Intraocular pressure changes in eyes with small incision lenticules and laser in situ keratomileusis.

BACKGROUND: Measuring intraocular pressure after refractive surgery is a challenge in the diagnosis, evaluation, and treatment of glaucoma. Intraocular pressure characteristics after laser in situ keratomileusis (LASIK) are well known. However, intraocular pressure measurement and characteristics after small incision lenticule extraction (SMILE) are still unknown, providing an interesting point of comparison in terms of biomechanical differences from LASIK. METHODS: An intraocular pressure analytical model utilising fluid dynamics (simulating air puff) was developed using OpenFoam and Scilab.

Management of long-standing partially torn and flipped laser in situ keratomileusis flaps.

UNLABELLED: We describe 2 cases of traumatized and torn laser in situ keratomileusis (LASIK) flaps, partially flipped anteriorly or posteriorly, fixed for 8 months or 4 months, and accompanied by epithelial ingrowth. The 2 patients had had uneventful bilateral LASIK 6 years and 1 year before the trauma. In Case 1, the anteriorly flipped flap was removed with transepithelial phototherapeutic keratectomy. Next, mitomycin-C 0.04% was applied for 30 seconds. In Case 2, the portion of the flap that was flipped posteriorly and buried under the remaining intact LASIK flap was restored to its original

Intraoperative flap re-cut after vertical gas breakthrough during femtosecond laser keratectomy.

We describe the management of intraoperative vertical gas breakthrough (VGB) during femtosecond laser flap creation in 3 patients. All eyes were immediately re-cut using 2 different microkeratomes, and the laser in situ keratomileusis treatments were completed on the same day. There were no postoperative complications. Corneal abrasion might predispose to VGB. Management of VGB is effective using microkeratomes. Caution is advised during placement of the suction ring and in eyes with preexisting corneal abrasion or loose epithelium if femtosecond laser keratectomy is used. The approach in term

High-resolution optical coherence tomography visualization of LASIK flap displacement.

Uneventful myopic laser in situ keratomileusis (LASIK) was performed in both eyes of a 33-year-old woman. Two weeks after LASIK, examination of the left eye revealed flap striae radiating inferonasally from the superior hinge. The flap was relifted and repositioned to remove the irregular astigmatism and reduce the striae. Before the flap was relifted, Fourier-domain optical coherence tomography (FD-OCT) showed the gap at the temporal flap, which had not been detected by biomicroscopy. After the flap was relifted, FD-OCT documented that the gap was closed. High-resolution FD-OCT was helpful in

Utilization of refractive surgery technology in keratoconus and corneal transplants.

Keratoconus is considered a contraindication for refractive surgery because of the potential corneal instability following surgery. On the other hand, penetrating keratoplasty is the usual surgical modality when the keratoconic patient cannot tolerate a contact lens. In some selected cases, when the central cornea is still transparent, refractive surgical procedures, particularly Intacs implantation, may be used to help the patient to improve visual function.
