Uterus-preserving management of a high-flow uterine artery pseudoaneurysm with arteriovenous fistula following cesarean section: a case report.

Postpartum uterine vascular lesions are a rare but important cause of secondary postpartum hemorrhage. This report describes the case of a 29-year-old woman presenting five weeks after repeat cesarean section with recurrent vaginal bleeding. Transvaginal color Doppler ultrasound demonstrated a 2.7 cm anechoic lesion adjacent to the cesarean scar with marked hypervascularization and a high-flow pattern (peak systolic velocity up to 310 cm/s, low resistance indices) with pulsatile venous outflow, suggestive of a combined uterine artery pseudoaneurysm and arteriovenous fistula. Computed tomograph

Hemodynamics regulate spatiotemporal artery muscularization in the developing circle of Willis.

Vascular smooth muscle cells (VSMCs) envelop vertebrate brain arteries, playing a crucial role in regulating cerebral blood flow and neurovascular coupling. The dedifferentiation of VSMCs is implicated in cerebrovascular diseases and neurodegeneration. Despite its importance, the process of VSMC differentiation on brain arteries during development remains inadequately characterized. Understanding this process could aid in reprogramming and regenerating differentiated VSMCs in cerebrovascular diseases. In this study, we investigated VSMC differentiation on the zebrafish circle of Willis (CoW),

Computational study on hemodynamic changes in patient-specific proximal neck angulation of abdominal aortic aneurysm with time-varying velocity.

Aneurysms are considered as a critical cardiovascular disease worldwide when they rupture. The clinical understanding of geometrical impact on the flow behaviour and biomechanics of abdominal aortic aneurysm (AAA) is progressively developing. Proximal neck angulations of AAAs are believed to influence the hemodynamic changes and wall shear stress (WSS) within AAAs. Our aim was to perform pulsatile simulations using computational fluid dynamics (CFD) for patient-specific geometry to investigate the influence of severe angular (≥ 60°) neck on AAA's hemodynamic and wall shear stress. The patient'

Hydraulic In Vitro Characterization of MR-Conditional Blood Pumps.

BACKGROUND: Many cardiac surgical procedures use cardiopulmonary bypass (CPB), which in neonates requires the heart-lung machine (HLM) to be … exhibited pressure-dependent performance (slope -2.21 ± 0.45 mmHg/[L/min]). All pumps generated non-pulsatile flow (SHE < 1000 erg/cm3). CONCLUSION: All three pumps meet basic MR-conditionality and flow requirements, supporting their potential for use in MRI-guided studies during neonatal cardiac surgery.

Central Hemodynamics for Management of Arteriosclerotic Diseases.

Arteriosclerosis, particularly aortosclerosis, is the most critical risk factor associated with cardiovascular, cerebrovascular, and renal diseases. The pulsatile hemodynamics in the central aorta consists of blood pressure, flow, and stiffness and substantially differs from the peripheral hemodynamics in muscular arteries. Arteriosclerotic changes with age appear earlier in the elastic aorta, and age-dependent increases in central pulse pressure are more marked than those apparent from brachial pressure measurement. Central pressure can be affected by lifestyle habits, metabolic disorders, an

Less frequent opening of the aortic valve and a continuous flow pump are risk factors for postoperative onset of aortic insufficiency in patients with a left ventricular assist device.

BACKGROUND: Postoperative development of aortic insufficiency (AI) after implantation of left ventricular assist devices (LVADs) has recently been recognized, but the devices in the previous reports have been limited to the HeartMate I or II. The purposes of this study were to determine whether AI develops with other types of LVADs and to elucidate the factors associated with the development of AI. METHODS AND RESULTS: Thirty-seven patients receiving LVADs without evident abnormalities in native aortic valves were enrolled (pulsatile flow LVAD [TOYOBO]: 76%, continuous flow LVAD [EVAHEART, Du…

The use of colour-coded and spectral Doppler ultrasound in the differentiation of benign and malignant breast lesions.

