A cost-effectiveness analysis of off-label atypical antipsychotic treatment in children and adolescents with ADHD who have failed stimulant therapy.

The objectives of this study are: (1) to estimate the expected health outcomes of atypical antipsychotics (AAPs) and other non-stimulant attention-deficit/hyperactivity disorder (ADHD) medications and (2) to evaluate the cost-effectiveness of AAPs compared to other non-stimulant ADHD medications. We used decision analysis to compare three alternatives for treating children and adolescents with ADHD who failed initial stimulant treatment: (1) AAPs, (2) a selective norepinephrine reuptake inhibitor (atomoxetine), and (3) selective α2-adrenergic agonists (clonidine and guanfacine). Probability es

Evaluation of the Visual System by the Primary Care Provider Following Concussion.

Concussion is a common injury in childhood and has the potential for substantial impact on quality of life. Visual issues have been increasingly recognized as a common problem after concussion. Many children initially seek care for concussion with their pediatrician, making it even more important for pediatricians to recognize, evaluate, and refer children with visual issues after concussion. This clinical report is intended to support the recommendations in the companion policy statement on vision and concussion and provides definitions of some of the physiologic aspects of the visual system

Advances in the applications of polymer biomaterials for in vitro follicle culture.

The ovarian reserve (OR) indicates ovarian function by representing the quantity and quality of ovarian follicles, and it gradually decreases with increasing age. With the prolongation of women's lives, the protection provided by estrogen is lost for decades in postmenopausal women, and the related cardiovascular and cerebrovascular diseases, osteoporosis, and decreased immunity are the main risk factors affecting women's quality of life and longevity. Pharmacologic hormone replacement therapy (PHRT) has been controversial, and the construction of artificial ovary (AO) has attracted increasing

Neurobiological consequences of maternal cannabis on human fetal development and its neuropsychiatric outcome.

Despite the high prevalence of marijuana use among pregnant women and adolescents, the impact of cannabis on the developing brain is still not well understood. However, growing evidence supports that the endocannabinoid system plays a major role in CNS patterning in structures relevant for mood, cognition, and reward, such as the mesocorticolimbic system. It is thus clear that exposure to cannabis during early ontogeny is not benign and potential compensatory mechanisms that might be expected to occur during neurodevelopment appear insufficient to eliminate vulnerability to neuropsychiatric di

Complementary and alternative medicine for IBS in adults: mind-body interventions.

Standard treatment for IBS focuses on the management or alleviation of the predominant gastrointestinal presenting symptoms, such as diarrhea or constipation, often using pharmacological therapy. For many patients, this approach is unsatisfactory, and patients frequently seek the advice of complementary and alternative medicine (CAM) practitioners in order to explore other treatment options. CAM practices include a broad range of modalities, and mind-body interventions hold particular promise as treatment modalities for IBS because psychological factors could have an important role in IBS symp

Human intestinal B cells in inflammatory diseases.

The intestinal lumen contains an abundance of bacteria, viruses and fungi alongside ingested material that shape the chronically active intestinal immune system from early life to maintain the integrity of the gut epithelial barrier. In health, the response is intricately balanced to provide active protection against pathogen invasion whilst tolerating food and avoiding inflammation. B cells are central to achieving this protection. Their activation and maturation generates the body's largest plasma cell population that secretes IgA, and the niches they provide support systemic immune cell spe

Needs and Attitudes of Older Chronic Back Pain Patients towards a Wearable for Ultrasound Biofeedback during Stabilization Exercises: A Qualitative Analysis.

Chronic back pain has a high prevalence, especially in older adults, and seriously affects sufferers' quality of life. Segmental stabilization exercise (SSE) is often used during physiotherapy to enhance core stability. The execution of SSE requires the selective contraction of deep abdominal and back muscles. Motor learning can be supported using ultrasound imaging as visual biofeedback. ULTRAWEAR is a mobile ultrasound system that provides deep learning-based biofeedback on SSE execution, which is currently under development. We interviewed 15 older chronic back pain patients (CBPPs) to inve

The role of a thymus-pineal axis in an immune mechanism of aging.

The Immune Theory of Aging cannot explain the cause of immune decline. It is hypothesized that the pineal gland acting in utero and during neonatal life in altricial mammals serves as a component of the immune system. Evidence in support of the presence of a thymus-pineal axis is presented. It is postulated that the pineal gland carries a considerable burden of immunological defense during maturation of the thymus, and also acts in the programming of the immune system. By relating thymus and immune function to the pineal and its known role as a neuroendocrine transducer for the entrainment and

Emergency department point-of-care ultrasound in out-of-hospital and in-ED cardiac arrest.

BACKGROUND: Point-of-care ultrasound has been suggested to improve outcomes from advanced cardiac life support (ACLS), but no large studies have explored how it should be incorporated into ACLS. Our aim was to determine whether cardiac activity on ultrasound during ACLS is associated with improved survival. METHODS: We conducted a non-randomized, prospective, protocol-driven observational study at 20 hospitals across United States and Canada. Patients presenting with out-of-hospital arrest or in-ED arrest with pulseless electrical activity or asystole were included. An ultrasound was performe

Provision of palliative and end-of-life care by community health nurses: A scoping review on the challenges and potential solutions.

BACKGROUND: The demand for home-based palliative care is increasing, driven by ageing populations and a rise in multiple long-term conditions. Community health nurses are pivotal to enabling people to be cared for and die at home, yet their contributions to palliative and end-of-life care remain under-recognised and poorly understood. Previous international reviews have focused on specific themes but in-depth synthesis of evidence from one national health system has not been undertaken. AIM: This scoping review aimed to identify, map and synthesise evidence within a single national health sys

Improving cell-free metabolism through direct integration of artificial respiratory chains.

Energy-conserving mechanisms are essential in supporting cellular life. Yet in synthetic biology, it remains a challenge to reconstruct such processes from the bottom-up and integrate them with other biological functions to create complex systems with life-like properties. Recent efforts to build higher-order cell-free metabolic networks have suffered from the fact that their central oxidation reactions are not coupled to energy conservation, causing kinetic and thermodynamic limitations. Here, we developed an artificial respiratory chain that we tailored to sustain rapid electron transfer in

Ataxia telangiectasia.

Ataxia telangiectasia (AT) is an autosomal recessive multisystem genetic disorder caused by a mutation in the ATM gene encoding for the ATM protein. AT systemic manifestations include cutaneous telangiectasias, radiosensitivity, immune deficiency with recurrent sinopulmonary infections, and a tendency to develop lymphoid malignancies. These complications are explained by the major role played by ATM in DNA repair. AT is also the second most common childhood onset neurodegenerative disorder of the cerebellum, presenting with progressive ataxia and oculomotor apraxia and often accompanied by ext

Care-Related Quality of Life of Caregivers for Patients With Low Vision and Blindness: A Cross-Sectional Study.

PURPOSE: Healthcare systems typically focus on patient care, often overlooking the substantial challenges faced by informal caregivers, particularly those caring for individuals with severe or chronic conditions. This study explores the relationships between care-related quality of life (CarerQol), self-rated caregiver burden, and sense of coherence (SOC), identifying potential predictors that influence CarerQol for caregivers of patients with low vision and blindness. METHODS: This cross-sectional, descriptive study assessed caregiving burden using the CarerQol instrument, which measures sev
