for this age, a mechanical assist device and assessment for cardiac transplantation were not offered. Subsequent comprehensive echocardiographic dyssynchrony assessment and the presence of left bundle branch block were suggestive of response to cardiac resynchronization therapy. Dual-chamber epicardial pacing was initiated, resulting in prompt marked clinical and echocardiographic improvement, which continued until complete normaliza…

Resynchronization with left ventricle lead placement through the foramen ovale.

Left ventricle (LV) lead placement in a coronary sinus branch for cardiac resynchronization therapy may fail because of anatomical variants, phrenic nerve stimulation, and/or lead instability. We report a case of successful resynchronization from a lead inserted from the left subclavian vein and positioned through a patent foramen ovale (PFO). In conclusion, endocardial LV lead insertion through a PFO enables effective resynchronization delivery without the risks associated with a thoracotomy or atrial transseptal puncture.

CineECG Repolarization Gradients Predict Acute Hemodynamic Response in CRT Patients.

INTRODUCTION: A variable proportion of non-responders to cardiac resynchronization therapy (CRT) warrants the search for new approaches to optimize the position of the left ventricular (LV) lead and the CRT device programming. CineECG is a novel ECG modality proposed for the spatial visualization and quantification of myocardial depolarization and repolarization sequences. OBJECTIVE: The present study aimed to evaluate CineECG-derived parameters in different pacing modes and to test their associations with acute hemodynamic responses in CRT patients. METHODS AND RESULTS: CineECG was used to …

Effect of pulmonary artery pressure-guided therapy on heart failure readmission in a nationally representative cohort.

AIMS: Pulmonary artery pressure (PAP)-guided therapy in patients with heart failure (HF) using the CardioMEMS (CMM) device, an implantable PAP sensor, has been shown to reduce HF hospitalizations in previous studies. We sought to evaluate the clinical benefit of the CMM device in regard to 30, 90, and 180 day readmission rates in real-world usage. METHODS AND RESULTS: We queried the Nationwide Readmissions Database (NRD) to identify patients who underwent CMM implantation (International Classification of Diseases 9 and 10 codes) between the years 2014 and 2019 and studied their HF readmission

Diagnosis of slow ventricular tachycardia by cardiac resynchronization devices utilizing a desynchronization detection algorithm.

Some cardiac resynchronization therapy (CRT) devices equipped with left ventricular (LV) sensing can develop a specific desynchronization rhythm. Contemporary BIOTRONIK devices are designed with an algorithm called "CRT pacing interrupt" exclusively designed to record the occurrence of the specific form of desynchronization. We report six patients in whom the CRT pacing interrupt function permitted the diagnosis of slow ventricular tachycardia (VT). Slow VT was defined as slower than the programmed VT intervention rates. Although the CRT pacing interrupt function is not designed to detect slow…

[Placement of a biventricular resynchronization device in a patient with a persistent left superior vena cava: report of one case].

Persistent left superior vena cava is the most common venous congenital malformation and is usually asymptomatic. Its presence could increase the difficulty for transvenous lead implantation. We report a 71-year-old woman with an idiopathic dilated cardiomyopathy, atrial fibrillation and heart failure that required biventricular resynchronization therapy. During the placement of the device a persistent left superior vena cava was detected. The device was placed without problems and the patient had a satisfactory postoperative evolution.

Successful percutaneous cardiac resynchronization despite an occlusive Thebesian valve.

We report the case of a patient with symptomatic heart failure referred after an unsuccessful attempt at cardiac resynchronization therapy. An occlusive Thebesian valve prevented entry into the coronary sinus ostium. Careful analysis of the patient's cardiovascular physiology and anatomy revealed the "fortuitous" presence of a persistent left superior vena cava. Cannulation of this vessel permitted percutaneous retrograde placement of a left ventricular lead into a posterolateral cardiac venous branch resulting in successful cardiac resynchronization. This unique case provides strong evidence …

Effects of CRT on myocardial innervation, perfusion and metabolism.

Heart failure leads to specific changes in cardiac perfusion, metabolism, and innervation. Typically, in the early phase of heart failure, l … ads to striking, mainly regional imbalance in these parameters. Recent studies have documented that cardiac resynchronization therapy (CRT) has prominent effects on myocardial perfusion, metabolism, and innervation. Cardiac resynchronization therapy normalizes many of these parameters and these changes can be considered to be the signs of successful resynchronization. In contrast, …
