Recurrent periventricular hemorrhage in cerebral proliferative angiopathy: Case report.

Cerebral proliferative angiopathy (CPA) is a rare vascular lesion. Bleeding from CPA is uncommon, but the risk of rebleeding is high once it bleeds. We describe a case of CPA with multiple intra- and periventricular hemorrhages during 30-year follow-up. Recurrent bleeding in these areas are common in moyamoya disease. These lesions may share the cause of bleeding: proliferation of the periventricular vessels functioning as collateral pathways. Revascularization surgery for CPA may attenuate the vascular proliferation in the vicinity of the ventricle, which may prevent rebleeding.

Carotid Endarterectomy in Moyamoya Disease: A Case Report.

We report the case of a 55-year-old female who presented with symptomatic high-grade stenosis of the right internal carotid artery, years after her diagnosis of moyamoya disease (MMD), which had been treated with extensive carotid and cerebral vascular reconstructions. A percutaneous carotid revascularization was unsuccessful, and a carotid endarterectomy with bovine patch angioplasty was subsequently performed. The presence of symptomatic atherosclerotic carotid disease in combination with MMD, with all the perioperative challenges, has not been previously described.

Facial steal syndrome: identification and endovascular management.

We describe a case of intraprandial vertebro-basilar insufficiency secondary to posterior cerebral circulation ischemia. Secondary collateralization was seen from facial and external carotid artery branches to the posterior circulation causing facial arterial steal during mastication. This condition was treated endovascularly. Occluded common and external carotid arteries were revascularized via angioplasty and stenting. The patient was able to undergo coronary artery bypass and right hip replacement in the following two months without cerebrovascular complications.

Ipsilateral subclavian to external carotid and STA-MCA bypasses for retinal ischemia.

The surgical treatment of a patient with retinal ischemia in the left eye and occlusion of the left common and internal carotid and right internal carotid arteries is presented. After demonstration of filling of the left external carotid artery from vertebral collaterals, as saphenous vein graft from the left subclavian to the left external carotid artery was performed prior to a left superficial temporal artery to middle cerebral artery bypass. This case demonstrates the feasibility of revascularization in the presence of occlusion of the ipsilateral common and internal carotid arteries.

Long-Term Clinical and Angiographic Outcomes of Internal Thoracic Artery Harvesting Technique.

INTRODUCTION: Recent studies have suggested that skeletonization of the internal thoracic artery (ITA) may be associated with worse angiographic and clinical outcomes after coronary artery bypass grafting. We investigated the impact of ITA skeletonization on angiographic results and long-term clinical outcomes after coronary artery bypass grafting. METHODS: Between January 2010 and June 2022, 2909 patients met inclusion criteria and were grouped by ITA harvest technique: skeletonized (n = 1440) or pedicled (n = 1469). Propensity score matching yielded 961 pairs. Long-term outcomes included al

Absence of the Anterior Communicating Artery on Selective MRA is Associated with New Ischemic Lesions on MRI after Carotid Revascularization.

BACKGROUND AND PURPOSE: ICA-selective MRA using a pencil beam presaturation pulse can accurately visualize anterior communicating artery flow. We evaluated the impact of anterior communicating artery flow on the perioperative hemodynamic status and new ischemic lesions after carotid revascularization. MATERIALS AND METHODS: Eighty-three patients with carotid artery stenosis were included. We assessed anterior communicating artery flow using ICA-selective MRA. The preoperative hemodynamic status was measured using SPECT. We also measured the change in regional cerebral oxygen saturation after

The Occipital Artery as an Alternative Donor for Low-Flow Bypass to Anterior Circulation After Internal Carotid Artery Occlusion Failure prior to Exenteration for an Atypical Cavernous Sinus Meningioma.

BACKGROUND: In skull base tumors involving the cavernous sinus, indications for aggressive resection are sparse and must be carefully examined because of their invasiveness. With careful evaluation, techniques including internal carotid artery sacrifice with or without extracranial-intracranial bypass may still be an option in some cases. Moreover, previous surgery with the sacrifice of potential donor vessels requires adjusting the revascularization strategy. We describe an occipital artery-middle cerebral artery bypass before skull base tumor resection. CASE DESCRIPTION: A 47-year-old woman

Results of surgical management for posterior cerebral artery aneurysms: 7-year experience in the endovascular era.

PURPOSE: Unlike most posterior circulation aneurysms, posterior cerebral artery (PCA) aneurysms can be surgically approached, we believe, without significant technical difficulty or procedural morbidity. We report our recent experience to assess procedural complications and clinical outcomes for PCA aneurysms. METHODS: In our retrospective review of 15 consecutive patients with PCA aneurysms, six patients underwent surgical treatment, six patients had endovascular coiling, (three patients from the endovascular group also underwent surgical revascularization), and three patients were treated c

Predicting Cerebral Hyperperfusion Syndrome Following Superficial Temporal Artery to Middle Cerebral Artery Bypass based on Intraoperative Perfusion-Weighted Magnetic Resonance Imaging.

