Aplasia of the vena cava inferior as a cause for recurring thrombosis of the lower extremities and pelvic veins.

Congenital anomalies of the caval vein are often associated with other abnormalities such as heart defects, situs inversus or a polysplenia-asplenia-syndrome. An isolated, congenital malformation like aplasia of the inferior vena cava is a rare finding. A review of the embryology and abnormalities, diagnostics, clinical signs and treatment is given together with the histories of two patients having thrombosis of the lower extremities and pelvic veins, caused by aplasia of the inferior vena cava. After thrombotic complications caused by vena cava aplasia there is high risk of recurrence. Those …

Ectopic cervical thymic masses in infants: a case report and review of the literature.

Ectopic cervical thymic tissue is a rare cause of neck masses in infants with only nine cases reported in the literature. This entity should be considered in the differential diagnosis of infantile neck masses to prevent inadvertent total thymectomy and its possible deleterious effects on the developing immune system. The case report of a 2-month-old infant with ectopic thymic tissue presenting as a solid cervical mass is reviewed as is the literature of such lesions. The embryology of the thymus, the effect of early total thymectomy in laboratory animals, and the management of neck masses in …

Vesico-ureteral reflux: a genetic condition?

Vesico-ureteral reflux (VUR) is a frequent condition, but in most instances, the precise cause is unknown. We here review the evidence of a genetic aetiology of VUR, inherited as an autosomal dominant trait, with variable expression. We discuss the possible pathogenetic relationship between VUR and other types of uropathies and possible strategies towards the identification of genes underlying VUR are presented. The isolation of the gene(s) responsible for uropathies will not only lead to a better insight into the embryology of the urological system, the pathogenesis of uropathies, but also to…

Congenital anomalies of left renal vein and its clinical importance: a case report and review of literature.

Detailed knowledge of the anatomy and anomalies of renal veins is necessary for retroperitoneal surgery and venographic procedures. According to Thomas (1970, Arch. Surg. 100: 738-740), the anomalies of renal veins are more frequent than estimated. The number of surgical procedures and radiologic examinations related to the retroperitoneum are increasing, and therefore pathologic conditions of the retroperitoneal area have been discussed more frequently. We report on a retroaortic left renal vein joining the left common iliac vein, discuss the embryology and clinical importance of renal vein a…

Imaging of the parathyroid glands.

Primary hyperparathyroidism resulting in hypercalcemia is the most common presentation of parathyroid pathology and usually is related to a parathyroid adenoma and, less commonly, to hyperplasia. Imaging of the parathyroid glands focuses on the detection of adenomas in patients with primary hyperparathyroidism. The role of imaging (as well as the modality used) for preoperative localization of the parathyroid glands continues to be controversial. The embryology, anatomy, and physiology of the parathyroid glands are reviewed. The diagnostic utility of radiological imaging-including ultrasonogra…

Duplication of the midface.

Facial duplication is one of the rarest forms of craniofacial anomalies. Only a few cases have been reported and described in the literature. We describe here two cases of midfacial duplication, one in a male and the other in a female. Both were examined clinically and radiologically including computed tomographic scanning, with three-dimensional computer reconstruction in the second patient. A histopathological examination was also carried out in both patients for the removed abnormal masses. The embryology of the face, together with the morphopathogenesis and the surgical management and outc…

First brachial cleft sinuses: an analysis of current management strategies and treatment outcomes.

First branchial cleft sinuses account for less than 1% of all branchial cleft anomalies. A thorough understanding of the embryology and developmental anatomy is essential for successful management. Nine cases of first branchial cleft sinuses treated at The Hospital for Sick Children by the Department of Otolaryngology between 1984 and 1990 were reviewed. A large proportion of these lesions were initially misdiagnosed despite significant symptomatology. This resulted in a high rate of infectious complications. Early diagnosis, prompt control of infection, and early surgical excision are recomme…

Congenital anomaly of the second cervical vertebra predisposing to progressive cervical myelopathy. A case report.

The authors describe a previously unreported constellation of developmental anomalies of the C2 vertebra, which predisposed to the development of cervical myelopathy in a young patient. The embryology of the vertebral column is reviewed briefly, and the origin of each anomaly of the C2 vertebra is traced to one of the two early stages in development as follows: 1) the formation of the mesenchymal vertebra or 2) its induction to cartilage. This case emphasizes the importance of congenital stenosis in the development of cervical myelopathy in young patients. The outcome is favorable when it is t…

[Syringoceles of Cowper's glands in children].

