Complete Spontaneous Regression of the Intraspinal Synovial Cyst at the C1-C2 Junction Following with Atlantoaxial Fusion of Non-Union Odontoid Fracture: A Case Report.

The author reported an extremely rare case of intraspinal synovial cyst at C1-C2 junction, located posteriorly to the odontoid process, compressing the upper cervical spinal cord. The patient had a history of spinal trauma 40 years ago without prior definitive treatment, presenting with two months of left leg and arm numbness with neck pain. Imaging studies including plain radiographs, computed tomography (CT), and magnetic resonance imaging (MRI) of the cervical spine showed a large synovial cyst associated with atlantoaxial instability from non-union fracture of the odontoid process. The atl

Human Bite-induced Cellulitis Due to Streptococcus pyogenes.

The rate of infection after a human bite is 10%-30%. Streptococcus spp. are the most common pathogens that may cause serious infections, such as necrotizing fasciitis. The authors describe a 5-year-old girl admitted to the emergency room with cellulitis due to S. pyogenes after human bites. She presented with fever and pain in her left arm, and her medical history revealed a human bite by her cousin on the arm 3 days prior. The wound culture yielded S. pyogenes. She was successfully treated with intravenous ampicillin-sulbactam regimen. S. pyogenes may cause serious wound infections following

Ureteroarterial fistula.

The authors intended to perform a comprehensive review of the literature pertaining to ureteroarterial fistulae and apply the findings to a case. A comprehensive literature search was performed using the Keywords: ureter, artery and fistula. The available articles printed in or translated to English were analysed for overall trends. The results were then compared with the case of a patient (index patient). Review of the literature reveals that 57% of all ureteroarterial fistulae form in women at an average age of 58. The most common presenting complaint is haematuria. There appears to be a shi

Case series: the paradox of epilepsy surgery and psychiatric disorder.

De novo psychiatric disorder following epilepsy surgery is an infrequent but very interesting phenomenon. The authors described 4 distinct cases with medically intractable epilepsy who had epilepsy surgery and developed postsurgical psychiatric disorder. The onset of psychiatric disorder was during dramatic improvement of their epilepsy after surgery. There was no history of psychiatric disorder in their familial members or in the patients prior to the surgery. Since three patients also had mental retardation, presurgical cognitive impairment may be one of the risk factors for developing posts

Postural and locomotor contributions to affordance perception.

The authors sought to evaluate the relative importance of locomotor control and postural control in the perception of affordances. While seated in a stationary wheelchair, participants made a series of judgments about the minimum lintel height under which they could roll in the wheelchair. Prior to making judgments, participants were given brief (∼2 min) experience with wheelchair locomotion. They expected that this practice would influence the accuracy of subsequent affordance judgments. During practice, participants moved under their own power (using their hands on the wheels) or with an exp

Pediatric laryngeal electromyography.

Vocal fold immobility (VFI) is a challenging management issue in pediatric otolaryngology. VFI is most commonly distinguished between unilateral (UVFI) and bilateral (BVFI) dysfunction. UVFI and BVFI are different pathophysiologic and clinical entities with distinct symptoms and etiologies. It has been generally accepted in the adult literature to wait at least 1 year prior to carrying out more permanent type procedures for VFI. This period has been extended out even further in children as the literature has suggested that vocal fold function may return many years later. Unfortunately, there i

Interface fungal keratitis after endothelial keratoplasty: a clinicopathological report.

The authors report two cases of interface fungal keratitis following Descemet's stripping automated endothelial keratoplasty (DSAEK). Two patients developed culture-proven interface keratitis with Candida albicans and Candida glabrata following DSAEK. Both patients presented with white interface opacities at approximately 1 month after uncomplicated DSAEK. Adjunct confocal microscopy identified fungal elements prior to surgical therapy. A penetrating keratoplasty was performed in both cases after failed medical therapy with fungal elements confirmed by corneal histopathology. Interface fungal

TV tip-over morbidity and mortality in children.

Twenty-eight non-fatal cases of TV related injuries were noted in San Diego in the 2 years prior to September 2008. We reviewed the scene, witness reports, past history, and autopsy findings from three fatal cases and distinguished them from abusive head trauma. The recent literature was also reviewed. Our fatal cases resulted from TVs falling on small children and causing severe head injury. The literature review showed increasing injuries and deaths relating to TV tip-over, TV stands and dressers. Most of the fatalities were head injuries in small children. The reporting methods may not be c

The evolution of the treatment of traumatic cerebrovascular injury during wartime.

The approach to traumatic craniocervical vascular injury has evolved significantly in recent years. Conflicts prior to Operations Iraqi and Enduring Freedom were characterized by minimal intervention in the setting of severe penetrating head injury, in large part due to limited far-forward resource availability. Consequently, sequelae of penetrating head injury like traumatic aneurysm formation remained poorly characterized with a paucity of pathophysiological descriptions. The current conflicts have seen dramatic improvements with respect to the management of severe penetrating and closed hea

Are all ventricular septal defects created equal?

The authors report the occurrence of infective endocarditis in a 32-year-old man with a ventricular septal defect and a left ventricular-to-right-atrial shunt who adhered to the revised 2007 American Heart Association guidelines for infective endocarditis. The patient had received antibiotic prophylaxis prior to multiple previous dental procedures. At a recent dental evaluation for fillings, he was informed that he no longer needed prophylaxis. Fatigue and fevers developed 1 week later, and he was treated with an oral course of ciprofloxacin. The symptoms recurred, and blood cultures grew Stre

Glioblastoma multiforme with an unusual location and clinical course.

