Leiomyosarcoma of the colon presenting as acute suppurative peritonitis.

A surgical case of leiomyosarcoma arising from the ascending colon, presenting as acute suppurative peritonitis, is herein described. A 70-year-old woman complaining of lower abdominal pain presented to our clinic on October 12, 1994. She was admitted with a tentative diagnosis of peritonitis. At emergency laparatomy, purulent intraabdominal fluid was present, and a fist-sized mass was seen in the ascending colon just proximal to the hepatic flexure. A right hemicolectomy was thus performed based on a diagnosis of perforating colon cancer. The histologic findings were consistent with leiomyosa…

Sclerosing peritonitis associated with luteinized thecoma of the ovary.

A unique case of bilateral luteinized thecomas of the ovary associated with sclerosing peritonitis is reported and the clinical and pathological features of this and previously reported cases are reviewed. The patient, 52 years of age, presented with abdominal distension and diarrhea. Pelvic imaging studies revealed bilateral ovarian tumors with ascites. Total abdominal hysterectomy and bilateral salpingo-oophorectomy with adhesiotomy of the small bowel were performed. Histologically, the ovarian tumor was composed of closely packed spindle to round-shaped cells, and within the spindle cell po…

[Evaluation of abdominal treatment with magnetite suspension in peritonitis].

The work discusses the results of the effect of intraoperative treatment of the abdominal cavity with a magnetite suspension on the T-cell link of the immunological reactivity of the organism of patients with diffuse forms of peritonitis. The preparation intensified lymphocyte proliferative activity. It was found that increase of the lymphocyte stimulation index in intraoperative treatment of the abdominal cavity with magnetite suspension in peritonitis was consequent upon increased production of interleukin-1 and other growth factors by macrophages.

[Sorption in the treatment of acute peritonitis (experimental study)].

The data on the effective use of the new sorbent KAY and the carbon sorbent "Volna" in the treatment of acute generalized peritonitis were compared. Endosorption with sorbent KAY was conducted on an experimental model of generalized peritonitis. It produced a favorable effect on the functional morphology of the peritoneum by leveling many morphological signs of general intoxication of the organism and in this way creating the conditions for a favorable outcome of the postoperative period. The studies showed that the use of sorbent KAY in the treatment of acute generalized peritonitis facilitat…

Prenatal sonographic diagnosis of meconium peritonitis: a case report.

A case of meconium peritonitis that was diagnosed ultrasonographically in the third trimester is presented. Fetal ascites, intra-abdominal calcification, left communicating hydrocele, and polyhydramnios were detected on antenatal ultrasonography. Specks of calcification were also demonstrated on abdominal radiography postnatally. Laparotomy confirmed the diagnosis of perforated terminal ileum with meconium peritonitis. The obstetric and neonatal implications of meconium peritonitis are discussed with literature review.

Abruptio placentae associated with perforated appendicitis and generalized peritonitis.

A primigravid woman at 35 weeks' gestation was admitted with abdominal pain, fever, and vomiting. Forceful contractions and signs of fetal distress suggested abruptio placentae. During caesarean section, seropurulent exudate and a perforated appendix were found; an appendectomy was performed. A mechanism linking appendicitis with abruptio placentae is suggested.

A rodent model of cirrhosis, ascites, and bacterial peritonitis.

We sought to develop a rodent model of spontaneous bacterial peritonitis and report here the preliminary results of carbon tetrachloride-induced cirrhosis in which ascites and bacterial peritonitis predictably develop. Of 41 rats that survived the initial carbon tetrachloride toxicity, 38 (92.7%) developed cirrhosis with ascites. Of these 38, 21 (55.3%) developed 24 episodes of ascitic fluid infection without iatrogenic colonization. No surgically treatable source of infection was identified at autopsy in any rat; therefore, the infections were presumed to be "spontaneous." Eight (50%) of the …

[Nasobiliary drainage for spontaneous bile peritonitis due to cholangiocarcinoma].