Moyamoya disease leads to the formation of stenosis in the cerebrovasculature. A superficial temporal artery to middle cerebral artery (STA-MCA) bypass is an effective treatment for the disease, yet it is usually associated with postoperative cerebral hyperperfusion syndrome (CHS). This study aimed to evaluate cerebral hemodynamic changes immediately after surgery and assess whether a semiquantitative analysis of an intraoperative magnetic resonance perfusion-weighted image (PWI) is useful for predicting postoperative CHS. Fourteen patients who underwent the STA-MCA bypass surgery were include

STA-MCA bypass following sphenoid wing meningioma resection: A case report.

INTRODUCTION: Sphenoid meningioma engulfed cerebral arteries has always been a challenge. To achieve a gross total resection, vessel sacrifice may be unavoidable. PRESENTATION OF CASE: A 22-year-old man with a history of head trauma a week ago complained of a headache for one week. On examination, he was alert, denied paralysis and cranial nerves palsies. Preoperative MRI showed a hypervascular left sphenoid wing meningioma embedding left internal carotid artery and proximal segment of the middle cerebral artery. In operation, a branch of the MCA was divided when dissecting the tumor. The MCA

Active push deployment technique improves stent/vessel-wall interaction in endovascular treatment of acute stroke with stent retrievers.

BACKGROUND: The optimal interaction between stent struts and thrombus is crucial for successful revascularization in endovascular stroke therapy with stent retrievers. Deploying the stent retriever by actively pushing it into the thrombus increases the radial force with which the stent struts expand into the thrombus. OBJECTIVE: To examine the active push deployment (APD) technique in an in vitro model and present our clinical experience with this technique. METHODS: In an in vitro experiment we investigated the configuration of a Solitaire and a Trevo ProVue device (both 4×20 mm), depending

The tandem bypass: subclavian artery-to-middle cerebral artery bypass with dacron and saphenous vein grafts. Technical case report.

BACKGROUND: Fusiform or dolichoectatic intracranial aneurysms often cannot be managed with conventional surgical or endovascular techniques, and instead require trapping and revascularization techniques. On rare occasions in elderly patients, extracranial sites used for anastomosing the bypass have been previously repaired with synthetic vascular prostheses. This circumstance in an elderly subarachnoid hemorrhage patient led to a novel bypass procedure, the tandem bypass: a long extracranial-to-intracranial bypass with two grafts of different materials assembled in series. Surgical treatment for pediatric moyamoya disease: use of the superficial temporal artery for both areas supplied by the anterior and middle cerebral arteries.

OBJECTIVE: To revascularize ischemic territories of both the anterior cerebral artery (ACA) and the middle cerebral artery (MCA), a simple and effective combined bypass operation was performed in 36 pediatric patients with moyamoya disease during the past 8 years. METHODS: The branches of the superficial temporal artery (STA) were used to revascularize the ACA and MCA territories. In children older than 5 years, the parietal branch of the STA was usually used for an end to side anastomosis with a cortical branch of the MCA. In children who were younger than 5 years, the parietal branch of the

[Successful treatment of moya-moya disease using encephalo-duro-arteriosynangiosis].

Aim of the study was to demonstrate the possibility of effective surgical revascularization of the brain in children with moya-moya disease. 3 year old girl with moya-moya disease underwent encephalo-duro-arteriosynangiosis. At 14 months follow-up signs and symptoms of cerebral ischemia totally resolved. Angiography confirmed brain supply from external carotid artery via the newly formed anastomotic network. Authors emphasize the necessity to seek moyamoya disease in children with symptoms of brain ischemia to allow timely and effective surgical treatment.

Treatment of a cerebral dissecting aneurysm in anterior circulation: report of 11 subarachnoid hemorrhage cases.

This report presents 8 cases of internal carotid artery aneurysms, 1 case of a middle cerebral artery aneurysm, and 2 cases of anterior cerebral artery aneurysms, together with a discussion of the treatment of aneurysms in anterior circulation. All cases showed subarachnoid hemorrhage. Two of the 8 internal carotid artery aneurysms were trapped with a low-flow bypass; however, both patients died of an immediate hemodynamic infarction or vasospasm-induced infarction. Five of the 8 internal carotid artery aneurysms were trapped after revascularization with high flow bypass. Four of those patient

Kissing balloon treatment for stenosis of the internal and external carotid artery orifice associated with moyamoya disease: illustrative case.

BACKGROUND: Moyamoya disease (MMD) is a cerebrovascular disorder that is characterized by progressive stenosis in the terminal portions of the internal carotid artery (ICA). Although a substantial number of patients with MMD exhibit stenosis of the ICA orifice (ICAO), it infrequently occurs in patients with MMD with external carotid artery orifice (ECAO) stenosis. OBSERVATIONS: A 48-year-old female with no past medical history presented to the authors' hospital with right hemiparesis and motor aphasia. Head magnetic resonance imaging revealed bilateral acute cerebral infarctions in the left f

Time course of angiogenesis in solid pineal autografts.

Angiogenesis and reperfusion of blood vessels were analysed qualitatively, at the light- and electron-microscopical levels, in solid pineal autografts placed intracerebrally in adult rats (post-transplantation survival times: 1, 3, 7, 10, 14 and 28 days). Reperfusion of blood vessels was studied in sections from immersion-fixed brains incubated to demonstrate the endogenous peroxidase activity of erythrocytes within the lumen of blood vessels. The possible presence of the blood-brain barrier (BBB) within the grafts was also investigated by injecting native horseradish peroxidase (HRP) intraven