The aim of this study was to evaluate the role of colour-coded and spectral Doppler sonography to predict the benign or malignant nature of breast lesions. A total of 112 women with mammographically suspicious breast lesions were investigated prior to surgery. Thirty-nine breast carcinomas and 73 benign lesions were evaluated for the resistance index, pulsatility index and the flow velocity. A resistance index of > or = 0.70 was characteristic of malignant tumours with a sensitivity of 82% and a specificity of 81%. The positive predictive value was 70% and the negative predictive value 89%. Do

Transcranial colour coded sonography as an aid to measurement of blood flow velocity in the basal cerebral arteries.

Unlike conventional transcranial Doppler, transcranial colour coded sonography allows imaging of the basal cerebral arteries. We used this technique to determine blood flow velocities, with compensation for the angle of insonation, in the anterior and posterior cerebral circulation of 60 volunteers categorised by age (20-39, 40-59, > 60 years). Successful vessel insonation declined with increasing age: 40/40 middle cerebral arteries insonated 20-39 y, 31/40 insonated > 60 y. Peak, mean and end diastolic blood flow velocities also decreased with age in all arteries: 20-39 y middle cerebral arte

[Left ventricular diastolic function: value of its study with mitral and pulmonary venous Doppler phonocardiography and apexography].

The study of mitral flow and pulmonary venous flow by Doppler provides a non-invasive method of assessing diastolic function. But there are difficulties in differentiating normal pattern from "pseudo-normal" (Appleton Type II) (E/A Mitral > 1). Phonocardiography enables the recording of early (B3) and late (B4) diastolic gallops. The apexogramme enables calculation of the a/H and D/H ratios (amplitude of the "D" wave resulting from rapid early diastolic filling to the total amplitude of the apexogramme). These two techniques record pulsatile phenomena ("pulse waves") arising from variations in

Effect of blood pressure on vascular hemodynamics in acute tachycardia.

Paroxysmal supraventricular tachycardia is accompanied by hypotension, which can affect vascular hemodynamics. Here, we hypothesized that a fall in blood flow as a result of hypotension has a larger effect on hemodynamics in medium-sized peripheral arteries compared with increased pulsatility in rapid pacing. To test this hypothesis, we experimentally and theoretically investigated hemodynamic changes in femoral, carotid, and subclavian arteries at heart rates of 95-170 beats/min after acute pacing. The arterial pressure, blood flow, and other hemodynamic parameters remained statistically unch

Quantification of near-wall hemodynamic risk factors in large-scale cerebral arterial trees.

Detailed hemodynamic analysis of blood flow in pathological segments close to aneurysm and stenosis has provided physicians with invaluable information about the local flow patterns leading to vascular disease. However, these diseases have both local and global effects on the circulation of the blood within the cerebral tree. The aim of this paper is to demonstrate the importance of extending subject-specific hemodynamic simulations to the entire cerebral arterial tree with hundreds of bifurcations and vessels, as well as evaluate hemodynamic risk factors and waveform shape characteristics thr

Quantitative measurement of normal and hydrocephalic cerebrospinal fluid flow using phase contrast cine MR imaging.

Measurements of the cerebrospinal fluid (CSF) flow using phase contrast cine magnetic resonance (MR) imaging were performed on a phantom, 12 … e cardiac cycle in all subjects. However, the patients with NPH showed variable patterns of the CSF pulsatile flow and these patterns could be divided into four types according to velocity and amplitude. The amplitudes of each type were as follows: type 0 (n = 1), 87.6mm; type I (n = 2), 58.2mm (mean); type II (n = 6), 48.0 +/- 5.0mm (mean +/- SEM); and type III …

An in vitro study of the onset of turbulence in the sinus of Valsalva.

During systole a small portion of the mainstream aortic flow is intercepted by the sinus ridge, or downstream corner of the sinus of Valsalva. This fluid curls back toward the ventricle to form a large eddy, or vortex, that spins within the sinus cavity. The fluid motion within similar recirculating flows is known to be unstable with an early transition to turbulence. The stability of the aortic sinus vortex was examined in the current study in an in vitro pulsatile flow rig. The geometry of the experimental test section was the same as the geometry of the natural aortic root. Different model …
