Corneal ectasia in ectodermal dysplasia: Novel Tumour Protein 63-related phenotype in Rapp-Hodgkin syndrome.

A man in his 20s with genetically confirmed Rapp-Hodgkin syndrome experienced a progressive decline in visual acuity in the left eye over 12 months. Corneal assessment revealed bilateral keratoconus: the left eye showed central thinning (Tmin 408 µm) and steepening (Kmax 55.98 D), while the right eye had forme fruste keratoconus (Tmin 465 µm, Kmax 45.6 D). Slit-lamp biomicroscopy demonstrated Vogt's striae, an incomplete Fleischer's ring and prominent corneal nerves. Systemic examination identified hypodontia, nail dystrophy, a fissured tongue and characteristic facial dysmorphisms. Whole-exom

Phototherapeutic keratectomy for calcific band keratopathy secondary to multiple myeloma.

To report a case of calcific band keratopathy (CBK) secondary to multiple myeloma (MM) successfully treated with phototherapeutic keratectomy (PTK). An 80-year-old woman presented with progressive vision loss in her left eye with a best corrected visual acuity (BCVA) of 4/10 in the left eye (OS). Slit-lamp examination revealed a central band-like corneal opacification indicative of CBK. Anterior segment optical coherence tomography (AS-OCT) showed a subepithelial hyper reflective layer. Further evaluation confirmed an indolent IgAk MM, with no indication for systemic therapy. PTK was performed

Embedded Episcleral Foreign Body Mimicking Nodular Anterior Scleritis.

A 56-year-old man was referred to our clinic for unilateral nodular scleritis unresponsive to systemic corticosteroids. A localized, nodular hyperemia on the nasal bulbar conjunctiva surrounding a central cyst-like lesion together with vascular engorgement was observed on slit-lamp examination of the left eye. No abnormal fundoscopic findings were noted. Surgical exploration revealed an embedded episcleral brown colored, soft to touch, splinter-like organic foreign body (FB) which was confirmed by the histopathological examination. Nodular hyperemia resolved during the postoperative follow-up

Multimodal imaging of torpedo maculopathy with optical coherence tomography angiography.

A 24-year-old woman presented for routine clinical evaluation. Her best-corrected visual acuity was 20/20 and slit-lamp examination of the anterior segment was normal in both eyes. Fundus examination of the left eye revealed a hypopigmented lesion, one disc diameter temporal to the fovea, with a hyperpigmented tail extending temporally from the main lesion, consistent with a torpedo maculopathy. Fundus autofluorescence showed an hypoautofluorescence with hyperautofluorescent border. Swept-source optical coherence tomography (OCT) showed a normal inner retina and a degeneration of the outer ret

Challenges in the diagnosis and management of simultaneous, bilateral, toxic anterior segment syndrome after phacorefractive surgery.

A 59-year-old woman presented with a 24-hour history of severe, bilateral, painless vision loss starting 1 day after immediately sequential, bilateral, phacorefractive surgery with multifocal intraocular lens (IOL) implantation at another institution. Best-corrected visual acuity was counting fingers at 1 foot in the right eye and 20/100 in the left eye. Slit-lamp evaluation showed a dense fibrin membrane on the anterior surface of the IOL and significant anterior chamber inflammation in both eyes. B-scan ultrasound revealed bilateral vitreous haze, without membrane formation. The rapid onset,

An Unusual Presentation of Corneal Intraepithelial Neoplasia: A Case Report.

INTRODUCTION: Corneal squamous neoplasia is less common than that of conjunctiva and can cause diagnostic confusion. CASE: A 40-year-old male presented with gradual onset blurring of vision in left eye for 8 weeks. He had received treatment for dry eyes, then for herpetic dendritic keratitis, but without improvement. On slit-lamp examination with diffuse light, apparently the cornea looked clear with some dilated conjunctival vessels nasally. But in the retro-illumination, on the corneal surface, there was a translucent inverted "V" shaped lesion with irregular fimbricated margin. He underwen

Voriconazole-impregnated multilayered AMT and intracorneal plus intracameral injection in recalcitrant fungal corneal ulcer.

We describe the management of a referred man in his 50s with corneal ulcer in the left eye. Slit-lamp biomicroscopy revealed an infiltrate (4.5×3.5 mm), involving full corneal thickness, corneal melt less than one-third corneal thickness and hypopyon (3.0 mm). Branched septate hypha on direct microscopy and Aspergillus flavus in Sabaraud's dextrose agar confirmed the clinical diagnosis of fungal corneal ulcer. We used natamycin 5% suspension 1 hourly, voriconazole 1% 1 hourly, gatifloxacin 0.5% three times and atropine 1% three times per day, for 2 weeks. However, there was no improvement and

Canine adipose tissue-derived mesenchymal stem cell therapy in a dog with renal Fanconi syndrome.

A 39-year-old woman presented to our hospital with a history of photorefractive keratectomy (PRK), performed two weeks prior; slit-lamp examination revealed diffuse conjunctival congestion, corneal ulcer and stromal infiltration. After 5 days of antifungal and antibacteric treatment, the infiltrate progressively increased so that a therapeutic penetrating keratoplasty was necessary. The microbiological analyses revealed the presence of fungal filaments. Twenty days after surgery the patient had recurrent fungal infiltrate in the donor cornea with wound dehiscence. We performed a second penetra