Some tubular or cystic diverticula observed in the ventral wall of the male bulbous urethra that originate from dilatation of Cowper's gland ducts have been termed "syringoceles". They can cause dysuria, hematuria and obstruction. We report 14 syringoceles and 1 true diverticulum from segmental absence of spongious urethra. We review embryology, anatomy and diagnosis in order to determine the best therapeutic approach. We performed endoscopic resection of the distal valve-like border of the diverticulum in 6 patients (42.85%) with good results. The remaining 8 patients, in whom surgery was not…

The small midline occipital encephalomeningocele: definition of a syndrome.

An unusual group of encephalomeningoceles is described. Two children were noted at birth to have a raised area of abnormal skin at the occipital midline. Both of these lesions were found to perforate the superior sagittal sinus and to end between the leaves of the falx cerebri, and to contain a small amount of abnormal cerebral tissue with no connection to either occipital lobe. Previous case reports are reviewed; these lesions are true encephalomeningoceles, rather than glial heterotopias. The embryology of such lesions is considered, and likely differs from the much more common occipital enc…

[Anomalies of the first branchial cleft].

These are cysts and fistulae which closely adherent to the external auditory canal and drain through the high lateral cervical suprahyoid site. Misdiagnosis is common with these lesions and inadequate removal leads to surgical failure. A review of the embryology of this region is necessary since we intend to explain the clinical aspects and the variable relationship to the facial nerve. Eight cases are described and the literature pertaining to first branchial cleft syndrome is reviewed. Diagnostic and management problems will be discussed. A full exposure of the parotid gland and facial nerve…

The vascular pathogenesis of gastroschisis: intrauterine interruption of the omphalomesenteric artery.

Recognition of the disruptive vascular nature of the structural defects associated with gastroschisis and an appreciation of the embryology of the umbilical region suggest that gastroschisis results from an intrauterine interruption of the omphalomesenteric artery. This mechanism accounts for the usual location of gastroschisis to the right of the umbilical cord, the integrity of the rectus muscles in affected children, and many of the clinically observed differences between gastroschisis and omphalocele. The vascular basis of this defect explains its negligible recurrence risk and should aler…

Congenital miosis.

The inheritance of congenital miosis in 2 pedigrees is described. The inheritance was autosomal dominant in one and autosomal recessive in the other. The pupils were 0.5-2.5 mm diameter, reacted normally to light and accommodation but dilated poorly with mydriatics. In both pedigrees, the affected members had enlarged corneas. The five members in the autosomal dominant family were myopic and had translucent peripheral irides. Iridodonesis was presented in both members of the autosomal recessive pedigree. The embryology and innervation of the iris muscles together with the ocular abnormalities …

[Current considerations on male secretory sterility].

The problems of male sterility are considered in the light of fundamentals underlying the embryology and histophysiology of spermatogenesis and its hormonal control. Some forms of infertility are now understood but no treatment for them has yet been found. The mechanisms of other forms still elude firm definition: this is the case of, among others, some oligoasthenospermias in which, according to our experience, excessive estrogen production appears to play a physiopathologic role. In the prevailing uncertainty about therapeutic methods, the best short-term solutions in this field are in-vitro…

[Cysts and fistulas of the first branchial cleft with parotid symptomatology].

On the basis of two cases, one a fistula and the other a cyst of the first brachial cleft with chiefly parotid manifestations, a review of the embryology is given which explains the anatomical features. The elements in their clinical diagnosis, complemented by fistulography and sialography are compared with operative findings, the essential steps in which are as follows: 1) Identification of the facial nerve which by necessity overlies the lesions. 2) Partial resection of the superficial lobe of the parotid. 3) The essential resection of the external auditory meatus, the keystone of radical cu…

Anatomy of the Middle Meningeal Artery: Implications for Middle Meningeal Artery Embolization in Chronic Subdural Hematoma.

Chronic subdural hematoma (cSDH) is among the most common neurosurgical conditions, particularly affecting older adults and patients receiving anticoagulation. While burr-hole evacuation has long been the standard therapy, recurrence rates and associated morbidity remain significant. Middle meningeal artery (MMA) embolization has recently emerged as a promising alternative or adjunctive treatment, offering durable hematoma control with reduced recurrence. To understand the rationale and safety of this endovascular approach, detailed knowledge of MMA anatomy, embryology, and variations is essen…