We present a unique case of a brain tumor patient with atypical location and progression. He was initially presented with mood and anxiety type symptoms together with aphasia and left-sided paraesthesias. Magnetic resonance imaging and CSF were negative and the patient was diagnosed with PTSD as he recently experienced a small motor vehicle accident. Two months after the first presentation, MRI revealed multifocal juxtacortical, leptomeningeal hyperdensities in the bilateral frontal lobes. MRI-guided frameless stereotactic biopsy defined a diagnosis of GBM 1 week prior to death which occurred

Anatomic hemispherectomy for intractable epilepsy in a patient with unilateral schizencephaly.

Schizencephaly is a rare congenital cortical brain malformation defined by unilateral or bilateral clefts of the cerebral hemispheres. These malformations are often associated with medically intractable epilepsy. Surgical solutions include lesionectomy, lobectomy, or hemispherectomy. The authors describe the case of an anatomic hemispherectomy for medically intractable epilepsy in an 8-year-old boy with a large schizencephalic cleft. Seven years prior to his epilepsy surgery, the patient underwent placement of a ventriculoperitoneal shunt for communicating hydrocephalus that resulted in severe

Radiofrequency-induced thermotherapy of nasopharyngeal angiofibroma and immunohistochemical analysis of vessel proliferation: a case report.

INTRODUCTION: Nasopharyngeal angiofibroma presents with symptoms of nasal obstruction and epistaxis. The treatment of choice is embolization followed by surgery. CASE PRESENTATION: A 52-year-old man underwent surgery for nasopharyngeal angiofibroma after adjuvant radiofrequency-induced thermotherapy. To the best of the authors' knowledge, this is the first case of angiofibroma with clinical follow-up after thermocoagulation therapy supported by quantitative, double immunohistochemistry. We found this case of angiofibroma to be of interest owing to the presentation of symptoms leading to biops

Anomalous venous drainage preventing safe posterior fossa decompression in patients with chiari malformation type I and multisutural craniosynostosis. Report of two cases and review of the literature.

The authors report on two children in whom an anomalous posterior fossa venous drainage pattern prevented safe posterior fossa decompression. Both patients had Chiari malformation Type I, multisutural craniosynostosis, and crowded posterior fossa structures. Both patients had been treated with ventriculoperitoneal shunts for hydrocephalus. Pfeiffer syndrome had been diagnosed in one of the patients, and the other was suspected to have osteogenesis imperfecta. Although both patients were believed to have symptoms resulting from brainstem compression, posterior fossa decompression was not offere

Detection of important venous collaterals by computed tomography venogram in multisutural synostosis. Case report and review of the literature.

The authors describe the novel use of computed tomography (CT) venography in the preoperative evaluation of a child with Crouzon syndrome who was being considered for Chiari decompression. This 18-month-old girl presented with hydrocephalus (treated with a ventriculoperitoneal shunt) and persistent symptomatic Chiari malformation and associated syrinx. A CT venogram was obtained because of the well-described relationship between multisutural craniosynostosis and abnormal intracranial-to-extracranial venous drainage. The CT venogram showed widely dilated vertebral and paravertebral veins locate

Should addictive disorders include non-substance-related conditions?

AIMS: In anticipation of the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-V), to consider whether addictive disorders should include non-substance use disorders. METHODS: The author reviewed data and provided perspective to explore whether disorders such as pathological gambling (PG) should be grouped together with substance dependence, given that they share many features. RESULTS: PG and substance dependence currently reside in the DSM, fourth edition, text revision (DSM-IV-TR) within separate categories, with PG classified as an impulse control disorder (

Polyarteritis nodosa and cochlear implantation.

Polyarteritis nodosa is a systemic disease which affects the small to medium-sized muscular arteries. Sudden or progressive, bilateral hearing loss is a presenting otologic manifestation. To date, no case of cochlear implantation in patients with polyarteritis nodosa has been reported. The authors present a case of polyarteritis nodosa (confirmed by biopsy) in a 71-year-old man with progressive, bilateral sensorineural hearing loss who underwent cochlear implantation. A successful full insertion of the Nucleus 3G electrode array was achieved without surgical or post-operative complications. Th

[Imipenem-cilastatin use in a university hospital in Chile].

Imipenem is an expensive broad-spectrum antimicrobial, reserved for infections caused by multi-resistant nosocomial pathogens. Since 2001 our university hospital applies a restriction policy that allows rejecting or authorizing its use after a supervising evaluation with pre-specified criteria for appropriate or inappropriate use. An audit was performed for all the supervisions made during the periods of March-April and September-October, 2004, totalling 136 treatments. In global terms, 58.1% of treatments were considered appropriate and 11.8% inappropriate; other 20.6% had been discontinued b

Bilateral peritonsillar abscesses.

In the discussion about the management of the peritonsillar abscess (PTA) in regard to the pros and cons of tonsillectomy à chaud versus à froid, the risk of obscure contralateral abscesses is often neglected. To the authors' knowledge, there are only a few series of PTA being analyzed for the abscess rate of bilateral PTA. A group of 541 abscess tonsillectomies was retrospectively analyzed for the presence of a bilateral manifestation of peritonsillar abscess. Twenty-one patients (3.88%) had bilateral abscesses. None of these had been detected prior to the operation. Of the 541 patients, 2.22

[A model for training in laryngeal exposure].

Laryngeal exposure, applying forward and upward force by laryngoscope after placement of the blade head at the vallecula, is most difficult for begginers to learn in tracheal intubation. It is not so easy to practice at home as the laryngoscope is shaped to lift the tongue upward only. Beginners must establish a proper image of moving the left hand for laryngeal exposure and must be trained in this technique prior to tracheal intubation of patients. Not only begginers but also veterans may struggle with intubating mannequins. The author made a simple model for training laryngeal exposure by Ma