We report a case of cholangiocarcinoma presented with bile peritonitis, in which endoscopic nasobiliary drainage (ENBD) as well as pleural and abdominal drainages improved the general condition leading to a radical operation. The case was a 79-year-old woman with the chief complaints of fever and right hypochondralgia due to pleural effusion and infected ascites contaminated with bile including E. coli. However gall-stones were not recognized in any biliary trees. US-guided drainage was performed into the pleural and abdominal cavities because of poor condition. After continuous drainages, per…

[Disorders of vital functions in acute peritonitis in children].

Changes in vital functions in endotoxicosis were appraised in 41 patients with acute peritonitis during stages of intensive therapy in a complex fashion. Correlative relations were revealed between the severity of endotoxicosis and the function of blood circulation, blood gases, and respiration. Endotoxicosis was characterized by increased level of middle mass molecules, circulating immune complexes, leukocytic index of intoxication, asparagate and alanine transferase. Hemodynamics functioned in hyperdynamic regimen in the reactive stage of peritonitis, in the normodynamic regimen in the trans…

Sonographic findings in tuberculous peritonitis of wet-ascitic type.

Sonograms in 46 patients with tuberculous peritonitis of the wet-ascitic type were analysed retrospectively. The ascites was clear in 24 patients (52%). There were fixed membranes, septa and debris in eight patients (17%), floating debris in six patients (13%), mobile strands or membranes in four patients (9%), and fixed septa in four patients (9%). The patients were divided into two groups depending on the amount of ascites: small amount of ascites with clear fluid in eight patients (17%), and moderate or large amount of ascites with clear or complex fluid in 38 patients (83%). Associated fin…

Acinetobacter peritonitis in patients on CAPD: characteristics and outcome.

Acinetobacter species are pleomorphic nonfermenting aerobic Gram negative bacilli which are important organisms causing peritonitis in patients on continuous ambulatory peritoneal dialysis (CAPD). We describe the characteristics and outcome of Acinetobacter peritonitis (AP) over an 18 month study period. There were 13 episodes of AP (9 identified as variant anitratus, 1 as variant calcoaceticus, 1 as variant, 1 woffi and 2 unspecified) in 11 patients (7 females and 4 males, mean age +/- SEM, 50.1 +/- 4.2 years), accounting for 14.3% of the total number of episodes of peritonitis. Technique fai…

[Immunologic effect of hemosorption in patients with acute peritonitis].

An examination of 65 patients with acute diffuse peritonitis has shown that pronounced T-immune deficiency is observed in the postoperative period of acute peritonitis. Hemosorption was shown to be efficient in patients with high content of toxin which allows considering the method of extracorporeal detoxication as effective means of immune-correcting therapy in peritonitis patients with high level of toxinemia who make up a group of risk.

Secondary bacterial peritonitis due to Listeria monocytogenes after paracentesis.

Although it is a relatively rare cause of peritonitis, Listeria monocytogenes must be considered in cirrhotic patients with ascites and a suggestive clinical presentation. We believe this is the first report of a case of peritonitis due to L monocytogenes in a patient without sepsis, and the sixth reported case of bacterial peritonitis in a patient with cirrhosis.

[New therapeutic aspects in the treatment of diffuse peritonitis].

The diffuse peritonitis is a syndrome during the course of which biochemical reactive chains are activated according to a cascade principle. Most of these biochemical reactive chains develop within an acid environment. The solutions applied for abdominal lavage so far had neutral pH value. The present acute peritonitis experiment has shown that a peritoneal pH increase during lavage therapy stopped the septic reactive chain and thus improved significantly the rate of survival in the test animals. These experimental findings ar now subject for clinical examination.

[Pneumococcal peritonitis in an adult. Report of a case].

A case report is presented of primary pneumococcal peritonitis, a disease rare in the adult. The etiology, differential diagnosis and therapy are discussed and the literature is reviewed. The authors believe that laparotomy should be performed in the case of acute abdomen with diffuse peritonitis, even if pneumococcal peritonitis is suspected.
